4 Your annual enrollment checklist Here s what you should do to prepare for enrollment. Read this guide. Find out about the benefit changes for next year and what you need to do to enroll. Complete the Annual Enrollment Health Questionnaire now through February 26. Although not required, we encourage you and your covered spouse/domestic partner to take the questionnaire. It s a great way to learn about your current risks and help you set wellness goals for the year ahead. Log on to the BP HR & Benefits Center at bp.com/lifebenefits and enroll. Remember, you will need your user ID and password to log on. For instructions on how to obtain your user ID/password, see page XX. Confirm your eligible dependents As part of the enrollment process, you are required to validate all dependents to ensure only valid dependents are covered under our health and protection plans. If adding coverage for a disabled dependent, he/she must also be verified through a separate process with Aetna. An eligible, covered child who is totally and permanently disabled at the time he/she turns age 26 is eligible for continued coverage if approved by Aetna, the claims administrator. 4 Simplify your life

5 What s changing for 2016/2017 Below is an overview of the important changes to your BP benefits for next year. A full description of the changes can be found beginning on page 6. Coverage outside the US No changes For an overview of your coverage when outside the US, see page 7. Coverage inside the US Medical plan changes Emergency room copays and premiums will increase. Introduction of telemedicine benefit through Teladoc. See page 12. Wellness program Introduction of Discover BP course for the Million Step Challenge, digital workshops and self-directed health coaching. No Fitbits in 2016, except for new hires/newly eligibles. If you are a US expatriate on assignment outside of the United States The two options available to you during annual enrollment are: Expatriate Medical Plan No Coverage When you enroll in the Expatriate Medical Plan: Your coverage outside of the US is provided by Cigna, and most medical services and medications are covered 100% by BP with little or no out-of-pocket costs for you. Note that CIGNA coverage outside of the US is through a plan that is not sponsored by BP America. As a result, you might have different requirements applicable to that coverage. Your coverage inside the US is provided through Aetna under the HealthPlus PPO Option of the BP Medical Plan. REMEMBER! The annual enrollment period beginning February 1, 2016 through February 26, 2016 is your only opportunity to enroll for BP medical coverage for the 2016/2017 plan year. The benefits you elect are effective April 1, 2016 through March 31, 2017, unless you have a Qualified Status Change (QSC) during the year (for example, you marry, divorce, or add a new child). 5 Simplify your life

7 Medical coverage Your coverage levels under the Expatriate Medical Plan match the benefits offered to all expatriates from most of the world when outside of the US. The table below shows a snapshot of your medical coverage when outside the US. How to obtain services BP covers 100% of the health care expenses incurred for most services you obtain outside the US. When you and your covered family members seek medical care, you do not pay copays or coinsurance. That means very little, if any, out-of-pocket expenses for you! Your costs for medications outside the US are also covered 100% by BP. 7 Simplify your life

10 Medical coverage Your medical coverage for services obtained while in the US under the Expatriate Medical Plan is administered by Aetna (medical) and Express Scripts (ESI) (prescription drug) under the HealthPlus PPO option of the BP Medical Plan. The below table shows a snapshot of the in-network medical coverage available to you through the BP Expatriate Medical Plan. 11 Medical plan changes Medical plan premiums increasing Medical premiums are increasing for 2016/2017 across all plans. While rate increases vary depending on which plan you elect, BP s overall increases are relatively low and continue to trend below the national average. Emergency room copays increasing Emergency room (ER) copays will increase for the upcoming plan year. When in the US, you currently pay a flat $100 copay for ER visits. Beginning April 1, 2016, emergency room visits will require the following payment: $150 copay plus 20 percent coinsurance, no deductible. 11 Simplify your life

