2 BLACKPOOL, TEACHING HOSPITALS NHS FOUNDATION TRUST POST: CONSULTANT OPHTHALMOLOGY TENURE: JOB PURPOSE: KEY INTERNAL RELATIONSHIPS: Permanent To work as a Consultant Ophthalmologist To work with consultant colleagues to provide a service for patients with ophthalmic diseases Clinical Director / Head of Department Ophthalmology Consultant Ophthalmologists Consultant colleagues in the Associated Directorates Directorate Management Team Clinical Administrative Support Staff KEY EXTERNAL RELATIONSHIPS: General Practitioners NHS Blackpool, NHS North Lancashire (PCTs) Blackpool Unitary Authority and Lancashire County Council Social Services THE POST: This is a replacement post for a Consultant Ophthalmologist with a subspecialty interest in oculoplastics or surgical / medical retina to compliment the needs of the Ophthalmic Directorate, working across the geographical areas of Blackpool, Wyre and Fylde. The appointee will join seven consultant colleagues and participate in a 1:8 on call rota. The successful candidate will join a multi-professional team based on the Victoria Hospital site and will also give clinical opinions when requested by consultant colleagues The post holder will be employed by Blackpool, Fylde and Wyre Hospitals NHS Foundation Trust. The Department of Ophthalmology The Work of the Department of Ophthalmology This district has the highest elderly population (21.4% of local population 65 or over) in the North Western Region, leading to a corresponding preponderance of age related eye disease viz: cataracts, glaucoma and age related macular degeneration. A sub-regional AMD treatment service is provided (PDT and antivegf) Paediatric Ophthalmology is well served, there being close liaison with Paediatric consultants and with the Community. A community screening programme for children is carried out in the district by Orthoptists. There is close and active liaison with the diabetic Physicians (Dr J Mackay Dr Ahmed and Dr Elrishi) regarding screening, assessment and laser photocoagulation of diabetic Medstaff/JobDescriptions/ophthal/Cons/Rev FINAL 2

3 retinopathy. A community based diabetic eye screening programme is well established and the Trust has recently appointed a new consultant ophthalmologist to lead in developing a diabetic eye disease service. The consultants of the department, in addition to undertaking a general ophthalmic service for the district, have special interests as follows: - Mr G Naylor - Mr W Pollock - Mr I Rahman - Mr SD Raj - Mr A Khalil - Mr T Saleh - Mr S Mishra - Paediatric ophthalmology and ocular motility Vitreo retinal surgery Corneal disease and surgery Oculoplastics Medical retina (AMD) Glaucoma Medical retina (Diabetes) (locum) Out Patients Consultant clinics are held at the Victoria Hospital, one other clinic being held at Fleetwood Hospital and one at Lytham. The out patient department at the Victoria Hospital compromises the following:- 1. Seven consulting rooms. 2. Large Orthoptist s room. 3. Orthoptic diagnostic area with OCT 4. Treatment / minor operations room 5. Two visual field rooms, with Humphrey automated visual field analysers. 6. Visual acuity testing area. GDX and corneal topography are available within the outpatient dept MRI, CT and ocular untrasound within the radiology department Fluorescen angiography in the Medical Retina Unit Biometry (IOL Master and UD6000) in the Ophthalmic Surgical Unit. There is a busy Orthoptic Department of five Orthoptists: Medical Retina Unit Head Orthoptist Senior I Orthoptist x 2 Senior II Orthoptist x 2 3 Consulting rooms Photography room for fundus photography and FFAs Laser room YAG laser, PASCAL and PDT OCT room Clean room for intravitreal injections (plus further clean room in the adjacent ophthalmic surgical unit) Separate AMD including injection clinics are held in MRU plus laser and diabetic clinics Medstaff/JobDescriptions/ophthal/Cons/Rev FINAL 3

4 In-Patients In-Patient facilities are based at Victoria Hospital within the newly opened surgical block (phase 6 of the hospital s development) but most surgery is undertaken as day case on the ophthalmic surgical unit Ophthalmic Surgical Unit All surgery is undertaken in the twin theatred ophthalmic surgical unit. Both theatres are equipped with Zeiss floor mounted microscopes and B & L Stellaris phaco systems, one with posterior segment capabilities. Preoperative preparation including GA patients takes place within the same unit. One stop (middle grade led) cataract clinics are held on OSU 2.1 Duties of the Post 2.1 Clinical The clinical commitments of this post will include responsibility for inpatient and outpatient care at Blackpool Victoria Hospital (BVH). There is also a commitment to give opinions on Ophthalmic problems in peripheral hospitals: this commitment is not onerous. Outpatient work will be conducted at BVH in a dedicated ophthalmic suite in the main out patient department and within the Ophthalmic Surgical Unit Indicative Work Plan 2 Operating Lists +1 flexible list 2 General Out patient clinics 2 Specialist clinics 8 DCC PAs 2 SPAs INDICATIVE WORK PROGRAMME Monday Tuesday Wednesday Thursday Friday A.M. 5 2.3 Teaching He/she will also be responsible, in collaboration with colleagues, for the supervision and training of middle grade doctors (either specialist registrar or non-consultant career grade doctor) 2.5 Research and Development 2.6 Contractual Commitment The post holder will have continuing responsibility for patients in his/her care and for the proper functioning of the service and will undertake the administrative duties associated with care of patients and the running of the clinical department Blackpool, Teaching Hospitals NHS Trust will be the employer for the appointment and will issue the Contract of Employment. 