We realize that benefits are an important part of financial security and well-being of our employees and their families. Some of the benefits are provided automatically to employees at no cost. Other benefits allow for choice of a type and level of coverage from among several options. Please consult the following brief summary of each benefit, benefit materials, and Summary Plan Descriptions for more information on each of the benefits. If you chose not to enroll upon hire, you will have another opportunity to enroll during our annual open enrollment period in April-May, 2016 (coverage would not be effective until July 1, 2016). Benefits elections may be changed outside the open enrollment period if a person experiences a life status change, i.e. birth, death, marriage, divorce, change in your employment status or an employment status change for your spouse at another employer. Changes must be made within 30 days of the qualifying event. Forms necessary to make changes to your health plans are available in the Personnel Department at the CAO.

2 The Local Choice Anthem KeyAdvantage PPO Plans/Delta Dental A Preferred Provider Organization (PPO) is a health care benefit arrangement designed to offer health care services at an affordable cost. The PPO plan offers flexibility because you can utilize both in-network and out-of-network providers. Since in-network providers contract at a discount, the plan offers reduced costs if you use participating providers. See the Summary of Benefits and related materials for more information. Prescription drugs and diabetic supplies Your health plan covers prescription drugs if received through a pharmacy, a doctor s office, or a hospital. If you receive prescription drugs from your doctor, they will be covered as other medical services or supplies. If you receive prescription drugs from your hospital, they will be covered as a hospital service. Just present your Anthem Insurance Card at a participating retail pharmacy or order up to 90 day supply using the mail order prescription form. Blue View Vision Blue View Vision provides a preferred provider network of vision care providers. For the highest level of benefits at the lowest costs, you should receive care from a licensed optometrist, ophthalmologist, or optician that participates in the Blue View Vision Network. Coverage includes co-payments for eye exams, allowances on frames and lenses and additional benefits and additional in network discounts. The Anthem BC/BS ID card identifies you as a member. Show your ID card to your vision care providers in most cases, they will file the claims for you. See the Summary of Benefits for more information. 2

3 The Local Choice Anthem High Deductible Health Plan/Delta Dental This is a self managed high deductible health plan administered by Anthem. The HDHP in-network providers contract at a discount, the plan offers reduced costs if you use participating providers. Although rates are discounted, there are no doctor visit or drug co-pays. You are charged the entire discounted rate for services. See the Summary of Benefits and related materials for more information. Health Savings Account (HSA) Enrollment in a High Deductible Health Plan (HDHP) allows you to set up a personal Health Savings Account (HSA) through Health Savings Administrators, LLC to help you manage health care expenses or save for retirement. The Health Savings Account is your money and you can continue making contributions to the account every year that you are enrolled in the High Deductible Health Plan. If you terminate employment or retire the HSA account money is your money to spend on qualified medical/dental expenses. If you change health insurance plans from a HDHP you can no longer contribute to the HSA but you can use the money in the account for qualified medical expenses. HSA Contribution by WPS One-Time Employee only $1, Employee/one child $1, Employee/Spouse $1, Family $1, **Family/Couple $2, **(Husband and wife both work for WPS) If you are a late hire the HSA contribution will be prorated. 3

