3 2016 OPEN ENROLLMENT It s time to review your benefit options and make your Open Enrollment decisions. This year medical plan premiums will increase by 5% except for the High Deductible Health Plans for full-time employees, which have no paycheck deductions. Depending on your medical plan and the number of covered dependents, you will see premiums increase from $5 to $28 per month. Salt Lake County will also bear its appropriate share of the cost increase and will continue to cover 80% of the overall costs of health care. The cost of health care continues to increase and continues to be a critical financial issue for both individuals and Salt Lake County. To help control health care cost increases, we all need to take an active role in our health care choices and personal health. Steps such as using the Salt Lake County HealthyMe Employee Medical Clinic, participating in the Healthy Lifestyles Wellness Program, and making wise choices in the medical plan you select and how you spend your health care dollars can make a different in the future costs of health care. Take the time during Open Enrollment to learn about your medical plan options, how you may be able to save money by switching to the High Deductible Health Plan, and how to make the most out of the benefits. WHAT TO KNOW FOR 2016 Cost increases. Paycheck deductions will increase by 5% for the Traditional PPO Plan. Part-time employees on the High Deductible Health Plan will also see a 5% increase to their premiums. Salt Lake County contributes to the Health Savings Account (HSA). For employees enrolled in the High Deductible Health Plan (HDHP), Salt Lake County will continue to contribute the annual contribution amount of $600 for employee only coverage and $1,200 for family coverage. The County contribution will be deposited with the April 7 paycheck. Employees enrolled in the HDHP can also receive up to an additional $500 for wellness incentives to their HSA. See page 11 for details. 90-day prescription supply available at retail locations for Regence. Employees and their family members who are enrolled in a Regence medical plan will now have the option of 90-day maintenance prescription supply at participating pharmacies. USE THIS GUIDE This guide is your overview and your reference to keep on hand for details of the changes coming to your benefit plans in 2016, to notify you of action required during Open Enrollment, and to help you make informed choices. Make your decisions early in the enrollment period to avoid the last-minute Open Enrollment rush. YOUR BENEFITS Open Enrollment is your opportunity to review your benefits and make changes to your coverage, or add or remove a dependent from coverage. Enrollment must be completed by February 26, 2016, at Review your medical and dental coverage. Add, change, or drop coverage for your dependents. If you do not take action during Open Enrollment the following will occur: You will remain enrolled the same benefits you have today, with the same dependents covered, except for the following: Health Savings Account (HSA) Medical Flexible Spending Account Limited Flexible Spending Account Dependent Daycare Flexible Spending If you do not enroll or re-enroll in the plans above, your annual contribution will be $0. You must re-enroll every year by electing your annual contribution amount. YOUR HEALTH Your health matters. Salt Lake County provides you comprehensive medical coverage and wellness programs to help you improve or maintain your health. Put your health first by taking advantage of these convenient programs and resources. Get your annual preventive exam, which is covered at 100% when you use an in-network provider. Employees enrolled in the HDHP also receive $125 contribution to their Health Savings Account (HSA). Participate in Healthy Lifestyles. Get 30 minutes of physical activity daily. Thirty minutes of physical activity can significantly reduce health risks such as obesity, diabetes, and heart disease.»» Get help quitting tobacco by using resources available to you via Healthy Lifestyles. 4

5 LEARN THE DETAILS BEFORE YOU MAKE YOUR DECISIONS Before you make your enrollment decisions, take time to understand what s new and changing, the options available to you and your potential costs. The information in this guide provides important details to help you make your decisions. The following changes and enhancements are effective April 1, WHAT S NEW AND CHANGING MEDICAL PLANS CHANGES Traditional PPO Plan High Deductible Health Plan (HDHP) 5% premium increase. 5% premium increase for part-time. PRESCRIPTION DRUG CHANGES Regence Medical Plans 90-day prescription supply available at participating retail pharmacies. MENTAL HEALTH CHANGES Traditional PPO Plan High Deductible Health Plan (HDHP) Mental Health Services, such as counseling, from a nonparticipating provider will be covered at the out-of-network rate. Currently, services from a non-participating provider are not covered. HEALTH SAVINGS ACCOUNT CHANGES High Deductible Health Plan (HDHP) Health Savings Account (HSA) annual maximum contributions: $3,350 (no change) $6,650 to $6,750 for family coverage $1,000 catch-up contribution for age 55+ (no change) Salt Lake County will deposit the following amount into your HSA with the April 7, 2016, payroll: Employee only coverage - $600 Family Coverage - $1,200 You can also earn additional money when you and your covered spouse/adult designee participate in the following wellness activities: Annual Preventive Exam: $125 Healthy Lifestyles Online Personal Health Assessment: $100 Annual Flu Vaccine: $25 5

