2 Our Commitment to Performance Quality Improvement Family Outreach Center has made a commitment to building a quality management system that incorporates Performance Quality Improvement (PQI) principles, structures and processes. FOC s emphasis on PQI is a mechanism used to determine the achievement of the agency s mission and to keep the leadership and governance body informed thru the establishment of expectations, the monitoring of outcomes and service coordination, and a way to be apprised of consumer concerns. mental health services offered in English and Spanish Family Outreach Center s Outpatient Mental Health & Substance Abuse programs provide counseling services to address mental health and substance abuse treatment for children, youth, adults, and families who reside in Kent County.

In 2013 FOC received ACE Accreditation to provide quality training in the community. We have also seen an expansion of services and are fortunate to be a part of the Community of Practice model being implemented by the Urban Core Collective (UCC). 4 mental health.... let s talk about it mental health and addiction services Family Engagement Therapy works with women who have school-aged children in their care and have significant substance use issues. It is a community based program that strives to help individuals and families thrive in their environment. The program works with various community agencies to assist women and their families in how to access and utilize community resources. The program continues to help women develop positive community relationships to GENDER Female 1,114 Male 1,163 AGE GROUP In 2013, Family Outreach Center served 2,036 unduplicated clients. Without the services provided by the mental health staff at KCCF, individuals with mental illness will be returned to the community with little to no improvement in their mental health condition and without connection to appropriate community services.

6 adult case management Family Outreach Center s Native American Community Services Program (NACS) is designed to reach out and assist Native American individuals and families with mental health issues, substance use disorders, and/or co-occurring disorders. Our goal is to increase service availability and community-based supports that will help and improve the overall functioning of these families. Services are available to all Native American individuals and family members regardless of tribal affiliation. General Comments: Something I learned about myself was talking with my therapist is how to channel my emotions. Learning that it is getting easier for me to talk about my situation, but I still need to work on my communication

8 mental health.... let s talk about it children s mental health services The In-Home program provides Home-Based, Case Management, & Trauma-Focused Cognitive Behavioral Therapy to over 150 families on a monthly basis. Our services are treatment focused with the goal of preventing the out-of-home placement of children. School Based Outreach and Kent School Services Network. The clinicians working in these programs provide counseling and case management services for children and families in schools or in their homes. Access to mental health and other community based services has significantly increased for this population as a result of neighborhood based on-site availability. Prevention and education groups, class room presentations and other resources are also offered thru this community school model. Project C.H.I.L.L. (Children receiving Help In Living and Learning) provides services for children between the ages of six and seventeen, whose parents are receiving mental health services at Family Outreach Center or other behavioral health providers. Our goal is to teach resiliency skills and techniques to reduce risk and the likelihood of these children exhibiting symptoms in the future. we re here to listen.... reaching out to strengthen family life

9 mental health services Family Outreach Center also offers the following community-based services: Community Liaison/ Resource Connecter Services provide low intensity case management assistance and referrals for consumers. Doctor Services provides assessments, medication reviews and support for adults and children served by the agency s State licensed and board certified psychiatrists. Taraji (Faith and Hope) Community Outreach Services is an agency program designed to bridge the gap between the mental health system and the church community, incorporating the principles of faith and traditional counseling methods.

Family Outreach Center was formed as a result of the initiative of community leaders who were concerned about the deterioration of family life within segments of the population residing in the inner-city of Grand Rapids. Our Board of Directors has formulated the following mission statement for Family Outreach Center which reflects our desired impact: Family Outreach Center is here to lead the way in promoting healthy family living by reaching out to underserved we re here to listen.... reaching out to strengthen family life populations and those who are low-income in our community.

Some are open to the general public. Others are culturally specific.