4 Flexible Spending Accounts (FSA) This program allows you to set up a pre-tax Medical Reimbursement Account to pay for qualifying medical expenses and a pre-tax Dependent Care Reimbursement Account to pay for qualifying dependent care expenses. These pre-tax deductions allow you to reduce your taxable income and your reimbursements for qualifying expenses are tax free. Take care by WageWorks is our third party administer of our Flexible Spending Accounts (FSA). You must complete the Flexible Benefit Plan Election Form even if you are not electing a flexible benefit account. You can contribute up to $2,550 per plan year to a Medical Reimbursement Account and up to $5,000 per plan year to a Dependent Care Reimbursement Account (up to $2,500 per plan year if you are married and file separate federal income tax returns). WPS flexible spending accounts begin July 1 st each year and end June 30th of the following year. Your total contribution will be divided by the number of pay periods remaining in the plan year. That amount will be deducted from each pay period. Once you have incurred eligible expenses, you can file for reimbursement. When you use your takecare Flex benefits Visa Debit Car to pay for qualified purchases, the money is instantly deducted from your flexible benefit account(s). You ll save 25% - 40% on qualified purchases or services, when you use your flex account dollars. The IRS regulates these pre-tax accounts, so when deciding how much to allocate to an FSA, you need to plan carefully. IRS regulations state that any balances remaining in a medical or dependent care account 90 days after the end of the plan year will be forfeited. You will need to use this year s funds before the September 15, 2015 deadline or you will forfeit any balance remaining in the account from this year s funds. Once you allocate money into your FSA you cannot change the amount of your contribution during the year unless you have a qualifying life status change. Health and dental insurance premiums are automatically deducted before taxes for every employee through the flexible benefits plan unless you request otherwise. If you do not want insurance premiums deducted on a pre-tax basis, you must notify the Human Resources Department, in writing when you complete the enrollment forms. 5 Life Insurance Winchester Public Schools participates in the Virginia Retirement System Group Life Insurance Program. You are covered under the Basic Group Life Insurance Program from the first day of employment at no cost to you. The natural death benefit is equal to your compensation rounded to the next highest thousand and then doubled. The accidental death benefit is double the natural death benefit. See the plan document for additional information and benefits. Optional Life Insurance You may purchase additional coverage for yourself through the Optional Group Life Insurance Program. If you apply for Optional Life Insurance within 31 days from the date of employment, you may receive all options, up to a maximum death benefit of $375,000, without providing evidence of good health. If you elect this coverage, you also may cover your spouse and dependent children. Optional group life insurance provides benefits for natural and accidental death or dismemberment. You pay the premiums through payroll deduction. See the Optional Group Life Insurance materials for more information. Minnesota Life Commonwealth of Virginia Voluntary Group Long Term Care Insurance Program The employee-paid COV Voluntary Group Long Term Care Insurance Program provides a maximum monthly benefit for covered long-term care expenses. Genworth Life Insurance Company has been contracted as the insurer for the program. If you apply within 60 days of employment, you do not have to provide proof of good health (evidence of insurability). Proof of good health is required for any family members who apply, provided you apply after 60 days from your employment date or if you are a deferred member or retiree. For more information, contact Genworth Financial at Free Student Tuition Nonresident students whose parents or guardians are full-time employees (entitled to the full benefits package available to school/city employees) of the Winchester Public Schools may enroll their children in Winchester Public Schools tuition free. Nonresident students whose parents or guardians are part-time employees (at least half-time [minimum of twenty (20) hours/week] of the Winchester Public Schools are eligible for a reduced tuition fee to be set annually by the Superintendent s regulation. Such enrollment shall be contingent upon meeting the criteria specified the WPS School Board Policy JECC Nonresident Student Attendance. See the policy for additional details. Tuition Assistance Staff development is essential to advancing the mission of the school system. Funds are available for tuition assistance to employees seeking college degrees (doctorate, master s, bachelor s, associate s), certificates, General Equivalency Diplomas (GED), or additional training. Tuition assistance shall be limited to budget funds for the fiscal year (July 1 June 30). Employees will be reimbursed 100% not to exceed $600 per course for a maximum amount of $1,800 per fiscal year after submitting required documentation. 6 Retirement Plans Virginia Retirement System Plans The VRS Plan 1 is a defined benefit plan. You are covered under the provisions of the VRS Plan 1 if your membership date is before July 1, A defined benefit plan provides a lifetime monthly benefit in retirement based on your age, total service credit and average final compensation at retirement. Average final compensation is the average of your 36 consecutive months of highest compensation as a covered employee. You are covered under the provisions of the VRS Plan 2 if your membership date is between July 1, 2010 and December 30, The VRS Plan 2 is a defined benefit plan. A defined benefit plan provides a lifetime monthly benefit in retirement based on your age, total service credit and average final compensation. Average final compensation is the average of your 60 consecutive months of highest compensation as a covered employee. Winchester Public Schools employees in both VRS Plan 1 and Plan 2 contribute 5% toward their retirement through VRS. Contributions are made through payroll deductions and are not subject to income tax withholding at the time of deferral. However, the contributions are subject to social security, Medicare, and federal unemployment taxes. VRS Hybrid Retirement Plan If your membership date is on or after January 1, 2014, you will be covered under the VRS Hybrid Retirement Plan. This plan is a combination of a defined benefit plan and a defined contribution plan. Employees make mandatory contributions of 4% to the VRS Hybrid Retirement Defined Benefit Plan and 1% to the 401(a) Defined Contribution Plan. You may make additional voluntary contributions to a Hybrid 457 defined contribution plan in which you choose how the monies are invested from a range of options. These voluntary contributions are matched in part by Winchester Public Schools. You bear the investment risk, and the amount of your benefit depends on the amount you contribute and investment earnings over the life of the plan. As a member of the VRS Hybrid Retirement plan you are covered by a Group Short Term Disability Benefits Program and a Group Long Term Disability Insurance program. The disability benefits programs are paid for by Winchester Public Schools. For more information on Plans 1, 2, and the Hybrid Retirement Plan visit 6

