3 NIH INTERACT Evaluation Webinar Tips These webinars are intended for the 78 facilities randomized to the immediate intervention group Champions, co-champions, facility leadership and staff, medical care providers, and corporate leadership involved in project implementation Facilities randomized to the delayed intervention group will receive training in early 2014 they should not attend these webinars or be given usernames and passwords to the NIH section of the INTERACT website Thank you for your consideration

5 Objectives Review the definition and goals of the Medication Reconciliation Process Review why it matters to you and your organization Review how to conduct an accurate Medication Reconciliation

6 What is Medication Reconciliation? Identifying accurate list Comparison Recognizing discrepancies Making changes if necessary Updating with order changes Communication

7 Why does this matter? Patients admitted to the hospital receive new medications Poor access to complete medication list Upon discharge needed medications may be omitted Medication discrepancies occur in nearly 1/3 of patients admitted to the hospital There is no universally accepted optimal method of reconciling medications at time of transfer