AFFORDABLE CARE ACT (ACA) PROVISIONS The following is an assessment of the 2016 provisions of the Patient Protection and Affordable Care Act (ACA). Out-of-pocket maximums for an individual cannot exceed $6,850. This change affects family coverage for our High Deductible Health Plans (HDHP). The total out-of-pocket maximum on the family HDHP will remain at $7,000. An individual covered on a family HDHP will have a maximum out-of-pocket of $6,850. If an individual reaches the $6,850, the remaining family members would pay $150 of services until the $7,000 family out-of-pocket maximum is met. Employers are required to pay additional reinsurance fees and taxes. 6

7 MEDICAL PLAN COST CHANGES FOR 2016 Paycheck deductions are increasing for the Traditional PPO Plan, while the High Deductible Health Plan paycheck deductions remain at $0 for full-time employees. The Traditional PPO Plan and part-time High Deductible Health Plan rates will increase by 5%. The County portion of the cost will also be increasing overall by 5%. Paycheck deductions remain the same for the dental plan. LOWER COST, SAME COVERAGE AND CHOICE WITH THE HIGH DEDUCTIBLE HEALTH PLAN The High Deductible Health Plan (HDHP) encourages you to become a smart shopper of health care, while providing levels of coverage, quality, and care comparable to the Traditional PPO Plan. The High Deductible Health Plan shifts costs to when you actually use health care instead of a fixed cost (like paycheck deductions). The High Deductible Health Plan comes with a Health Savings Account (HSA), funded by both you and Salt Lake County. Your HSA can be used to pay for health care, prescription drugs, vision and dental expenses. You receive the same quality coverage with the High Deductible Health Plan as you do with the Traditional PPO Plan, and recommended preventive care services are also covered at 100%, such as well-child exams, annual check-ups and immunizations when you use a contracted provider. You have the same provider network choice, SelectHealth or Regence, as you do with the Traditional PPO Plan. When considering cost, think about your total out-of-pocket cost (paycheck deductions plus what you pay at the time of service). Also consider when you pay for the plan and services, through paycheck deductions or when you actually access care. Review the following pages to compare your total cost. 7

8 MEDICAL PLAN COMPARISON CHART Salt Lake County offers two types of plans-high Deductible Health Plan (HDHP) and Traditional PPO Plan. Each medical plan offers a comparable range of coverage and quality services. The key differences are when and how you pay for the plan and services, and the total out-of-pocket cost to you. The chart below summarizes the features of each medical plan type and what you pay for services. For each plan you will be responsible for a deductible and coinsurance or copay. The deductible is the annual amount you pay out of pocket for medical and prescription drug costs before plan begins to pay. Your prescription drug costs count towards your medical plan deductible and out-of-pocket maximum. You first pay the total cost of the prescription drug until have met your plan deductible, then you pay: 30 day retail supply: Tier 1: $10 copay Tier 2: 25% coinsurance with $25 minimum and $75 maximum Tier 3: 50% coinsurance with $50 minimum and $100 maximum 90 day mail order supply: Tier 1: $20 copay Tier 2: 25% coinsurance with $50 minimum and $150 maximum Tier 3: 50% coinsurance with $100 minimum and $200 maximum Specialty and injectable medications: 20% coinsurance 8

