NOTE: This statement MUST be anonymous. Only your CWID should identify you. Applications with names, degree programs, or study abroad program identification included in the Statement of Financial Need will not be considered for a scholarship. For sections 3: responses are to be typed (not handwritten) on a separate sheet and attached to this application clearly identify and answer both sections separately on the same sheet of paper include your CWID in the top right corner of your attached statement page responses should be thoughtful and articulate the essay in section 3 must total 250 words or more. Applications with essays that contain less than 250 will not be processed. Section 1: Current Employment Status (check the appropriate box and fill in the blank if applicable): I am currently employed. I work approximately hours per week. I am currently employed. I work approximately hours per week. I also work hours per week during the summer break. I am NOT currently employed, but I work approximately hours per week during the summer break. I am NOT currently employed and I do not work in the summer. Section 2: Family Demographics Affecting Funding (complete as applicable): Note: The answers in this section do not determine funding and are optional. They simply provide information to the committee on your current family circumstances and free you from needing to provide this information in section 3, if you choose to do so. I have siblings. During the school year I have siblings also enrolled in a college or university. During the school year, siblings are residing at home with parents, guardians, or other family members. Section 3: Personal / Family Circumstances and Opportunity Costs (type and attach on the separate page): For these scholarships, the FAFSA provides an objective measure of your need. If applicable, please explain in this section: (1) any changes to your family situation since you completed the FAFSA form; (2) other rationale that demonstrates how your ability to fund college education or study abroad experiences is not fairly reflected in your FAFSA results. If you did not complete a FAFSA form for the current academic year, please explain why. Also, please explain if the time spent on your proposed study abroad program will affect your ability to earn income or have some other financial effect other than the direct cost of the course, travel, and living expenses. If you are incurring no financial opportunity cost, then you do not need to address opportunity costs. Direct course, travel, and living expenses, as well as personal sacrifices, i.e. attending a family event or missing friends or relatives left behind in Stillwater, are not opportunity costs and should not be listed.
