2 MISSION STATEMENT Asian Pacific Health Care Venture, Inc. is a community health center whose mission is to advocate for and provide health care services to all persons in a culturally competent manner. We offer service with a particular focus on low-income families and underserved Asians and Pacific Islanders. We also offer programs of health education and community economic development within our catchment area as well as other regions in Los Angeles County. 3 BRINGING HEALTH CARE TO THE SAN GABRIEL VALLEY When APHCV first began providing direct health care services in 1997, a tough decision had to be made, where do you establish a health center? Los Angeles is a vast and diverse community, especially when you look at the different Asian ethnicities and how far they are spread out. From the Thai community in East Hollywood, Pilipinos in Echo Park, Cambodians in the South Bay, Vietnamese in the San Gabriel Valley, and everywhere in between the Asian community is spread far and wide, each with their own unique cultural and health care needs. APHCV does its best to meet those needs and has long provided a medical home for the community at the APHCV-Los Feliz Health Center, but that wasn t enough. In March 2012, APHCV opened the APHCV-El Monte/Rosemead Health Center bringing a medical home to patients living in the Rosemead, El Monte, Monterey Park and greater San Gabriel Valley. Bilingual and bicultural support is provided throughout the continuum of care in Chinese, Vietnamese and Spanish. All language access is provided in-house to ensure that patients receive accurate and comprehensive information regarding their health care. In addition to comprehensive family practice health care, the APHCV-El Monte/Rosemead Health Center provides enrollment assistance to families who may qualify for public health programs such as Medi-Cal, Medicare, Cal Fresh and WIC. The new health center helps eliminate barriers to accessing health care by bringing a medical home right to our patients in the San Gabriel Valley. IMPROVING QUALITY CARE FOR THE UNDERSERVED APHCV believes that all members of our community deserve to have the best medical teams providing their care, latest treatments and medications to bring people back to health, and the latest technologies to ensure patients have the information they need to stay healthy and the tools to track and improve their health outcomes. In 2012, APHCV launched its electronic health records (EHR) system, transforming the way we deliver care. Clinicians and members of the patient health care team now have complete medical records with laboratory results, prescribed medications and general health history available at their fingertips. Patients can see how their health is progressing by looking at color graphs tracking disease trends and treatment outcomes. EHR provides the community served by APHCV s health centers with the benefits of cutting edge medical technology that everyone deserves. Over the last 3 years, APHCV has been prepping for EHR implementation and this year, the efforts of that hard work came to fruition with staff at all APHCV health centers pulling together to learn the new system and get patients the timely health care they need. The availability of data to track general health and treatment trends across patient populations has vastly improved with EHR making APHCV more efficient and improving the quality of care for our patients. BEING HealthWISE APHCV has partnered with the Asian Pacific Family Center (a division of Pacific Clinics) on an innovative project to create a multidisciplinary health care team, joining primary care physicians with experienced behavioral health care professionals. More and more we are learning that being healthy is sometimes more than just eating healthy, exercising regularly, and following the doctor s orders. Sometimes there are underlying issues that prevent an individual from taking care of themselves, whether that is stress or anxiety or something more serious. Through this new HealthWISE program, medical and mental health providers work side by side caring for each patient to ensure they are receiving the care they need to lead a healthy life. As part of an integrated healthcare team, both primary care and mental health providers can identify the individual issues of each patient that are preventing them from living a healthy life. Together they come up with a care plan to ensure patients get the types of medication, counseling and support they need to be successful in their recovery and overall health. The HealthWISE program is a pilot project funded by the County of Los Angeles, Department of Mental Health focused on providing care for the underserved Chinese community in the San Gabriel Valley and will continue through 2015.
