2 What is Open Enrollment? Open Enrollment is Your Once a Year Opportunity: To enroll for benefits Make changes (add or remove a dependent, change plans) Enroll in a new line of coverage All Changes Effective July 1, 2014

3 Other Than Open Enrollment - You may enroll for Benefits at any of the following times: After completing initial eligibility period (new hires) Within 30 days following a qualifying event: Marriage Divorce or legal separation Birth or adoption Receipt of a Qualified Medical Child Support Order (QMCSO) Change in your dependent s eligibility for benefits Change in work status that impacts your benefits Change in your partner s work status that impacts benefits Death of a partner or covered dependent

4 Dependents Eligibility Definition Required Documentation SPOUSE/DOMESTIC PARTNER CHILDREN Eligible dependent children include: Natural Children Step children Legally Adopted Children Children placed for adoption Children for whom legal guardianship has been awarded to employee or his/her spouse/partner A member of the opposite sex to whom you are legally married. A domestic partner is a member of the same or opposite sex that meets the criteria as outlined in the affidavit Medical only: Dependent children up to the age of 26 According to the Health Reform Definition. Dental, Vision, and Life: Unmarried children residing in the U.S. who are under the age of 19, or 25 if there is evidence the children are: Full time students at accredited schools, not regularly employed on a full time basis and who are primarily dependent upon the employee for support and maintenance; Ordered to be covered by a Qualified Medical Child Support Order or other court or administrative order. Please see additional eligibility for Qualifying Disabled Children A copy of the top half of the front page of the employee s most recently filed federal tax return that includes your spouse. You may black out all the financial information and ALL BUT the last 4 digits of your social security number; or Photocopy of marriage certificate if marriage has occurred within one year of eligibility. For the domestic partner, at least three of the documents described in the Affidavit. Natural Children: Photocopy of birth certificates showing employee s name. Step Children: Photocopy of birth certificates showing employee s spouse s name and a copy of marriage certificate showing the employee and parent s name. Adoptions/Legal Guardianships: Photocopy of Affidavits of Dependency, Final Court Order with presiding judge s signature and seal or Adoption Final Decree with presiding judge s signature or seal. Children of the domestic partner: Photocopy of birth certificates showing the partner s name. If applicable: A copy of the top half of the front page of the employee s most recently filed federal tax return that includes your child/ren. You may black out all the financial information and ALL BUT the last 4 digits of your social security number.

9 Healthcare FSA Elect before-tax dollars to use toward eligible healthcare expenses: Medical Dental/Ortho Vision $2,500 Annual Maximum Contribution as required by Healthcare Reform Annual contribution is 100% available for reimbursement from the first day of the Plan Year. With rising healthcare costs, every penny counts!

10 Dependent Care FSA Elect before-tax dollars to pay for eligible dependent day care services while you (and your partner) are working or attending school full-time. Daycare centers, Nannies, Nursing Homes Dependent Care FSA funds are only available for reimbursement as they are deducted from your paychecks and contributed to the Plan (money-in, money-out). They are not immediate as with the healthcare FSA. Qualifications: Individual that regularly spends at least 8 hours a day in your home Dependent care for children under age 13 Dependent who is physically or mentally incapable of self-care Your daycare provider must claim your payments as income and pay tax

11 FSA Forfeiture FSA balances do NOT rollover at the end of the Plan Year! You must spend all the money in your account by the end of the Plan Year, or your remaining balance will be forfeited. TIPS: Be conservative. Set aside only dollars you will actually use. Access your account to check balance frequently. The CSCC plan includes a 60 run-out period. The claim must incur during the FSA plan year of 7/1 6/30 and submitted by 8/30 to be eligible.

12 MetLife term-life and AD&D insurance To make changes complete an enrollment/update form and submit documents before the May 30, 2014 deadline. Examples of changes: Beneficiary update Coverage amount (a health questionnaire is required to increase coverage amount) Adding or removing coverage (a health questionnaire is required to add coverage)

13 VSP No changes to current plan design. Remember to obtain your richest benefit, you should consider utilization of a VSP provider that is in-network. Our VSP representative Brian is present today if there are any questions.

14 Delta Dental No changes to current plan design Remember to obtain your richest benefit, you should consider utilization of a PPO provider. Our Delta Dental Representative Elana is present today as well.

16 What s new? Core/PPO Plan Emergency Room co-pay has increased from $150 to $250. Affordable Care Act co-pay administration changes and outof-pocket expense increases. $4500/$9000 (includes copay) Core/PPO and HDHP Pharmacy will be converted to a restricted generic plan (there is a cost increase to employees if a brand name is requested by the employee; no added cost if a physician requires the brand name). Tier-3 copay increase on the prescription drug list from $60 to $80. Working Spouse Premium - effective January 1, 2015, there will a fee for spouses who are eligible for medical coverage through their employer but elect Columbus State primary coverage: 24 pay-period fee is $50.00/pay and 18 pay-period fee is $66.66/pay.

19 What do I need to do? During open enrollment, you need to: Determine which benefits you would like to be enrolled in for the new year Plan year July 1 st - June 30 th (deductible and Out of Pocket run calendar year for medical and dental) FSA requires an enrollment form annually If you are not making any other changes, no forms are required. If a spouse or partner is enrolled on the CSCC plan, you are required to submit a spousal/partner affidavit in order to avoid the Working Spouse Premium.
