Initial Accreditation Programs in this category have completed an application and self-study and had an on-site evaluation. The length for initial accreditation is eighteen (18) months to three (3) years. No action taken in the category of Initial Accreditation Continuing Accreditation - Programs in this category have completed an application and selfstudy and had an on-site evaluation. The maximum award for continuing accreditation is eight (8) years. A Progress Report to demonstrate compliance with outstanding citations may be requested by the Board of Directors. Probation Programs in this category have been determined to not be in substantial compliance with the relevant accreditation Standards. Probationary Accreditation is usually limited to one (1) year. It may not extend beyond two (2) years. No action taken in the category of Probation. Voluntary Withdrawal of Accreditation Programs in this category have voluntarily withdrawn from the JRCERT accreditation process. Everest University/Brandon Campus - Radiography Program 3924 Coconut Palm Drive Tampa, FL Initial Accreditation Granted - 10/16/2009 Program Closed 10/31/2015 Involuntary Withdrawal of Accreditation Programs in this category have been found to be in substantial noncompliance with one or more relevant Standards. The Award Letter link for the applicable program may be accessed at the JRCERT Web site. This action may be appealed. Consistent with JRCERT policy, during the appeal procedure, the program returns to its previous accreditation status. No action taken in the category of Involuntary Withdrawal of Accreditation.

6 Substantive Changes Programs in this category are requesting review of changes that are classified as substantive by JRCERT Policy No action taken in the category of Substantive Changes Teach-Out Plan or Agreement - Programs in this category will be closing. The Teach-Out Plan must be approved by the JRCERT Board of Directors and indicate how the program will maintain compliance with the Standards until closure. In conjunction with a Teach-Out Plan, a Teach-Out Agreement must be approved by the JRCERT Board of Directors and ensure that students are given reasonable opportunities to complete their education at another JRCERTaccredited program.