12 Introducing Teladoc How it works BP is introducing Teladoc, an added medical benefit that gives you 24/7 access to a national network of U.S. board-certified doctors when enrolled in an Aetna medical plan. Set up an account via teladoc.com/aetna Through Teladoc, you can call a physician from anywhere - home, work, or on the road when in the US. Teladoc doctors diagnose non-emergency medical problems, recommend treatment, and can even call in any necessary prescription(s) to your pharmacy of choice for conditions such as: Respiratory infections Allergies Complete your medical history Ear infections Urinary tract infections Colds and flu Sore throat Pink eye Request a consult and a US board-certified physician will call you back within one hour. 13 BP has you covered The BP active medical plan meets government standards for providing minimum, essential coverage under the Affordable Care Act (ACA). The BP active medical plan currently provides more coverage at a lower cost to you than a health plan that can be purchased through the health care exchanges. By participating in the BP plan, you also pay for coverage on a before-tax basis, which provides you additional cost savings. Don t forget about your other BP benefits Available only during annual enrollment This guide focuses on benefits changing for the 2016/2017 plan year. Be sure to review your other benefit options and consider whether you need to make changes during annual enrollment as changes to the benefits below can only be made during the enrollment period or if you have a Qualified Status Change. 14 Beneficiaries reminder Things change. That s the one thing in life we can be sure of. That s why it s so important to review your beneficiary designations at least once a year and what better time than annual enrollment! All too often, failure to update account beneficiaries leads to unnecessary delays, taxes or legal battles for those you intend to protect. Eventually someone gets your money, but it may not be the people you would have chosen. If you haven t done so lately, take a few moments to review and update your beneficiaries. It s a quick and painless process that can make all the difference to your family s future welfare. To update beneficiaries for your financial protection plans (Basic Life and Group Universal Life Insurance): Online: Log on to the BP HR & Benefits Center via bp.com/lifebenefits Click the Health & Protection Benefits tab Under the Overview heading, select Beneficiaries from the drop down menu Phone: Call the BP HR & Benefits Center at or if outside the US at , 7 a.m. 7 p.m. Central time, Monday through Friday. Note: To update beneficiaries for an equity award, you will need to separately log on to your personal brokerage account through Fidelity. To update beneficiaries for your BP Retirement Plan(s) Online: Log on to NetBenefits at Click Your Profile at the top Click the Beneficiaries link in the About You section Click the Update button to add new beneficiaries,* make changes to your current beneficiaries, or to alter the percentages each will receive Phone: Call BP Retirement Services at Fidelity at , 7:30 a.m. 11:00 p.m. Central time, Monday through Friday. *You will need your beneficiary s Social Security number, date of birth and address. 15 Your financial health is important too! Although you can make changes to your savings plan contributions any time throughout the year, annual enrollment is a good time to ensure your savings strategy is still aligned with your retirement savings goals. BP 401(k) savings plans allow you to save a percentage of your eligible pay towards your retirement (up to legal limits). You can elect to contribute on a before-tax, after-tax or Roth 401(k) basis, or a combination of all three. The IRS limits on savings plan contributions are reset each year for the costof-living adjustments, and the limits for 2016 are outlined below. 16 Just a reminder Medical premiums As an expatriate, you have coverage through two providers both Cigna and Aetna, depending on whether you are inside or outside the US. You will only pay one premium for your benefits coverage, which is applicable to the HealthPlus Option. Dental coverage Your dental benefits will not change for the 2016/2017 plan year. Dental coverage will continue to be provided by Cigna both inside and outside of the US. Enrollment The annual enrollment period (February 1 - February 26, 2016) is your only opportunity to enroll for BP medical coverage for the 2016/2017 plan year. The benefits you elect are effective April 1, 2016 through March 31, 2017, unless you have a Qualified Status Change (QSC) during the year (for example, you marry, divorce or add a new child). The benefits you elect are effective April 1, 2016 through March 31, 2017, unless you have a Qualified Status Change (QSC) during the year. The period to change your existing coverage elections due to a QSC is 30 days. 16 Simplify your life

17 How to enroll The online annual enrollment period is Monday, February 1, 2016 through 11:59 p.m. Central time, Friday, February 26, Even if you re happy with your current coverage, we encourage you to review your enrollment elections this year. You ll need your BP HR & Benefits Center user ID and password to access the site. Forgot your User ID? Click I Forgot My User ID on the LifeBenefits main page. You will be prompted to provide the last four digits of your SSN, your birth date, and if needed, your ZIP code. You will then be directed to the password page to enter your password. Before you begin Forgot your password? Click I Forgot My Password on the LifeBenefits main page. If you created a hint when you set up your account, it will display to remind you of your password. If you have no hint on file or still do not remember your password after reviewing your hint, you will be prompted to answer security questions and then be prompted to reset your password. Have a preferred on file? You can choose a password reset via your preferred or by US mail if you do not have a preferred on file. 17 Simplify your life