3. Appraisal There is a requirement to participate in annual appraisal by the Clinical Director in line with GMC regulations. 4. Associated Duties and Responsibilities 4.1 Main Duties and Programmed Activities Except in emergencies or where otherwise agreed with your clinical manager, you are responsible for fulfilling the duties and responsibilities and undertaking the programmed activities set out in your new Job Plan, as reviewed from time to time in line with the provisions in Section 11 below. 4.2 Associated Duties You are responsible for the associated duties set out in Schedule 2 of the Terms and Conditions. These include, amongst other things a requirement to: Collaborate with colleagues in primary and secondary care to implement protocols for the management of patients with ophthalmic disease. Keeps him/herself up to date with developments in the specialty. It is expected that active consideration will be given to existing technologies in terms of cost effectiveness. In addition the use of new techniques where they can be justified clinically will be encouraged. Conforming with state of the art advances will ultimately depend on other priorities within the total unit. To contribute to post-graduate and continuing medical education. The trust supports the requirement of CME/CDP as laid down by the Royal College of Ophthalmologists and is committed to providing time and financial support for these activities. 4.3 Objectives The purpose of including agreed personal objectives in your Job Plan is to set out in clear and transparent terms what you and clinical manager have agreed should be achieved in the year in question. 6 4.4 On Call Duties and Emergency Responses When not on an on-call rota we may, in exceptional circumstances, ask you to return to site for emergencies if we are able to contact you. You are not, however, required to be available for such eventualities. Where emergency recalls of this kind become frequent, we will review on call rota. 5. Individual office accommodation with computer and internet access is provided. 6.2 Responsibility for Junior Staff The post holder will provide supervision and support for junior. 7. POLICY AND STRATEGY 7.1 The Strategic Agenda Policies that drive our Agenda include: - The NHS Plan The National Service Frameworks National Clinical Guidelines (NICE) e.g. macular degeneration P.C.T. s Modernising Health and Social Services Improving Working Lives Trust training and development Strategy 7.2 Organisational Excellence The Trust is actively pursuing a total quality approach through self-assessment. 8 7.3 Health & Safety The Trust has a comprehensive Health and Safety Policy with corporate policies and procedures at Directorate level. It is the responsibility of each employee to observe these policies. 7.4 Risk Management A comprehensive Risk Management Strategy that includes a serious incident reporting system is in operation. All employees are expected to work within the system. 7.5 Clinical Governance Clinical Governance requires that all clinical staff accept personal responsibility for knowing what constitutes best practice in their field, defining policies to translate this into practice within their own service and measuring the clinical outcomes of the service. The Trust is adopting an approach to Clinical Governance which will draw together its preexisting self-assessment programme, clinical audit programme, complaints and litigation management, risk management, continuing medical education and clinical leadership. 8. RESOURCES 8.1 Blackpool, Teaching Hospitals NHS Foundation Trust Victoria Hospital The Blackpool Victoria Hospital serves a resident population of approximately 333,000. There are large seasonal fluctuations in population with Blackpool and surrounding areas of the Fylde coast attracting up to 16 millions visitors a year. The Victoria Hospital is a District general Hospital with 849 beds and serves the three surrounding boroughs if Blackpool, Wyre and Fylde. The hospital has a consultant staff numbering over one hundred and ten encompassing all major medical and surgical specialties. In addition there are visiting consultants in the specialties of nephrology, neurology, neurosurgery, plastic surgery, and radiotherapy. The hospital provides some regional services in Haematology. The large Radiology Department has state of the art Sprial CT and MR scanners. There are facilities for Nuclear Radiology in addition to plain radiography and Ultasound. The department is in the advanced stages of conversion to a film-free digital imaging service. A new Regional Cardiac Centre was commissioned in 2005 and a new surgical centre has just been opened. 