4 Supporting Tender Information: Contractual Contract Organisation The Tenderer shall submit a description of the organisation proposed for the execution of the Works. The description shall include an organisation chart with the numbers and titles of key personnel and the numbers and categories of office and site personnel, including their competencies, authorities, and responsibilities and reporting lines. The organisation shall include the disciplines of Safety, Quality and Environment.

5 Supporting Tender Information: Contractual Key Personnel and CVs The Tenderer shall attach here a separate CV for each of the following key personnel which it proposes to utilise in the performance of this contract: 1. Insert Job Title - See example list below Each CV shall provide at least the following: Position in the proposed contract team: (title) Name: Current home location: (Town only) Education/Qualifications: Speak/read/write English language: (yes/no to each) Present position in tenderer s company: If not employed by tenderer, current employer: Employment history for last 10 years: (role, employer (including dates), duties, responsibilities, project details, general and specific experience, etc.) Allocation to project:(if not full time - number of hours/days per week) All personnel involved in Safety Critical Work (as designated by the Employer s Representative) shall continue their involvement with the contract in accordance with the following: The Tenderer confirms, should its tender be accepted: 1. that it has the resources available with the necessary skills, competencies, knowledge and experience to execute the Works and or Services within the time period specified in the Invitation Documents and provision of such resources will not prejudice the timely performance of any other existing contract within Network Rail at the date of this Contract; 2. the committed resources will remain assigned to this Contract until released by the Employer s Representative or on completion of the contract which ever the sooner; 3. it will assign the key personnel listed above as agreed by the Employer s Representative; 4. that in the event of key personnel becoming unavailable (due to ill health/resignation of employment or the like) the Contractor agrees to provide a replacement of equal standard and status within 48 hours of the original key personnel becoming ill or giving notice of resignation. The Tenderer shall indicate hereunder the names of the individuals whose CV s are attached hereto.

7 Competency and Expertise Please list the key staff members and any other organisations you will use to provide the professional expertise required for this project. Please provide the following information for each professional discipline (max. 2 sides of A4 in total): Organisation name Name of lead staff member Their position and qualifications Up to six bullet points summarising their relevant experience For each organisation that will form part of the team led by the lead consultant, please summarise the relationship between the organisations (e.g. number of projects worked on together, lines of reporting, duration of joint working, responsibilities held by the respective organisations)

8 Design Quality of Recent Projects Please enclose images and summary information on up to three of your past projects. This should include streetscape / town centre schemes, with plans of the existing site and the final design, plus supporting graphic information as appropriate. Text should summarise the client, brief, budget, scope of works and other key information that influenced your organisation s role and the design outcome.

9 Proposed Work Stages and Responsibilities In line with the overall programme, please set out the intended work plan for the Project in the form of a table. Please populate the table by listing the following: The key activities to be undertaken by the applicant to achieve each milestone e.g. devise consultation plan, secure approval of consultation plan from the Council, organise public event, produce consultation boards The deliverables e.g. drawings, designs, surveys, meetings, etc. The main contributor(s) from the design team for those activities. We expect to have a single main point of contact in the successful applicant s team but would like to know who would contribute to each activity.

10 Resource Declaration The Tenderer confirms, should its tender be accepted:; 4. that it has the resources available with the necessary skills, competencies, knowledge and experience to execute the Works/Services within the time period specified in the Invitation to Tender and provision of such resources will not prejudice the timely performance of any other contract existing at the date proposed for award of this Contract, between the Tenderer and the Employer; 5. the committed resources assigned to this Contract at acceptance of tender will remain assigned to this Contract until released by the Employer s Representative; 6. it will assign the key personnel defined in Supporting Tender Information - Contractual, Key Personnel & CV s, as approved by the Employer s Representative, at award of contract; 7. that in the event of key personnel becoming unavailable (due to ill health/resignation of employment or the like) the Tenderer agrees to provide a replacement of equal standard and status within 48 hours of the original key personnel becoming ill or giving notice of resignation. 11 Authorised Signatories Satisfactory evidence of the authority of any person signing the Tender on the Tenderer s behalf is required to be submitted below. 13 Outline Method Statement The Tenderer shall submit outline statements of the methods by which it is intended to execute the Works from time of award of Contract until completion. The Tenderer shall therefore ensure that the Outline Method Statement provided; 1. is a clear statement of the Tenderer s intended methods of working; 2. utilises the defined Work Breakdown Structure; 3. provides details of any contingency arrangements; 4. takes full account of train operations and possession arrangements; 5. gives full details of any preliminary/advance and temporary work requirements.

Title: Puddle Dock Refurbishment Mechanical and Electrical Services Date: May 2013 Insurance Requirements The Tenderer s obligation to insure in the event of a Contract award is as specified in the Conditions of Contract and as further defined in the Appendix to Terms and Conditions. Should the Tenderer not carry, in the normal course of its business, insurance cover at the specified level, an excess liability facility will be required. With respect to the Public Liability Insurance, the Employer has made arrangements with its insurance broker that such a facility is readily available to it s Contractors. The Contractor is at liberty to make its own arrangements with regard to the above and is not obliged to utilise the facility offered unless choosing to do so. Upon award of the Contract, the Contractor shall cause its insurer to certify that the insurance covers required by the Contract and the Appendix to Terms and Conditions are in force. The Contractor shall produce proof of payment of premium, to the satisfaction of the Employer. However, in addition to this a non-compliant or alternative tender may be submitted where it can be demonstrated that significant benefits are offered. Network Rail reserves the right not to consider any non-compliant or alternative tender if a compliant tender has not been submitted. A separate and clearly identified submission must be completed in respect of any non-compliant or alternative tender. All non-compliances must be clearly listed in detail together with the benefits offered to Network Rail utilising Contract Arrangement and Contract Requirement Non-Compliant Tender Declarations in the Compliance Statements section of the submission documents.
