2 Dear Family Member/Patient Representative, You are about to make an important decision about the long term care or rehabilitation needs for your family member/friend. This can sometimes be an emotional decision. To help you through this process, we have developed this informational packet to answer some of the questions you may have. During this process you can expect the following actions to occur:? First, your case manager will complete a Patient Review Instrument also referred to as a PRI. This will help the facilities to which you apply to determine the needs of your loved one and the most appropriate level of care that is needed (skilled, acute rehab, custodial, or proprietary level of care). o o o Skilled Care- Care that needs to be provided by a licensed health care professional (nurse, physical or occupational therapist) on a daily basis. Examples include wound care, physical, occupational, and speech therapy. Rehab care can be provided within an acute program or within a skilled facility. Custodial Care- Care that includes assistance with activities of daily living (ADLs) such as bathing, feeding, toileting, and dressing. This level of care does not need to be provided by a licensed health care professional, but it is supervised by a licensed health care professional. Proprietary Care- Care that includes some assistance with bathing, dining, and medication administration. Your loved one will need to function at a level that does not require continual assistance for tasks such as toileting and eating meals.? Next, we will determine if our Medicaid Liaison needs to be involved to assist you with the Medicaid application process. This decision is based on the individual financial status of the patient.? You have been provided with a list of area skilled and/or proprietary facilities. Please choose at least five facilities you wish us to make referrals to and number them in order of your preference. We will make every effort to accommodate your requests, but please understand that no guarantees can be made as Lakeside is held to the rules and regulations of Medicare and Medicaid regarding the placement of patients. If a bed offer is made and you refuse that bed, it may jeopardize your insurance coverage in the hospital.? Financial applications are required by most area facilities and must be completed as soon as possible. Please give the completed applications to the social worker and/or case manager you are working with or to the facility to which the referral has been made. All financial information is kept strictly confidential and is required to expedite the placement process.

3 ? The social worker or case manager will then proceed with referrals to the facilities you have chosen. Again, once a bed offer is made we must act quickly in order to secure that bed and avoid insurance coverage problems. that? Please understand that we may have to request additional facility names in the even the facilities you have initially requested are unable to offer a bed to your loved one.? Homecare services may also be an option for your discharge from the hospital or later on from a skilled or rehab. facility. A referral can be made to the agency of your choice based on the location you will be discharged to (home setting or assisted living facility). We are here to answer any further questions you may have. The decision to place a loved on in a facility is never an easy one, however we are available to assist you through this process.