9 8.2 Honorary Appointments Consultants may be eligible for appointment as Honorary Lecturer with the University of Central Lancashire. Those who make a substantial contribution in research or undergraduate education are eligible for promotion to Honorary Senior Lecturer, Reader or Professor. 9.1 KEY RESULTS 9.1 Clinical Results The post holder will be expected to contribute to the ongoing quality improvement of clinical services. 9.2 Policy and Strategy The appointee will be expected to participate fully in the development of ophthalmic services. This may include Directorate, Trust and multi agency project involvement to improve the quality of health care for local residents. 9.3 Resource Management As part of the Ophthlamology Directorate Team the post holder will be expected to ensure that services are delivered within agreed parameters. 9.4 People Leadership The appointee will be expected to provide leadership to her/his clinical teams. 9.5 Education The post holder will be expected to take a full part in Undergraduate and Postgraduate Medical Education. 10 GENERAL REQUIREMENTS AND CONDITIONS OF SERVICE a) Membership of the Royal College of Ophthalmologists and a certificate of higher Specialist, entry on or admission to the Specialist register within 3 months are essential. b) Applications from candidates who are unable to work whole time, or who wish to join in a job sharing arrangement will be considered. c) A satisfactory medical examination is a condition of employment for medical and dental staff in the National Health Service. Therefore, the successful candidate s appointment will be subjected to medical clearance from the Trust s Occupational health Physician. In relation to Hepatitis B screening and vaccination, it is a requirement of all staff that they should undergo periodic testing and where a post is designated as potentially prone to exposure, be vaccinated. d) The post holder must comply with all relevant policies, procedures and training on infection prevention and control. 10 e) The appointee will be required to live within 30 miles of the hospitals served, or to have access e.g. By motorway in the equivalent travelling time. f) You will be expected to work with local managers and professional colleagues in the efficient running of services and will share with consultant colleagues in the medical contribution to management. g) Subject to the provisions of the Terms and Conditions of Service, you are expected to observe agreed policies and procedures, drawn up in consultation with the profession on clinical matters. In particular, where you supervise employees, you will be expected to follow the local and national employment and personnel policies and procedures. You will be expected to make sure that there are adequate arrangements for hospital staff involved in the care of your patients to be able to contact you when necessary on-call. h) All medical and dental staff employed by Trusts are expected to comply with all Heath and Safety policies. i) It is appreciated that the fixed commitments set out below can only apply if the medical, nursing and administrative infrastructure is in place, both in terms of quality and quantity. Any variations on the provision of the infrastructure should be brought to the attention of the general manager of the provider unit. j) By the same token, if urgent cases have taken up a large part of the previous night, it is acknowledged that fixed commitments may not necessarily be fulfilled. k) Where an individual accepts involvement in clinical management consultant and this involves specified duties, an abatement of programmed activities will be permitted. l) It is expected that when management duties are relinquished, the programmed activities will be taken up. m) The Trust offers an attractive relocation and related expenses package for key employee s upon recruitment to the organisation. n) Interview expenses are paid in line with similar NHS Organisations and successful applicants will be reimbursed with second class travel and expenses payable from the point of entry to the United Kingdom for interview expenses. 11 REVIEW OF THE JOB PLAN You and your clinical manager will have agreed a prospective Job Plan that sets out your main duties and responsibilities, a schedule for carrying out your Programmed Activities, your managerial responsibilities, your accountability arrangements, your objectives and supporting resources. You and your clinical manager will review the Job Plan annually in line with the provisions in Schedule 3 of the Terms and Conditions. Either may propose amendment of the job plan. You will help ensure through participating in Job Plan reviews that your Job Plan meets the criteria set out in the Terms and Conditions and that it contributes to the efficient and effective use of NHS resources.
