1 IMPORTANT SAFETY INFORMATION FOR SUBCONTRACTORS FWS Group of Companies is fully committed to prevention of workplace incidents and to full compliance with all legislated safety requirements in every jurisdiction in which we operate (in fact, our Safety Program will, in some instances, exceed local jurisdictional requirements.) To this end, FWS adheres to a comprehensive Health & Safety Program and compliance is required by all FWS personnel, our Subcontractors and all workers on site. This document is intended to give our Subcontractors a general summary of some of the prominent safety requirements that are expected. The complete version of our Health & Safety Procedures manual is available on every project site, as well as in our Regional Offices. Subcontractors are encouraged to review the comprehensive manual to familiarize themselves with our program. Contact may be made using any of the following methods: dlenters@fwsgroup.com DIRECT PHONE (403) CELL PHONE (403) Safety Meetings All new workers will be required to attend a safety orientation session prior to work. Depending on the project, these orientation sessions may be provided by FWS safety designates, however the responsibility for these orientations lies with the Subcontractor. These orientation sessions may take anywhere from ½ hour to 1 ½ hours, depending on the complexity of the Project. Weekly toolbox safety meetings are required by all Subcontractors. Each Subcontractor conducts their own toolbox safety meetings and provides documented minutes of their meetings to the FWS Superintendent for review and action as required. Documented evidence of these meetings must be submitted every week. These toolbox meetings generally last approximately 20 minutes. In some jurisdictions, Safety Committees are required. In these instances, every subcontractor will be required to have at least one Safety Committee member attend these meetings, which will generally be held on a monthly basis, and will take approximately one hour. 2 Incident Reports All incidents occurring on FWS projects need to be fully investigated and reported. Subcontractors are responsible to provide incident reports to the FWS Superintendent within 48 hours of the incident. Emergency Provisions Every Subcontractor will be responsible to have one first aid kit appropriate for the size of their allocated crew. Subcontractors are responsible to ensure they have at least one valid first aid trained person on site at any time while the Subcontractor is working. FWS will have available a full-size first aid kit, blankets and a stretcher on all project sites. FWS will have available an appropriate number of fire extinguishers located at the work areas. Subcontractors are responsible to provide at least one fire extinguisher in their site trailer, tool lock-up, and in areas where they are doing hot work. Emergency Mock practice drills may be conducted on occasion. This will involve a practice evacuation of the work areas to the mustering location. All personnel must participate during these drills. Hot Work Permits and Smoking All wood frame projects operate under an insurance warranty endorsement requirement which includes hot work permit conditions. All Subcontractors doing hot work are responsible to obtain a hot work permit from an FWS designate on a daily basis. Hot work permits are valid for one day only. Subcontractors should discuss with the Project Manager any additional insurance requirements applicable to hot work on wood frame projects. Smoking is not permitted on any wood frame (combustible) building project. Designated smoking areas will be provided and discussed with Subcontractors upon arrival to the site (zero tolerance.) Personal Protective Equipment Minimum standards for Personal Protective Equipment (PPE) include hard hat, safety footwear (boots), and safety glasses. Subcontractors are responsible for provision of ALL their own PPE. Minimum clothing requirements include long pants, and tee-shirt. No short pants, muscle shirts, or cut-off tee shirts. 3 Some project sites have a requirement for full-time use of reflective vests. Fall protection is required at all unprotected work locations greater than 3m. (10 ft) in elevation. Other PPE standards that will be enforced include, but not limited to: - Face Shields (cutting, grinding, etc.) - Hand Protection (depending on work) - Respirators - Body/Limb Protection - Hearing Protection Mobile Equipment All mobile equipment used on site may be subject to daily inspection. Heavy-duty hoisting equipment, such as cranes and forklifts, will require recent proof of engineering inspections. Operators of all hoisting equipment and man-lifts shall require proof of competency to operate such equipment. After Hours or Overtime Work Subcontractors working overtime or on weekends must comply with the following procedures: - Maintain total compliance with all OHS and/or WCB Regulations - No working alone situations will be permitted - Submit written confirmation to the FWS Superintendent that your company is responsible for and will supply a competent safety representative to satisfy all requirements of OHS and/or WCB Regulations and Codes - Subcontractors will comply in full to all FWS established safety policies - Overtime crew must have a first aid kit, first aid person, and a means of communication for EMS services if needed. 