9 HIGH DEDUCTIBLE HEALTH PLAN (HDHP) TRADITIONAL PPO PLAN What do I pay when I need a prescription? continued Maintenance medications (for HDHP only): Deductible is waived for certain drugs that are identified as preventive or treat chronic conditions such as cholesterol and high blood pressure. See your plan administrator for a complete list. You re protected by the out-ofpocket maximum This is the maximum annual amount you would pay out-of-pocket for in-network care. Employee only - $3,500 Family - $7,000 The plan pays 100% for all remaining in-network costs. What pretax accounts can I enroll in to help pay health care expenses (such as deductibles, coinsurance, and copays)? Health Savings Account (HSA) Unused dollars roll-over from year to year and go with you when you change plans, leave Salt Lake County or retire. Annual 2016 contribution limits are: Employee only - $3,350 Family - $6,750 Age 55+ can contribute an additional $1,000 Limited Medical Flexible Spending Account Annual contribution limit is $2,500 (Can only be used for dental and vision). The Total Cost Picture: You need to look at how much you pay out of your paycheck and how much you pay when you access care to determine your total cost. Best Case: You and your family only access preventive care services during the year and have no prescriptions. You would only pay the amount deducted from your paycheck as long as you accessed in-network preventive care. Worst Case: You and your family had a high utilization year with many prescriptions, surgeries, or hospitalizations. You would pay the amount deducted from your paycheck plus the amount up to the out-of-pocket maximum.»» Your Case: The reality is you will likely fall somewhere in between. 11 GOOD HEALTH PAYS! EARN UP TO $500 Achieving or maintaining good health benefits you and your family. Salt Lake County offers wellness programs and resources to help you and your family achieve optimum health. There are two types of incentives offered, if you are enrolled in the High Deductible Health Plan, you are eligible for both incentives: Healthy Lifestyles Health Savings Account Wellness Incentive HEALTHY LIFESTYLES All benefit eligible employees and their spouses/adult designees can participate in Healthy Lifestyles.By participating in healthy behaviors and wellness activities throughout the year, you can earn points towards a cash rebate (taxed) of up to $250. To sign-up, visit call , or myhealthylifestyles@slco.org Attend a biometric screening appointment with Healthy Lifestyles. At this appointment you will complete simple biometrics (i.e. blood pressure, cholesterol/glucose lipid test, body composition). Begin participating in wellness activities throughout the year. Once you attend your Healthy Lifestyles appointment, you can begin earning points for completing wellness activities such as medical exams, attending Healthy Lifestyles sponsored events, and participating in wellness challenges and programs. All completed activities are recorded through an online platform (WellSteps). Follow-up with yearly biometric screening appointment. The month you originally attend your first appointment will be your designated screening month. Each year, attend a biometric screening appointment during this same month. At this appointment Healthy Lifestyles will calculate your annual rebate and discuss your health screenings with you. HEALTH SAVINGS ACCOUNT WELLNESS INCENTIVE (FOR HIGH DEDUCTIBLE HEALTH PLAN) If you are enrolled in the High Deductible Health Plan, you can also earn up to an additional $250 (employee only) or $500 (employee + spouse/ adult designee) annually for your Health Savings Account (HSA) by completing the following activities: Annual preventive exam - $125 Personal Health Assessment (PHA) administered through Healthy Lifestyles - $100 Annual flu vaccination - $25 Tax-free money is deposited into your HealthEquity Health Savings Account quarterly (July, October, January, and April) following the completion of the activity. For example, if you complete your annual preventive exam on April 15, Salt Lake County will deposit $125 into your HSA with the July 22 paycheck. 11

12 SAVE WITH A PRETAX HEALTH ACCOUNT A pretax health account can be used to cover eligible expenses such as deductibles, coinsurance and copays. Your medical plan choice determines the health account(s) that can be paired with it. How the accounts work and the advantages of each vary, so it s important you understand the features. THREE GREAT REASONS TO CHOOSE THE HIGH DEDUCTIBLE HEALTH PLAN WITH A HEALTH SAVINGS ACCOUNT You ll save money on premiums. And you can put what you save in premiums into a pretax HSA. You can invest what you save. Instead of paying every month for coverage you may not even use, invest the premium in your own HSA. Earn interest on your savings, and as they grow, invest in mutual funds to increase your return. After all, the money is yours. Take it with you when you change jobs or retire. An HSA is a tax-savings retirement opportunity. Think of an HSA as a medical 401(k), only better. You put money in and deduct it from your taxes, plus Salt Lake County contributes too. Then you earn interest or receive investment returns your choice. The money is always tax free when you use it on eligible health care expenses. At age 65, you can withdraw the money for any reason and the money used for non-health care expenses is taxed like a 401(k). FLEXIBLE SPENDING ROLLOVER RULE You can carry over up to $500 of your unused medical or limited FSA balance at the end of the plan year. The carryover is available immediately, so up to $500 of your remaining balance for the benefit year can be carried over into and there is no grace period for you to use the funds. Your rollover amount will not affect your election limit for the upcoming plan year. For example, if you choose to elect the maximum of $2,500 for the upcoming year, plus any carryover from the current year (up to $500), you could start off the plan year with a balance of as much as $

