The Employee Health, Wellness and Benefits website should be your first resource; and you are certainly welcome to call our office with questions or to schedule personal appointments. Action during this year s open enrollment will only be necessary if you are making changes to your health insurance and/or enrolling in the flexible spending accounts. How you make changes/enroll might be different depending on the implementation of the Oracle Self-Service module and we ll keep you posted over the summer by mail and . Our website is updated as details unfold and as we develop even more resources to help you make your decisions. The chart below outlines the approved changes to plan design. A 9% premium increase was also approved and premiums for active employees and retirees are being developed; they will be posted to our website by the end of June. There is a direct connection between our Wellness Works! program offerings and health insurance costs. In general, healthy, happy employees cost less and early intervention and proper management of chronic diseases have similar benefits. 2 FAQs: Are you Retiring This Year? P A G E 2 If you are eligible for and considering electing retiree health insurance through LCPS, here are some frequently asked questions that may help you get a head start on your enrollment. Am I eligible for Retiree Health Insurance? You may be eligible for retiree health insurance if you are retiring and immediately collecting your VRS pension, have at least 15 consecutive years of full-time service and have been covered by an LCPS medical plan for at least the 3 years preceding your retirement. I am eligible for Medicare and enrolled in Medicare Part A. Should I enroll in Medicare Part B? If electing retiree health insurance through LCPS, Medicare-eligible retirees must enroll in Medicare Parts A&B and are provided a Medicare Supplement plan for their medical coverage. Be sure to start your application for Medicare Part B at least 2-3 months in advance of your active health insurance end date and provide a copy of your Medicare card, when received, to Employee Health, Wellness and Benefits. Any dependents eligible for Medicare must also enroll in Medicare Parts A&B, and meet the LCPS eligibility requirements, to be covered by the LCPS retiree health insurance plan. Will Medicare allow me to enroll at this time? If you were covered by a creditable plan and are losing coverage due to your retirement, you should be eligible for a Special Enrollment period with Medicare. You may be required to have your employer complete a Request for Employment Information form for Social Security/Medicare to verify your prior coverage and the fact that your active coverage is terminating. Please provide this form to Employee Health, Wellness and Benefits for completion. If I am enrolled in Medicare and the Cigna Medicare Surround plan, which medical insurance is primary? Medicare will be primary for your medical coverage and you will need to visit doctors and facilities that accept Medicare assignment and follow Medicare guidelines. Only Medicare approved expenses will be considered by the Cigna supplement plan. Will there be any change to my prescription plan? If you are a Medicare eligible retiree and enroll in retiree health insurance, you will have a new prescription drug provider, Cigna HealthSpring. If you are using mail order pharmacy, you will need to move your prescriptions to a Medicare D participating mail order pharmacy. A list of these pharmacies will be mailed to you when you are enrolled and you can submit your prescription beginning your effective date of coverage in the Cigna HealthSpring Rx Plan. Please allow 30 days for prescriptions to be set up and delivered to you. For retail prescriptions, simply take your new card to the pharmacy for your fill. Non-Medicare eligible retirees will continue to be covered by Express Scripts and no change to mail order prescriptions needs to be made. Will I have dental and vision coverage if I am Medicare-eligible? Yes. You will be enrolled in the Cigna Vision plan for your corrective vision benefit and will continue to be covered by Delta Dental for your dental benefit. I am not eligible for Medicare. What plans will I be enrolled in? If you are an eligible retiree under 65, and are not Medicareeligible, you can elect to continue on the same plans in which you are currently enrolled but as part of the retiree group. All plans are bundled for retirees so if retiree health insurance is elected, you will receive medical, dental, vision and prescription drug coverage. How do I enroll in Retiree Health Insurance: All retirees eligible for retiree health insurance must complete the Retiree Health Insurance Enrollment/Change form to elect retiree health insurance. Your Plans if Enrolled in LCPS Retiree Health Insurance Medicare Eligible (age 65 or otherwise eligible for Medicare) Medical: Medicare (primary) and Cigna Medicare Surround (secondary) Prescription Drug Plan: Cigna HealthSpring Rx (a Medicare Part D plan provided by LCPS) Vision Plan: Cigna Vision Dental Plan: Delta Dental Not Medicare Eligible (under 65) Medical: Cigna POS or OAP (whichever plan you currently have) Prescription Drug Plan: Express Scripts Vision Plan: Davis Vision Dental Plan: Delta Dental Benefit End Dates for Retirees Wondering when your health insurance benefits will end if you retire at the end of the school year? As long as you complete your contract period, your health insurance benefits should end as follows: 10 month Employees: August 31, month Employees: July 31, month Employees: End of month in which employment ends, in many cases June 30, 2015

When a claim is filed under your Cigna benefits plan, you get an EOB. It shows you the costs associated with the medical care you ve received. Knowing these numbers is an important part of being, and/ or getting healthy AND these numbers are mandatory for successful completion of the Cigna Health Assessment, which is required for the 2015 Wellness Works! Passport Program. To assist you with KNOWING YOUR NUMBERS - Inova Mobile Health Services will be available every month in 2015 to LCPS employees. 7 P A G E 7 LCPS Fitness Center Membership Reimbursement Program March 1, May 31, 2015 Starting or staying with an exercise routine isn t always easy. To help you stay motivated and achieve your fitness goals, the LCPS Employee Health, Wellness and Benefits office is providing reimbursement towards fitness membership fees for up to three months. You can be reimbursed up to $50 a month for up to three months ($150.00!), as long as you attend at least 12 times in each month you request reimbursement. All fitness center visits must occur between March 1, 2015 through May 31, This reimbursement is only available to active, full-time LCPS employees and you must be in an active status at the time of reimbursement. We are encouraging you to run/bike a 5k! If you successfully complete a 5k and provide the proper proof between the dates of May 1, August 31, 2015 (all materials must be submitted by close of business on August 31 st ), the LCPS Wellness Works! Program will reimburse the cost of the 5k registration fee, up to $30.00 (one time only). In addition, if you are running/biking a 10k, half marathon, full marathon, mud run or another similar type event, that counts too! We would love to share your success stories.