4 Fall Protection FWS operates under a 100% fall protection policy for all unguarded work greater than 3m. (10 ). On some projects, this standard may be more stringent (ie Cargill requirement for fall protection is 4 ). FWS fall protection policy supersedes any Provincial or Jurisdictional Regulation. This applies to all Subcontractors and Trades, regardless of occupation. Personal fall protection equipment is required for all workers on all elevated mobile man-lifts. WHMIS All workers must be trained in WHMIS. All controlled products brought onto site must be appropriately labeled, and material safety data sheets (MSDS) provided to the FWS Superintendent. All controlled products must be safely disposed by the Subcontractor responsible. Housekeeping and Material Storage Housekeeping in Subcontractor work areas will be maintained on a daily basis or as required through the insurance warranty endorsement. Weekly Clean-up: on some projects, FWS may elect to organize a weekly site-wide clean-up. All Subcontractors will be expected to participate to keep the site clean. Subcontractors are responsible for their own waste removal or as outlined in the instructions to bidders. Subcontractors must coordinate all material and/or equipment storage with the FWS Superintendent. Combustible material storage must be in compliance with that Project s insurance warranty endorsements. Ground Disturbance All ground disturbance (excavations) require utility locates and permits. 5 Sanitary Facilities FWS will maintain portable toilets appropriate to the level of man-power on site. On sites where females are present, a separate facility will be provided, with cost shared by the Subcontractors employing the female(s). Safety Violations and Disciplinary Measures FWS reserves the right to administer any appropriate disciplinary measures appropriate for the situation of non-compliance. This may include dismissal or suspension from the project. FWS will hold Subcontractor foremen accountable for safety performance of their personnel. This may include dismissal or suspension of the foreman for failing to carry out appropriate supervision of his/her personnel. Hazard Assessments and Safe Job Procedures All life-safety activities or tasks involving Subcontractors require the preparation of site-specific safe work procedures. The responsibility for preparation of these procedures lies with the Subcontractor performing such activities or tasks. Drugs and Alcohol Alcohol is not permitted at any time on the construction property, including job trailers. On some projects, FWS may implement a pre-access, random, and/or post-incident drug screening testing process, whereby every person on site must successfully pass a drug test prior to work on site. Direct any inquiries to the Project Manager for details on specifics for the project. Workers taking prescription medication, or having health issues that may affect their safety, or the safety of others, must inform the FWS safety designate of the condition. This information will be held in strict confidence. Respectful Workplace FWS has a Respectful Workplace Policy aimed at prevention of harassment, violence, and/or discrimination of others. Any worker violating this Policy will be subject to disciplinary measures, and may include suspension or dismissal from the Project. 6 Degrading or sexual posters or print material is not permitted anywhere within the boundaries of the Project site, including site trailers, lunchrooms, and/or tool lock-ups. Music Radios and Cell Phones Music radios are generally not permitted on FWS work sites. Radios have been known to interfere with emergency horns and signals and can be a general nuisance at the workplace. Walkmans, MP3 players and other personal music devices are not permitted. WCB and Insurance Requirements All Subcontractors must have valid Workers Compensation Board (WCB) accounts, in good standing. No company or person will be permitted to work without WCB coverage in place. All Subcontractors must carry General Liability insurance coverage, with a minimum $2,000,000 coverage. General Enforcement of Safety Subcontractors and workers are responsible for unconditional, genuine cooperation with all aspects of our Health & Safety Program, including compliance with all rules and regulations and for continually practicing safety while performing their duties. FWS reserves the right to enforce all aspects of safety, including FWS' policies and procedures, and all OHS/WCB Regulations and Codes. Failure by a Subcontractor to comply with the safety instructions of FWS Supervisors and/or Managers will lead to progressive forms of enforcement, up to and potentially including termination of Contract with cause. Construction Safety Representatives: the FWS Superintendent is the Primary Safety Representative on his/her assigned project and typically has the final say on all safety matters. Often, an FWS Site Safety Representative will be designated to support the Superintendent. In these cases, the Site Safety Representative has the authority of the Superintendent and of the Corporate Manager, Health and Safety to carry out his role accordingly.
