6 BANDO RICERCA FINALIZZATA-GIOVANI RICERCATORI COMITATO VALUTAZIONE GIOVANI RICERCATORI Elenco Motivazioni GR IDENTIFICATION OF NEW GENES FOR RARE NEURODEVELOPMENTAL DISORDERS BY HOMOZIGOSITY MAPPING/NEXT GENERATION SEQUENCING AND FUNCTIONAL STUDIES IN ZEBRAFISH This project focuses on use of a multimethod approach to discover novel causative genes in neurodevelopmental disorders. The topic is relevant to National Health System and appears feasible. The methodology includes homozigosity mapping, NGS and Zebrafish model. Beside the limited originality to use NGS to identify novel responsible genes, the impact of the outcome make the study a potentially important project. The rationale is well defined, the facilities and the budget are adequate. Strenghts include the preliminary data, focusing on a similar exploratory-study, and the adequate profile of the PI and his team/collaborators (UO1 and UO2). Weakness include the poor information about the specific selection of families to be enrolled in the study. The impact of the identification of novel genes responsible of neurodevelopmental disorders could be important for a better diagnosis and therapeutic approach in the future. After reviewing the scores and the comments of both the reviewers, the committe agree with the score of reviewer 2 and agree with a final score of 10. Final Score: 10 GR Role of type I IFN in dictating the immunogenicity and therapeutic efficacy of chemotherapy: from animal models to humans The objective of the present proposal is to investigate whether the immunogenic potential and, consequently, the therapeutic success of defined chemotherapeutic drugs resides in the early induction of type I IFN within tumor bed and to elucidate the underlying molecular mechanisms. The PI has a very good publication track record and exceptional collaborators. Istituto Superiore di Sanità is an outstanding environment for the proposed studies. Budget is appropriate. After reading the scores and the comments of the reviewers, the committee agrees with the final score of Final Score: 14,

Final Score: 18,25 GR Exploring the role of macrophages in physiological bone healing and their potential for innovative bone regeneration therapies. Status F2F: Confirmed The commitee reviewed the F2F assessments and elaborated the final scores being more consistent with the guidelines. The scores assigned by the reviewer #1 do not match the comments. Many criticisms have been noticed, mainly regarding preliminary dataand methodologies. The overall hypothesis is not clear, affecting the scientific quality of the study. Therefore, the committee has decided to validate the average between the scores. Final Score: 13 GR Periventricular Heterotopia: identification of new causative genes through massive parallel sequencing and RNA interference. Delta score is high because both reviewers have used high scores, making the difference to look higher. Nonetheless comments are similar and there are no anomalies therefore committee agrees on the average score. Final Score:

GR Regulation of the linear ubiquitin chain assembly complex by IAPs in the metastasization process of cancer cells scores. Final Score: 14,75 GR An integrated approach to clonal mast cell disorders scores. 13 GR Functional analysis of SCN1A mutations with ips cell derived human neurons: assay for clarifying genotype phenotype correlations and develop targeted therapeutic approaches. scores. Final Score: 27,5 GR Unraveling the genetic complexity of myeloproliferative neoplasms and the molecular pathways involved in disease progression The present proposal aims to perform whole-exome sequencing to identify mutations in Jack-, Mpl-, normal karyotype MPN samples. The studies may lead to the identification of novel mutations that contribute to progression to AML. Overall the proposal has been well written. Nevertheless, two major concerns have been raised. First, it is not clear whether the Applicant will be able to reach a significant number of samples that may support any statistic significance. Second, the idea of performing whole exome sequencing may be a limitating factor, since putative relevant mutations may occur in noncoding regions. The PI responsible for the submitted proposal has expertise in the specific field, as documented by the publication in medium-impact factor Journals. The Institution is adequate for completing the proposed studies. The Committee has agreed on a final score of 13.5, as in the proposed F2F score. Final Score: 13,5 GR BASAL GANGLIA FUNCTIONAL ABNORMALITIES CORRELATING WITH GAIT DISTURBANCES IN PARKINSON'S DISEASE. TOWARD NOVEL BRAIN SIGNAL CONTROLLED DEVICES FOR ADAPTIVE DEEP BRAIN STIMULATION. The present study aims to develop deep brain stimulation (DBS) patterns to prevent and inhibit freezing of gait (FOG), which is one of the major disturbances in Parkinson s Diseases. The applicant should clarify what is meant by adapting DBS to moment to moment clinical states. If successful, the proposal could eventually contribute to improvements in the efficacy of DBS in reducing freezing of gait and improving outcomes in Parkinson's disease patients. The proposal is well written and has logical aims. The novelty of the study relies in the development of a system that is responsive to freezing of gait. Facilities and infrastructure are appropriate for the proposed studies. Budget is also appropriate. The applicant is a young investigator at the first year of the PhD training. She is first author in several publications related to the topic of the proposal. Complementary expertise will be provided by the assembled team. Despite the high potential of the candidate to 720 8

