INFECTION AND HAZARD CONTROL POLICIES Introduction Dental hygienists must recognize that a health risk exists as a result of occupational exposure to blood and other potentially infectious materials, such as saliva, in the dental environment. Blood-borne pathogens, including the Hepatitis B virus, are serious threats to the health and well-being of health care workers and patients. This manual outlines policies and procedures which are designed to protect the clinicians and the patients from exposure to pathogenic organisms. Strict compliance with these guidelines will assure that treatment in the clinic will not jeopardize the health of patients or clinicians. Students and faculty members are classified as Category I according to the Centers for Disease Control classification system. In this category, personnel perform tasks which routinely involve the potential for exposure to blood, body fluids, tissue or other potentially infectious materials. The Program uses Universal Precautions as an approach to infection control. This approach treats all blood and body fluids, such as saliva, as if they are know to be infected with blood-borne pathogens. Universal Precautions are used for every patient and with every procedure to reduce risks to patients and clinicians. The Program is committed to ensuring that all students and faculty are working in a safe and healthy working/learning environment. Therefore, the policies are based on guidelines consistent with those issued by the Centers for Disease Control, OSHA and other related state and national health organizations. Since scientific information is prone to frequent change, the Program will update this policy annually, or as necessary, as new information on infectious diseases becomes available. It is the policy of the Program to review, evaluate, and respond on an individual basis to any instances of HIV/HBV or other infectious disease among faculty or students engaged in patient care activities. Applicants with Infectious Diseases Applicants who believe they are at an increased risk of contracting an infectious disease should seek testing and counseling prior to making application to the Dental Hygiene Program. Persons who are seropositive for HBV/HIV or other infectious diseases will not be excluded from application and admissions. However, applicants who are HBV/HIV positive may want to consider carefully their career goals. Points to keep in mind when considering dental hygiene as a career include: 1) As a person with a known debilitating disease, the individual may become disabled during the educational program or early in the career. 2.) Aspects of practice expose the person to infectious diseases. 3) Barriers to learning may have to be imposed due to certain invasive clinical activities. 4) Financial costs of the program given the personal health and career uncertainties. Faculty/Students with Infectious Diseases Persons who are seropositive for HBV/HIV or other infectious diseases will not be excluded from employment or restricted in their access to the institution s services or facilities because of their health status. They will be provided with all reasonable accommodations unless an individualized, medically-based evaluation determines that exclusion or restriction is necessary for the welfare of the individual or other members of the institution or patients. Patients with Infectious Diseases Persons who are seropositive for HBV/HIV or other infectious diseases will not be excluded from treatment in the dental hygiene clinic. The same Universal Precautions will be used in treatment of these patients. As with any complex medical condition, the patient s physician may be contacted to coordinate the correct dental hygiene treatment with the patient s other conditions. Confidentiality of Information The medical history of faculty, students and patients is kept confidential. Discussion of the history is limited to that information which needs to be conveyed to another professional to determine the correct course of dental hygiene treatment. All patient telephone consults are to be made from a quiet location such as the student desk area. Voice modification should be kept at a minimum. Immunizations All students who enter the Dental Hygiene Program must prove immunity to mumps, rubella, measles, tetanus, diphtheria, polio, hepatitis B and tuberculosis. This policy is in place to protect students from the