18 Follow these steps to enroll for your 2016/2017 plan year benefits: 1. Go to LifeBenefits at bp.com/lifebenefits to log on to the BP HR & Benefits Center online. Enroll online 2. Under Your Action Needed, select the option to complete your annual enrollment. 3. Review and validate your dependents on file with the BP HR & Benefits Center. 4. Make your enrollment elections,** including any contributions to Health Care Flexible and/or Dependent care spending accounts. 5. When you re finished enrolling, click Complete Enrollment and then OK. For your elections to take effect, you must complete this step and see the Completed Successfully page. 6. Print this page as confirmation of your enrollment elections. If the BP HR & Benefits Center has your valid address on file (under Your Profile), you ll receive an confirmation of your elections. 7. After annual enrollment, a confirmation statement will be mailed to your home. Please review it carefully and ensure your elections were accurately captured. Employees on international assignment, are encouraged but not required to complete the Annual Enrollment Health Questionnaire. ** Enrollment in the plan is your authorization for BP to deduct the required contributions from your paycheck on a before-tax basis, where permitted by law and with the exception of the Long-Term Disability Plan contributions. Enroll by phone If you don t have Internet access or prefer to enroll by phone, you can call the BP HR & Benefits Center toll-free at or (reverse charges accepted) to enroll in your 2016/2017 plan year benefits. Representatives will be available to assist you 7 a.m. 7 p.m. Central time, Monday through Friday. You ll need your BP HR & Benefits Center password. If you don t enroll... If you re currently enrolled in the BP Medical Plan and you don t take action during annual enrollment to enroll for benefits for the 2016/2017 plan year, you will be assigned coverage based on the coverage you have as of March 31, If you re not currently enrolled in the BP Medical Plan and you don t take action during annual enrollment to enroll for benefits for the 2016/2017 plan year, you will not have coverage under the plan for 2016/2017. 22 Some important things to know Notice of other means to qualify for the BP Wellness Program Your health plan is committed to helping you achieve your best health status. Contact StayWell at , and we will work to find a wellness program with the same reward that is right for you in light of your health status. Free or low-cost health coverage to children and families offered by Medicaid and the Children s Health Insurance Program (CHIP) If you are eligible for health coverage from BP or another employer but are unable to afford the monthly premiums, you may qualify for a premium assistance program that some states offer to help pay for your coverage. These states use funds from their Medicaid or CHIP programs to help people who are eligible for employer-sponsored health coverage but need assistance with paying their health premiums. This typically applies to households with an annual income under $44,000. If you or your dependents are NOT currently enrolled in Medicaid or CHIP, and you think you or any of your dependents might be eligible for either of these programs, you can contact your state Medicaid or CHIP office. Dial KIDS NOW or go to to find out how to apply. If you qualify, you can ask the state if it has a program that might help you pay the premiums for an employer-sponsored plan. Special enrollment opportunity If it s determined that you or your dependents are eligible for premium assistance under Medicaid or CHIP, BP is required to allow you and your dependents to enroll in a company-offered plan. To qualify for this special enrollment opportunity, you must be eligible for BP coverage but not already enrolled. In addition, you must contact the BP HR & Benefits Center and request BP health coverage within 60 days of being determined eligible for Medicaid or CHIP premium assistance. If you enroll within the 60 days, Medicaid or CHIP will pay for a portion of the BP health plan premium cost. For a list of states that participate in premium assistance, go to the plan s summary plan description at bp.com/lifebenefits, which contains state-by-state information and contact numbers. Reconstructive surgery benefits The Women s Health and Cancer Rights Act requires the company to notify plan participants of the reconstructive surgery benefits following a mastectomy. Benefits will include coverage for prosthesis; treatment of physical complications of the mastectomy, including lymphedema; all stages of reconstruction for the breast on which the mastectomy was performed; and reconstructive surgery on the nondiseased breast as necessary to produce a symmetrical appearance. Coverage will be provided in a manner determined in consultation with the attending physician and the patient. All benefits will be subject to the applicable deductible, coinsurance and copayments. HIPAA special enrollment rights Under HIPAA, if you lose your group health plan coverage, you may be able to get into another group health plan for which you are eligible (such as a spouse s plan), even if the plan generally does not accept late enrollees, if you request enrollment within 30 days. (Additional special enrollment rights are triggered by marriage, birth, adoption and placement for adoption.) Therefore, once your coverage ends, if you are eligible for coverage in another plan (such as a spouse s plan), you should request enrollment as soon as possible. HIPAA privacy notice When you enrolled in the BP Medical Plan, you should have received the plan s Notice of Privacy Practices. Note: BP regularly updates the Notice of Privacy Practices to include new regulatory changes. To view the notice, you may request a copy at any time by contacting the BP HR & Benefits Center at or by accessing LifeBenefits at bp.com/lifebenefits. Click Benefits handbook and HIPAA Notice of Privacy Practices under Related links. Imputed income If a dependent or domestic partner you enroll in a BP plan does not also qualify as your dependent for federal income tax purposes, there are tax ramifications. Tax law does not exclude from your income BP s contribution toward the cost of your dependent s coverage, meaning that you could incur imputed income on the value of the BP-provided coverage. The law also prohibits you from paying for such coverage on a before-tax basis. You re responsible for notifying the BP HR & Benefits Center if your covered dependent does not qualify as your tax dependent, and domestic partners will be presumed to be non-qualified unless you provide evidence to the contrary. Truthfulness of statements All coverage under the BP benefit plans is based on the truthfulness of statements made by you and your dependents. By enrolling, you certify that the information you provide about your and your dependents eligibility to participate in BP benefit plans is true and correct. Any fraudulent statement or withholding of information about your and/or your dependents eligibility may retroactively invalidate coverage and be a violation of the BP Code of Conduct, subjecting you to legal action including termination of coverage and recoupment of benefits paid. 22 Simplify your life Start over