Final Score: 24,25 GR Intellectual developmental disorders in Friuli Venezia Giulia Region, Italy: prevalence, burden of disease, pathways of care, services and sources of information. TL clearly has not read the project! The general idea of studying prevalence of intellectual developmental disorders in 0-17 years old children can be interesting but the methodologies and aims are unclear. Applicant has a good CV and publication records but not the same for co-workers. The committee proposes to accept the evaluation of the second reviewer. Final Score: 21 GR An integrated biomolecular, neuroradiological, and clinical study to investigate brain development, onset and progression in Huntington s disease scores. Final Score: 15,

GR Genome wide screening in patients with Ruptured and Unruptured Intracranial Aneurysm: a pilot study to identify genetic variants associated with risk of aneurysm rupture. scores. Final Score: 13,

GR Development of an engineered human ipscs derived bio pacemaker as an in vitro platform for drug screening This is a potentially interesting proposal that aims to verify the feasibility of in vitro generating human ipsc-derived engineered bio-pacemakers and their suitability as platforms for drug screening in order to test cardiotoxicity or proarrhythmic actions. Nevertheless, the overall interest is lowered by the lack of clarity in regards to the working methodologies and experimental procedures that appears to be not well focused, nor well described. The PI responsible for the submitted proposal is a Young Investigator that has shown proven to be a promising researcher as documented by the relative track of publications listed within the application. The Committee has agreed on a final score of Final Score: 17,5 GR Hospitalization and comorbidities in patients with parkinsonism and / or dementia: a study of prevalence and management proposals scores. Final Score: 22 GR Guidelines for the identification of quality standards for the realization of extra hospital structures for subjects in Vegetative or Minimally Conscious State The two Reviewers were concordant about the limitations of the publication background of the PI. The possibility to recruit a sufficient number of cases to run a reliable analysis for the construction of national guidelines remains questionable. After reading the scores and the comments of the Reviewers, the Committee accepts the proposed score issued from the face-to-face agreement of 23. Final Score: 23 GR Genetic basis of steroid resistant nephrotic syndrome and implications for therapy Status F2F: Rejected After reviewing the scores and the comments of the reviewers, the committe agree with the average final score of 12,5. The expertise of the applicant in the field is very good but the publications record (excellent for one paper) is limited. The project is ambitious and it is not well hypothesis driven, as indicated by the reviewer 2. Final Score: 12,

GR Role of micrornas in modulating platelet function in patients with acute coronary syndromes. the evaluation board agrees on accepting as final the aproposed F2F assesment Final Score: 11,5 GR Melanopsin retinal ganglion cells and circadian photoentrainment in Alzheimer's disease, Down syndrome and aging After reading the scores and the comments of the reviewers, the committee agreed with the score proposed by the reviewers in the advanced F2F forum. Final Score: 20 GR Early prediction of outcome in comatose patients with severe traumatic brain injury: evaluation of brain activity with 18F FDG PET scores. Final Score: 17,5 GR Impulse control disorders in Parkinson s disease. A neurophysiological study of cortical inhibitory mechanisms, functional connectivity, and volitional control of actions scores. Final Score: 28,

GR Testing the competition for c Kit ligand between mast cells and stem cells: implications for treatment with tyrosine kinase inhibitors in prostate cancer After reading the scores and comments of the two reviewers the commette consider to accept the averaged score Final Score: 14,75 GR IFNgamma as a potential therapeutic target in secondary HLH scores. Final Score: 12,5 GR Human Cerenko mammo tomography using beta emitting radiopharmaceuticals scores. Final Score: 13,5 GR Imaging of bone marrow sympathetic innervation. After reading the scores and the comments of the reviewers, the committee agreed with the final proposed F2F score Final Score: 19 GR The impact of aerobic exercise on cytokine production in patients with multiple sclerosis. The two Reviewers agreed over the scientific quality of this application, and were finally concordant over the validity of methodology, novelty and merits of the applicant. Therefore, after reading the scores and the comments of the Reviewers, the Committee accepts the proposed score issued from the face-to-face agreement of 15. Final Score:

