2 TM fmc caring Giving the Gift of Life For many, organ donation is an opportunity to turn tragedy into life. Choosing organ donation often brings a sense of peace to the loved ones of someone who dies suddenly. This was the case for the Warder family when 17-yearold Tristen passed away at Flagstaff Medical Center last year surrounded by his parents, family and friends. Several hours before, the family had decided to donate his organs, knowing they could save lives. Tristen s heart saved the life of a Phoenix man; his liver and a kidney saved the life of a Tucson man; and his eyes allowed an 11-year-old girl who had been born blind to see. Samara Sam Griffin, an FMC intensive care nurse, was with the Warders during their final journey with Tristen. As a certified donor nurse, Sam is trained to talk with families about the donation process and help them make the right decision for their loved one and themselves regarding organ and tissue donation. Sometimes we know a patient can t survive their injuries or illness, Sam said. When this is the case, my role is to provide care and medical support to the patient and family through the end of life process. It is important for the patient to die with dignity and for their loved ones to be able to grieve and make the end-of-life decisions that are best for them. Some chose organ donation, some do not. There is no right or wrong; either way we are here to support their decisions. Sam exemplifies FMC s commitment to supporting patients and their families through difficult times. Recently, Donor Network of Arizona recognized Sam for her outstanding work with families facing decisions about organ and tissue donations. She was given the Arizona 2012 Outstanding Nurse award. Additionally, Donor Network recognized Michael Reidy, M.D., with the Outstanding Physician award and Marvin Cody, Navajo Translator, with the Outstanding Family Support award for their work with families. In 2011, 168 organ donors in Arizona gave life to 478 people. Today, more than 116,000 people in the U.S. are waiting for a life-saving organ transplant, with more than 2,300 people in Arizona on the organ donation waiting list. Arizonans can sign up as registered organ and tissue donors when they apply or renew their driver s license or ID at the Motor Vehicle Division, online at DonateLifeAZ.org or by calling DONOR. 3 fmc patients & families Bedside Report, Patient and Family Centered Care At 7 a.m. and 7 p.m. each day, the busy nurses station on Flagstaff Medical Center s 2-South Medical/Surgical Unit is unusually quiet. That s because FMC s new bedside report initiative has brought a fundamental change to the way nurses are communicating about their patients. Gone are the days when nurses huddled at the nurses station to discuss their patients. Instead, nurses are taking those conversations to the bedside, where the patient and the family can be involved. Now, at each shift change, the outgoing nurse and the incoming nurse meet with the patient and family to discuss the plan of care, what s happened with the patient during the last 12 hours, and plans for the next 12 hours. Patients and loved ones really appreciate being included in the conversations and development of the care plan, said Kara Cummings, R.N., M.S.N., clinical manager, 2-South. Open and honest communication between patients, their loved ones and the healthcare team helps ensure patients are receiving the best possible care. Bedside report is just one way FMC is embracing Patient and Family Centered Care (PFCC), an innovative way of thinking about the relationship between patients and their families and healthcare providers. PFCC is founded on the understanding that family plays a vital role in ensuring the health and well-being of patients of all ages. The ultimate goal of PFCC is to create partnerships among healthcare practitioners, patients and families that will lead to the best outcomes and enhance the quality and safety of healthcare a patient receives in the hospital and after discharge. When patients and families are encouraged to take an active role in the healthcare process, patient outcomes are better. Patients and family members are more relaxed, which promotes healing. Family support is increased, which leads to earlier patient discharge, adherence to post-discharge instructions, reduced readmissions to the hospital and increased patient and family satisfaction with the overall hospital experience. Bedside report was a very positive part of our time at FMC, said Erick Laurila, husband of a recent patient on 2-South. Being part of the dialogue between the nurses provided us the venue to ask questions and make decisions. We felt more at ease because we had a chance to talk with the new nurse coming on shift and we knew what to expect in the next eight to 12 hours. 