1 AXIS Healthcare, LLC Contracted Service Coordination/Case Management Vendors Hennepin County The following agencies have contracts with Hennepin County to provide service coordination/case management 2356 University Avenue West, Suite 210, St. Paul, MN 55114, Contact Meribeth Arndt, Supervisor AXIS was founded in 1999 to coordinate care and services for persons with long term disabilities. AXIS was created as a joint venture between The Courage Center and Sister Kenny Institute, two long-standing non-profit organizations experienced in providing rehabilitation services for persons with disabilities. The AXIS mission is to coordinate appropriate care and services for persons with long term disabilities so that they have access to and choices regarding all of the resources available to them. Outcome research demonstrates that AXIS care coordination & case management help people maintain health, live independently in the community, and avoid unnecessary health related complications & expenditures. AXIS provides services primarily throughout the Twin Cities Metro area and the surrounding counties. AXIS has extensive experience helping people work with the complexities of the health care system, and helping adults with long-term disabilities (from 18+ to seniors) participate in the case management of their home and community based waivers. AXIS staff are health and human services professionals and paraprofessionals with knowledge of both health care systems and social services systems. Our staff works well with clients from diverse groups and cultures. Language Interpreter services are provided at no cost to clients. We have successfully worked in several areas within social services since 1980 The population we serve varies from young adults (18+) to seniors We have experience working with persons with mental health issues We are a for-profit agency with one of the lowest staff turnover rates in the industry

2 2277 Hwy 36, Suite 200 Roseville, MN MN Brain Injury Alliance Sara Schlegelmilch, Supervisor The MN Brian Injury Alliance is the only state-wide nonprofit organization in Minnesota devoted solely to serving the needs of the estimated 100,000 Minnesotans, including 12,000 children and young adults, who live with disability due to brain injury, their families and the professionals who work with them. Individuals served by the MN Brian Injury Alliance MUST have a diagnosis of a brain injury. This is defined as: Traumatic Brain Injury (BI) or an external injury to the head/skull which results in brain injury, may have produced a diminished or altered state of consciousness and results in impairment of cognitive abilities or physical functioning. It can also result in disturbance of behavioral or emotional functioning. Acquired Brain Injury (ABI) or an internal injury to the brain that occurred AFTER birth. Some causes may include anoxic/hypoxic injury (e.g. cardiopulmonary arrest, carbon monoxide poisoning, hemorrhage, airway obstruction ), intracranial surgery (tumor, epilepsy), vascular disruption (stroke), infectious disease, seizure disorders, toxic exposures, encephalitis, etc. Injury to the brain that is NOT hereditary, congenital or degenerative. The mission of the MN Brian Injury Alliance is to enhance the quality of life and bring the promise of a better tomorrow for all people affected by brain injury. Our goal is to work towards a world where all avoidable brain injuries are prevented, all non-preventable brain injuries are minimized, and all individuals who have experienced brain injury can maximize their quality of life. Case managers at the MN Brian Injury Alliance are contracted by various counties in Minnesota to monitor and implement a system of support for individuals. Working closely with each participant, the case managers use waiver funds to coordinate a tailored service plan, identify and connect with service providers, and regularly review the support plan in place to ensure that each individual receives the highest possible level of customized service. Waiver funds can be used to pay for services and supports such as independent living skills, homemaking, personal care services, supported employment, transportation, behavioral services, or day program fees Each person's needs and vision of a better quality of life is unique, so, accordingly, each plan of support organized by Case Management is different. The case management program also provides relocation service coordination (RSC) within contracted counties.

CIP works with adults throughout the Hennepin County, metropolitan area. Fraser 6328 Penn Avenue South Richfield, MN Fraser is a Minnesota nonprofit that is nationally renowned for serving children of all abilities and adults with special needs. Fraser provides direct services, information and support in the areas of education, housing and healthcare. Since 1935, Fraser has been helping children, adolescents and adults in the greater Twin Cities area reach their potentials. Fraser has provided Targeted Case Management in our Family Focus Program and Autism Day Treatment program as well as Flexible Case Management for a number of years. Fraser s service options also include Contracted Case Management and Relocation Service Coordination with Hennepin County primarily for individuals using the DD Waiver. Fraser specializes in services for children of all ability levels and adults with special needs.

4 Hammer, Inc East Wayzata Boulevard Wayzata, MN Beth Siewert, Program Director Hammer is a private, nonprofit agency that has been providing uniquely designed services to meet the desires and needs of children and adults with disabilities since Hammer offers a variety of services primarily based in Hennepin County, although we also support individuals throughout the state of Minnesota. Hammer s contracted case management services are designed and directed by the individuals we support. Our case managers are skilled in providing: Customized assistance: we develop unique plans using a team approach, based on individual needs and desired service outcomes Continuity: we recognize the value of developing and maintaining long-term relationships with individuals Experience: we provide quality services to children and adults with a wide range of disabilities Our mission remains true to Alvina Hammer s vision of 84 years ago: to provide adults and children with disabilities the opportunity to experience life to its fullest.

Our mission is to strengthen the capacity of individuals and families to improve their quality of life. Keystone serves adults 18 and older Meridian Services, Inc nd Avenue South Hopkins, MN Meridian Services, Inc. s mission: Supporting people to realize their dreams. Above all we encourage and teach self determination and self advocacy. Meridian Services, Inc. provides case management services throughout Hennepin County and in 25 other counties within the state of Minnesota. Leann Thompson, Program Our goal is to build a long-term relationship with our consumers/families and provide Operations Administrator consistent service We work with more than 300 Russian -speaking families and we have a Russian interpreter (cellular) on staff. We have the ability to serve several different languages via a long term relationship lthompson@orionassoc.net with an interpreter agency. We provide contracted service /case management services for Elderly Waiver, BI Wavier, DD Waiver, and Relocation Services (RSC) Meridian Services, Inc. has more than 27 years of experience providing case management services. Meridian Services, Inc. provides services to all ages. Our case managers are trained in all waiver types. This allows for a seamless transition if a clients changes waiver types.

Our goal is to provide people with an understanding of their available resources and assist them in advocating so that their specific needs are met. People Incorporated Service Coordination 1315 Girard Ave N Minneapolis, MN People Incorporated promotes and secures independence for people with mental illnesses and other brain disorders We predominantly serve the 7-County Metro area, however we can serve individuals in outstate Minnesota. Our staff has specialized training in the areas of mental health/illness, chemical dependency, and developmental disabilities. Staff are also trained and have expertise in working with cooccurring disorders. People Incorporated is a non-profit organization.

7 Senior Community Services (SCS) Wayzata Blvd., Suite 335 Minnetonka, MN Mission: To assist older adults in our communities to enhance and maintain their independence, involvement, opportunities, and quality of life. Geographical locations: From eight local offices, our service coordinators (all licensed Social Workers) serve all of Hennepin County except Minneapolis. Adele Mehta, Supervisor Waiver services: Elderly Waiver clients and CADI clients age 60 and older Experience: SCS is a non-profit agency that has served the community 57-plus years and has a.mehta@seniorcommunity.org provided non-waiver Hennepin County-contracted case management services to elders throughout suburban Hennepin for more than 30 years. Most staff have more than 15 years experience working with older adults and have well-established connections with the informal resources in each community. Additional information: EW clients of SCS cannot receive homemaker services from H.O.M.E., another program of the agency.