16 ADDITIONAL INSURANCE OPTIONS AVAILABLE TO YOU Open Enrollment provides you the opportunity to evaluate and enroll in additional insurance options and programs. FLEXIBLE DAYCARE SPENDING ACCOUNT The Flexible Daycare Spending Account allows you to save pretax dollars to pay for qualified daycare expenses for children up to age 13. Highlights of the program: Enroll each year during Open Enrollment to participate and set your annual election amount. Estimate your expenses carefully, because you ll lose any unused balance at the end of the year, per IRS rules. You can contribute up to $5,000 to pay for dependent daycare expenses you incur throughout the year. If you are married and filing federal taxes jointly, both you and your spouse can have a Flexible Daycare Spending Account, however your combined contributions cannot exceed $5,000. LIFE INSURANCE PLANS Open Enrollment is the perfect opportunity for you to review your life insurance needs, which can change when you get married, have children, or purchase a home. If you need additional coverage, you can enroll or increase your existing coverage but will be required to complete a statement of health. If you are currently enrolled and satisfied with the level of coverage, you will remain enrolled in your current coverage and no action is required. To enroll, go to REVIEW YOUR BENEFICIARY DESIGNATIONS Open Enrollment is also a good time to make sure you have the most up-to-date beneficiaries on your life insurance plans. Visit the to review and designate your beneficiaries. LEGAL PLAN U.S. Legal Services provides access to attorneys to cover most personal legal needs. Family Defender Plan covers you and your family (children up to age 23) and cost $8.45 per paycheck. Family Defender with extended coverage (covers your and your spouses parents) cost $10.75 per check. Indentity Defender provides Idenity Theft services and costs $4.98 per paycheck. If you are currently enrolled, you will automatically re-enroll unless you choose to cancel your coverage during Open Enrollment. To learn more, enroll or cancel coverage, go to 16

17 SHORT-TERM DISABILITY PLAN Short-Term Disability insurance is an important part of being prepared for the unpredictable events in life. Short-Term Disability provides partial wage replacement should you become disabled and require a leave of absence due to a non-work related illness or injury, or pregnancy. If you are currently enrolled in Short-Term Disability, your coverage will continue unless you waive coverage during Open Enrollment. If you are not currently enrolled, Open Enrollment is your only time to enroll in the plan. IT S ALL ABOUT CHOICE Most disability plans give you little choice about features like how much it pays or for how long. DisabilityFLEX insurance is different. It provides the flexibility to create your own plan based on your needs and what you can afford. To help you make the right choices, here are three important questions you should ask yourself: How long can I go without a paycheck if I m unable to work due to an illness or a disabling event? Keeping in mind my weekly expenses and lifestyle, how much of my paycheck is an absolute must if I m unable to work and collect my full income? What conditions would I be most likely to experience (e.g., pregnancy, back pain, carpel tunnel, etc.) and how long might I be out of work? CUSTOMIZE YOUR INCOME PROTECTION PLAN With this plan, you ll have a say in the following features: Benefit level. You can choose a weekly flat dollar amount up to 60% of your earnings. Anywhere from $100-$1,000 weekly benefit amounts (in $100 increments) are available. Benefit duration. Up to 13 weeks.»» Waiting (commencement) period. IIt s up to you to decide how soon your benefit begins after a disabling event. Depending on your needs, you can choose an 8-, 15- or 30-day commencement

18 COMPLETE YOUR ENROLLMENT ONLINE BY FEBRUARY 26 Once you have evaluated your options, visit to complete your enrollment. 1. Click the Enroll button. 2. Enter your PeopleSoft username and password. 3. From the Main Menu > Self Service > Benefits. 4. Click on the Select button in the Open Benefit Events box. 5. Ensure you review your choices for all benefits listed and make any necessary changes. 6. Submit your changes. 7. Print a copy of your confirmation for your records.