GR Insights into the molecular mechanisms of Anorexia Nervosa Although delta score is high there are no anomalies in the comments and the committee agrees on a score which represents the mean of the two scores. Final Score: 20,75 GR GRADE Dispute. Improving appropriateness and transparency of processes to develop guidelines on controversial clinical areas: evidence, values and context preferences to help mitigate disputes and enhance the applicability of recommendations to practice. scores. Final Score: 10 GR Preclinical optimization of treatment with inhaled argon to improve neurological outcome and survival after cardiac arrest scores. Final Score: 9,75 GR Mineral metabolism and microvascular function in patients with chronic kidney disease This proposal aims to investigate the role of 1,25dihydroxyvitamin D (1,25(OH)D) and Parathyroid Hormone in the pathogenesis of cardiovascular events in patients with chronic kidney insufficiency. Although this project has interesting ideas, the study design is extremely weak. For instance, patients have to be stratified according to the different etiologies of the renal disease as well as to other independent cardiovascular risk factors. Therefore a sample size of 20 appears to be insufficient to reach a significant statistical power. PI is MD, resident in nuclear medicine, with only 1 paper in the same field of the proposal. No senior authorships, raising the concern on the effective leaderships in this proposal. After reviewing the scores and the assessments of the reviewers, the commission agrees with the final score of Final Score: 21,

GR Age induced reduction of neurogenesis induces functional alterations of the blood brain barrier contributing to the detrimental outcome of stroke in the elderly. scores. Final Score: 10 GR Role of the combined ultrasonographic imaging and neurophysiological study for surgical treatment of peripheral nerves pathologies scores. Final Score: 21,75 GR Human liver steatosis: potential mechanisms linking lipogenic enzymes, gut peptides and intestinal permeability The Committee has evaluated the scores provided by the two Reviewers. The working methodology has not been properly defined. The proposal lacks of complete novelty. Both Reviewers have properly discussed and addressed all their concern and the Committee has agreed on the final score of 21, as proposed in the F2F evaluation. Final Score: 21 GR Electrophysiological, behavioural and functional connectivity markers of fluctuating cognition and attention disorders in Dementia with Lewy Bodies and Parkinson s Disease with Dementia scores. Final Score: 10,

GR Toward identification of biomarkers in patients with irritable bowel syndrome (IBS) scores. Final Score: 19,75 GR Can default and attentional network dysfunctions lead to Visual Hallucinations and Delusions in Lewy body disease? This project aims to investigate the aetiology of visual hallucinations (VH) in Lewy body diseases (LBD) using an original combined MRI/fMRI/DTI/MRS methodology. The proposal was evaluated by two reviewers. Both reviewers found that the proposal presents good resources and facilities. They also found the project innovative and well written. One of the reviewer found in the proposal some weaknesses that include the unclear role of the units for some aim and the uncertainity of the therapeutic outcome. The committee agrees that these weaknesses limit the impact of the proposal and also notes that the proposal might benefit from more experienced collaborators, more articles as first-author of the applicant and a clearer evaluation of the study design in the proposal. For the above reasons the committee suggests a final score of Final Score: 14 GR Association between an increased systemic inflamation level in periodontitis and cardiovascular diseases: a randomized protocol. After reviewing the scores and the assessments of the reviewers, the commission agrees with the final score of 32. Final Score:

GR Molecular and functional characterization of an HDAC regulated mir network that controls the composition of the chromatin remodeling complex muscle regeneration versus fibro adipogenic degeneration in neuromuscular disorders The aim of this proposal is to investigate the molecular mechanism behind the beneficial role of HDACs inhibitors in muscle regeneration. The rationale and hypothesis of the project are not clear. The proposal needs to be widely amended pointing out the strengths and weaknesses of the proposed strategy, defining relevant specific aims and describing the methodology more specifically (i.e. how the RNAseq data will be generated?), as well as discussing the impact in translation. After reading the scores and the comments of the reviewers, the committee agrees with the final score of Final Score: 20,5 GR NOVEL BIOMARKERS IN MALIGNANT PLEURAL MESOTHELIOMA: FROM MOLECULAR PREDICTORS OF RESPONSE TO THERAPY TO NOVEL MULTIMODAL THERAPEUTIC STRATEGIES Status F2F: Rejected It emerges from the face-to-face forum that the two Reviewers were actually concordant over the final score to be attributed. Therefore, after reading the scores and the comments of the Reviewers, the Committee agrees to accept the proposed score issued from the face-to-face forum of 11. Final Score: 11 GR In vivo and ex vivo translational study of the effect of Metformin on brain metabolism: implication for Alzheimer's disease pathophysiology scores. Final Score: 13 GR Deep Brain Stimulation and EEG fmri for drug resistant epilepsies: assessment of the role of subcortical brain structures in the neuro modulation of epileptic networks. scores. Final Score: 17,