4 Critical Care Units in the Fie Guardian Medical Transport s new Critical Care Ambulance is the first unit of its type in the nation. The square nose, enormous tires and sturdy demeanor give rise to thoughts of a fire engine. However, the oversized ambulance isn t equipped to fight fires; it is specially designed to provide critical care to patients who require more support over longer distances than a standard ambulance can provide. Guardian Medical Transport and Guardian Air joined forces to purchase and staff the new unit in an effort to provide additional transport options for patients. The unit provides: Life-sustaining care to critical patients, many of whom require specialized nurses in addition to highly-skilled paramedics Transport services and care for patients who require medical transport and emergency care but are unable to be transported by air or other ground ambulances due to size Guardian Air, a division of Flagstaff Medical Center, has provided advanced medical air transport services throughout the region since Guardian Air provides critical care transports 24 hours a day, seven days a week for adult, pediatric, neonatal and maternal patients. Each aircraft is staffed with a flight nurse, paramedic or respiratory therapist and a pilot. Medical helicopters are located in Flagstaff, Cottonwood, Winslow, Kingman and a new helicopter was recently placed in Tuba City. An airplane is based in Show Low. 5 fmc response ld Long-distance transports for those patients who are unable to fly or do not require the immediacy of air transport Traditional on-scene emergency medical service response as needed to support the efforts of other EMS responders An advanced level of care from Guardian Air nurses when needed The unit includes a winch and a gurney that supports up to 1,500 pounds, allowing larger patients to be easily and gently loaded into the ambulance. The patient area is specially equipped to accommodate larger patients, additional personnel and more resources and medical equipment for long-distance transports. This new unit fills a void when it comes to patient transports, said Mark Venuti, GMT director. With the addition of the new ambulance, we literally now have critical care units on wheels, in the air and at the hospital. And, we can transport patients who in the past couldn t be transported. This new unit is a great addition to the excellent medical care Flagstaff Medical Center provides to patients and visitors throughout Central and Northern Arizona and further. Guardian Medical Transport (GMT), a division of Flagstaff Medical Center, serves more than 6,200 square miles in Northern Arizona. GMT s 15 fully-equipped ambulances provide emergency medical services and non-emergent transports 24 hours a day, seven days a week. Each unit is staffed with a paramedic and an emergency medical technician (EMT). GMT has three stations in Flagstaff and one station in Tusayan (Grand Canyon). 6 fmc cover Teenage siblings Chris and Becca Piancino love to hang out together and try to beat each other at basketball. However, their daily games of H-O-R-S-E came to an abrupt end when the two were in a devastating rollover accident on Interstate 17 in October. Fortunately, Flagstaff Medical Center s Level I Trauma Center and Pediatric Intensive Care Unit (PICU) were just minutes away to provide the intense level of care the two needed. Chris was driving when their vehicle was clipped by a passing car, causing it to roll several times. The kids parents were driving ahead of them and saw the accident happen. It truly was like watching a film in slow motion; there was nothing we could do, said Stephanie Piancino, Chris and Becca s mom. It was the worst day of our lives. Guardian Medical Transport (GMT), Highlands Fire District and Flagstaff Fire Department responded to the scene. Paramedics provided on-scene live-saving care, which continued as the two young patients were transported to Flagstaff Medical Center (FMC) by GMT. Upon arrival at FMC, Becca was alert, with only minor injuries. But Chris injuries were complex and lifethreatening. He had broken vertebrae in his back and neck, a concussion, a broken sternum and wrist, and collapsed lungs. His condition demanded the expertise of the specially trained pediatric physicians and staff in FMC s PICU. While in the PICU, Chris needed a ventilator to help him breathe. He was placed in a drug-induced coma to give his body the best chance to heal. Nurses and respiratory therapists were continually by his side, while Pediatric Intensivist Susan Haeffner, M.D., monitored his delicate condition and coordinated his care. We owe our son s life to the paramedics, Dr. Haeffner, and to the amazing nurses and therapists at FMC, said Stephanie. The care our kids received was phenomenal. Dr. Haeffner was blatantly honest with us about the severity of Chris condition. She also told us she would be with us every step of the way and she was. Three days after the accident, Chris had improved enough that staff allowed him to wake up and he was taken off the ventilator. On day five he was moved out of the PICU. On day six, he went home. Less than three weeks after the accident, he was able to join Becca back at Coconino High School. Today, Becca is playing basketball on the freshman girl s team. Chris isn t able to play this year, but that doesn t stop him from participating. He hangs out with the junior varsity team and once he is medically cleared he will be able to serve as the team manager. I am bummed that I couldn t play, Chris said. But what really matters is that I am okay and my sister is okay and thanks to FMC, we will have many more opportunities to play basketball. Becca and Chris returned to the PICU to thank the staff. Dr. Haefner specializes in caring for critically ill and injured children in the hospital setting, from the time they are admitted to the hospital until they are discharged. To ensure a continuum of care, Dr. Haeffner works closely with the child s pediatrician or primary care physician regarding final diagnosis, medications and any follow-up plans prescribed at the time of discharge. Her specific areas of expertise include pediatric trauma, advanced critical care stabilization and management, advanced ventilation techniques and cardiac critical care. These units are designed to provide continuous care for those patients who are critically ill or medically unstable. Fortunately for the residents and visitors of Central and Northern Arizona, Flagstaff Medical Center s intensive care units are specially equipped with advanced life-saving and life-sustaining medical equipment and are staffed with specialty trained experts in critical care medicine. 