potential hazards of a preventable communicable disease. Students who elect not to be immunized must sign a declination waiver. Training in Infection Control Procedures No student will be allowed to deliver patient care in any setting until he/she has been instructed in infection control and has mastered the material by successful completion of written exam and process evaluation. Personal Protection in the Clinic Environment Personal Protective Equipment The following rules should be adhered to at all times when working intraorally or when exposure to blood, saliva or other potentially infectious materials can be reasonably anticipated. Disposable Gloves 1. Wear disposable vinyl or latex gloves 2. A new pair is used for each patient 3. Gloves that become contaminated ( dropped on floor, etc) during treatment should be discarded and replaced 4. Wear gloves when touching instruments, equipment or surfaces that may be contaminated. 5. Precautions during handling, use and storage of hazardous materials will be discussed, as well as general safety practices. Following is a partial list of hazardous chemicals which may be found in the dental clinic and labs. Fixer Developer Spray 2000 Cleaner Acidulated Phosphate Fluoride Sodium Fluoride Green Solution Bleach All Purpose Cleaner Phosphoric Acid-Sealant Etch Methyl Methacrylate Mercury Model Gloss Orange Solvent Isopropyl Alcohol Nitrous Oxide Gas Oxygen Gas Material Safety Data Sheets Material Safety Data Sheets (MSDS) are kept in a notebook in the Central Sterilization Room as required by federal law (Title w9 CFR Section ). These sheets provide the following information: a. Product identification (including the manufacturer s name and address) b. Hazardous ingredients c. Chemical data including boiling point, vapor pressure, water solubility d. Fire and explosion data including flash point, flammability limits and fire fighting procedures e. Health hazard data including immediate and long term effects, routes of entry, symptoms of exposure f. Handling and Use precautions Routes of Exposure Students and faculty are to be aware of the numerous routes of exposure for the many potentially hazardous chemicals in the dental environment. These routes include; inhalation, eye contact, skin absorption and ingestion. Safe Work Practices It is the responsibility of each individual to handle chemicals safely. Common sense underlies all safe work practices. Keep areas and counter tops clean and neat. Wear the appropriate protective clothing when in labs and clinic. Do not eat or drink in areas where chemicals are used. Keep containers clearly labeled and stored on sturdy shelving. Be aware of the location and use of emergency equipment and protocol. Emergency Equipment Emergency equipment to be used with chemical spills and accidents is available in the dental hygiene clinic. This equipment includes; 1) and eye wash station, 2) fire extinguisher and 3) first aid kit. The eye wash station is located on the CSR sink faucet and the first aid kit is located in the CSR. A fire extinguisher is located in the clinic on the wall near the restroom. Emergency Protocol

In the event of an exposure or a spill the student is to notify the faculty member immediately. The faculty member will render necessary first aid and make the decision to notify the emergency management system, if necessary. Protocol for Occupational Exposures Introduction An occupational exposure may occur in the following situations; 1. Contaminated needle stick 2. Puncture wound from a contaminated sharp dental instrument 3. Contamination of any obviously open wound, non-intact skin, or the mucous membranes by saliva, blood, or a mixture of the two. Protocol The following procedures apply if an occupational exposure occurs with either students or faculty: 1. Cleanse the wound immediately with soap and water. 2. Do not squeeze the wound to make it bleed. This will only cause tissue trauma which is more susceptible to infection. 3. In the case of student exposure, notify the supervising faculty immediately. 4. The faculty member will notify the patient that an exposure incident has occurred and request the patient s permission to have blood drawn for anti-hiv and HBSAg testing. This blood draw is to be done at the Pettis County Health Department as soon as possible after the exposure. The expense of the testing is borne by SFCC. 5. The exposed student or exposed faculty is to also have blood drawn for an anti-hiv and HBSAg testing. This blood draw is to be done at the Pettis County Health Department as soon as possible after the exposure. The expense of the testing is borne by SFCC. 6. Both the exposed student or exposed faculty and the patient will receive counseling prior to the blood draw by personnel at the Pettis County Health Department. 7. Follow-up procedures will be recommended by the Pettis County Health Department using current standards of care and based upon the nature of the incident and the results of the blood tests. 8. An Incident Report should be completed within 24 hours of the exposure. Exposure Prevention 1. Handle all used instruments with nitrile gloves. 2. Always use a sterile syringe and a new disposable needle and cartridge for each patient. 3. Handle needles carefully. Do not leave needles uncapped. 4. Recap needles using a needle guard or scoop technique. So not use a two handed technique. 5. Do not bend or cut needles. 6. Put all used needles in a sharps container as soon as possible after use. 7. Do not overfill the sharps container. 8. Never reach into a sharps container.

