4 Benefits: LIFE offers comprehensive medical care, adult day health services and home care at no charge to qualified older adults. Based on an individual s needs, LIFE can provide: comprehensive medical management by an interdisciplinary team of healthcare professionals; an on-call nurse and physician available 24 hours a day; adult day services including recreational programs and socialization; personal care; home care; housekeeping and laundry; rehabilitation therapies; transportation to the LIFE center and medical appointments; medications and medication management; meals/assistance with meal preparation; equipment for safety in the home; and dentistry, optometry, and audiology. LIFE partners with families to provide support and assistance in caring for their older loved ones. LIFE Butler County is a partnership of Lutheran SeniorLife and Butler Health System. 13 Intellectual Disabilities Resources Program: Intellectual Disabilities/Mental Retardation Supports Coordination Services Eligibility: Documented diagnosis of Mental Retardation (IQ 70 or below) prior to the age of 21 and limitations in adaptive functioning Benefits: Supports Coordination Services link to home and community based services and supports. Consolidated and Person/Family Directed Supports Waiver program applications can be completed. Waiver funding or County funding is needed to enroll individuals into Provider programs. There are waiting lists for services so families and consumers are encouraged to plan for future needs.