9 fmc intensive care Specialized physicians One of the hallmarks of FMC s comprehensive intensive care services is the dedicated team of critical care physicians known as intensivists. These highly skilled physicians are board-certified in critical care medicine and are specially trained to work in the intensive care or critical setting. FMC is on the leading edge of facilities who utilize intensivists in the hospital 24 hours a day, seven days a week. Intensivists have a diverse array of skills expressly designed to meet intensive care needs. They are familiar with the many complications associated with patients in the ICU. Studies have shown that when care is delivered by intensivists, patient outcomes improve. Having eight full-time intensivists is rare in a hospital the size of FMC, said Christopher Murphy, M.D., FMC intensivist. Because of the number of intensivists and the state-of-theart technology in our ICUs, we are able to provide the same high level of critical care that you would find in a big city. This is important given the size of the area we serve, it s geographic isolation and the fact that many people would not survive their injuries or illnesses if they had to make the long trip to a bigger city. Physical therapists on the move Studies show that getting patients up and moving as soon as possible following surgery or while recovering from an illness aids in healing, restores physical function, elevates mental and emotional moods and decreases complications. This philosophy is now part of the healing and rehabilitation process for patients in FMC s intensive care units. Our focus is on getting patients off sedation and up as soon as Michael Lorentzen, R.N., Christopher Murphy, M.D., and Dan Saftoiu, P.T., D.P.T. they are medically stable, even patients who are on ventilators, said Dan Saftoiu, P.T., D.P.T., C.C.S., a physical therapist who is certified to specifically work with patients in the ICU. Dan s expertise allows him to engage patients in light activity and movement such as sitting on the side of the bed, standing or walking and small movements like combing one s hair or brushing their teeth. We encourage and guide patients activity levels, which works to advance their recuperation and return to normal function faster. There also is a positive psychological component for patients and their loved ones, when a patient is able to get out of bed and function is restored. Getting out of bed brings hope to those who need it most. Bedside care Each day, intensive care nurses are at the bedside of those who are facing actual or potential lifethreatening conditions. These patients are more likely to be vulnerable and unstable, requiring complex continuous nursing care. FMC s intensive care nurses have extensive certifications, knowledge, skills and experience to promote healing. They are advocates for the patient and a liaison between the patient, their loved ones and the healthcare team. FMC s ICU nurses embrace the Patient and Family Centered Care model. The units have an open door policy that allows family members and loved ones to enter the unit throughout the day and night. Family members are invited to be part of the conversation at the bedside between the physician, nurses and the comprehensive team of healthcare professionals. In an effort to identify these underlying conditions, Flagstaff Medical Center and the Heart & Vascular Center of Northern Arizona have partnered with the Shadows Foundation to develop the Hearts Worth Saving screening program. The program provides advanced screenings that can detect heart conditions that may not show up during annual health and fitness tests. The sophisticated tests take place at the Heart & Vascular Center of Northern Arizona in Flagstaff. The Shadows Foundation is a local nonprofit that provides financial assistance to those affected by life-threatening diseases. It provides the administrative oversight for Hearts Worth Saving as well as pays for charges not covered by insurance so first responders do not have to pay anything for the life-saving tests. More than 60 first responders from Summit and Flagstaff fire departments; Guardian Medical Transport; Coconino County Sheriff s Office and Department of Public Safety were among the first to take part in the screenings. Additional screenings are scheduled throughout 2013 in the Flagstaff area with plans underway to roll the program out to first responders in the Verde Valley area. The screenings revealed I needed to make some changes before I had a heart attack, Matt said. I have changed my diet, I am controlling my high cholesterol and I exercise more often. And, I will never miss an annual check-up with my doctor, no matter how good I feel. Most of us get so busy taking care of others we neglect to take care of ourselves.