Operating and Safety Procedures for Dental Radiographs The following policies are to be followed in order to ensure a safe environment for all students, patients and personnel. Selecting Patients for Radiographs 1. Radiographs will only be made on those patients who present with a diagnostic need for the films. 2. Radiographs will be prescribed by the Supervising Dentist. In the case of films for radiology lab, these patients are to be patients who will be seen in the clinic and the Supervising Dentist has approved the films. 3. The need for radiographs is determined following guidelines established by the Department of Health and Human Services. Equipment and Film Use 1. Students are not to be exposing film without a faculty member present. 2. Students/faculty are to wear film badges at all times when in the clinic or radiology lab. 3. Students/faculty are to stand behind the protective barrier when exposing the radiographs. 4. Students/faculty are not to hold films in a patient s mouth during exposure. Film holders are used for this purpose. 5. Equipment is inspected on a regular basis as required by the State of Missouri. The results of these inspections are posted by all radiology units. 6. All patients are to wear lead aprons, including thyroid collars. Aprons are to be kept on the apron bars when not in use. Do not fold aprons. 7. Any student suspecting she may be pregnant is to report this information to the Dental hygiene Coordinator so that arrangements for limited exposures can be made. These arrangements will include wearing a lead apron during any radiograph instruction until a later time in the student s educational process. Film Processing 1. All films are to be carefully processed in order to limit the need for retakes due to processing errors. 2. Chemicals are to be kept fresh in the processor by following the chemical change chart posted in the CSR. 3. Gloves are to be worn anytime maintenance on the processor is being done. This includes the emptying of trash from inside the daylight loader. 4. Gloves are to be worn while handling films during processing. 5. A quality assurance film will be made monthly after cleaning processor. This will be done by either the radiology lab instructor or the clinic assistant. These films are to be labeled and kept in the radiology log book. The log book contains information regarding the change of chemicals and a record of all quality assurance films. Retakes of Films

All retakes must be approved by a faculty member. Retakes will not be recommended simply to make the film more attractive or raise a student grade. Retakes will be made only if the area to be viewed is not seen on any other films. Emergency Policies Introduction An emergency situation may occur at any time or place. Most serious emergency situations can be prevented by taking a careful pre-treatment medical history. All questionable patient conditions should be discussed with the supervising dentist before beginning any dental hygiene data collection or treatment. During an emergency, optimum patient care can be given only by calm, efficient personnel. This clam results from being prepared. A regular review of the emergency procedures and location of the emergency equipment will be conducted at the beginning of each clinical course for students and faculty. Monitoring of Equipment The Supervising Dentist will check the emergency kit each year prior to the beginning of the academic year to determine completeness of the kit and non-expired medications. The Program Coordinator will determine the functionality of the eye wash station and oxygen equipment annually at the beginning of the academic year. The fire extinguisher will be monitored annually by the fire marshal and the first aid kit will be replenished by lab faculty as needed. A Medical Emergency Situation in Clinic In the event of an actual emergency situation the following steps will be followed: 1. Tell the nearest student, I have a STAT situation. The fellow student will notify the faculty. 2. Assure the patient s airway. 3. Check for breathing and pulse. 4. Remain with the patient, calming and reassuring them. 5. Do not leave the patient. 6. Faculty will come to area and determine the need for notification of emergency medical system. 7. Faculty will direct a student to notify the emergency medical system using the clinic phone. 8. After notifying the emergency medical system the student will got to the outside of the building to direct the EMS team into the area. 9. During this time the clinic assistant is to be locating and bringing to the area the oxygen tank and the emergency kit. 10. If CPR is needed the faculty will administer. 11. All other patients in the clinic are to be kept calm during an emergency situation. If it appears necessary, other patients in nearby units may be asked to move away from the area. Students not directly involved in the situation are to stay with their patients and reassure them that the situation is being managed. Emergency Equipment in Clinic 1. Eye wash station located next to each operatory and in the CSR. 2. Fire extinguisher located near the restroom. 3. Oxygen tank located in the clinic storage area. 4. Emergency kit located in the CSR. An Emergency Situation in the Laboratory In the laboratory the students are to notify the faculty member if an emergency arises. The faculty member will provide emergency first aid and determine the need for EMS. Students will be asked to assist the faculty member in securing equipment, phoning the EMS team and, if

necessary, providing first aid. Students are to be prepared to do so by knowing the location of the nearest equipment and the correct procedures. Emergency Equipment in the Laboratory 1. Eye wash station located at the sink 2. Fire extinguisher located near the doorway. 3. First aid kit located in the instructor cabinet. Policy on Basic Life Support Training 1. All faculty teaching in the Dental Hygiene Clinic at SFCC must hold a current CPR certification card. 2. All students providing treatment in the Dental Hygiene Clinic at SFCC must hold a current CPR certification card. Students will be able to gain certification as part of the Medical Emergencies class offered in the first semester of the dental hygiene curriculum. 3. Any faculty member or student unable to perform CPR, as indicated by a disability or physician s statement, will be exempt from completing CPR training. 4. A copy of the current CPR card must be on file in the Program Director s office upon beginning the dental program curriculum or beginning a faculty appointment. 5. An audit of the CPR cards will be made at the beginning of each academic year to ascertain the completion of CPR by all eligible students and faculty.