7 Commonwealth of Virginia 457 Deferred Compensation Plan Defined Contribution Plan The Commonwealth of Virginia 457 Deferred Compensation Plan is a type of qualified tax advantaged deferred-compensation retirement plan. Employees defer compensation to the plan on a pre-tax basis. For the most part the plan operates similarly to a 401(k) or 403(b) plan. The key difference is unlike a 401(k) plan, there is no 10% penalty for withdrawal before the age of 59½ (although the withdrawal is subject to ordinary income taxation). Your funds are available to you upon termination of employment. However, while you are an employee, funds are only available in cases of unforeseeable emergency as defined by the Internal Revenue Service (IRS). For more information, review the summary materials or visit 403(b) Plan Defined Contribution Plan Winchester Public Schools sponsors two types of 403(b) private retirement investment plans. The traditional 403(b) is a plan that allows employees to invest pre-tax dollars into a qualified plan and a Roth 403(b) plan which allows employees to invest post-tax money into a qualified plan. WPS supports both before-tax payroll contributions to a qualified 403(b) plan and after-tax payroll contributions to a qualified Roth 403(b) plan. 8 Paid Time Off/Leave Plans Holidays (12 month employees) The school division observes selected holidays designated annually by the School Board upon approval of the school calendar. The school division shall be closed on these days, and employees in a 12-month contract will have these as paid holidays. New Year's Day Martin Luther King Day President s Day Good Friday Apple Blossom Friday Memorial Day Independence Day Labor Day Wednesday before Thanksgiving Day Thanksgiving Day Friday after Thanksgiving Christmas Eve Christmas Day after Christmas Annual Leave All employees who are classified as full-time twelve month employees accrue annual leave. Length of service is rewarded with higher annual leave earnings (see below) and carry-over accumulations based on the fiscal year cycle (July 1 through June 30). See WPS School Board Policy GCBD-6 Annual Leave for more information. 0 to 5 years of service 1.25 days/month 15 days per year After completing 5 years 1.5 days/month 18 days per year After completing 10 years 1.75 days/month 21 days per year Personal Leave Personal and/or emergency leave with pay is granted to all employees who are ineligible for annual leave but who work four (4) or more hours per day in a given job or are categorized as half-time. This leave is designed to provide a reasonable opportunity for employees to be absent from work to take care of personal business not covered under the sick leave plan or other legitimate reasons necessitating absence from work. Personal and/or emergency leave can only be taken in one-half day or full-day increments. Allocation of leave is outlined below. See WPS School Board Policy GCBD-4 Personal and/or Emergency Leave for more information. Licensed Personnel - 3 days per year Support Personnel - 2 days per year 8

9 Sick Leave Full-time employees are granted sick leave days at the beginning of each fiscal year or school year of employment. Employees that are 10 month will receive 10 sick leave days, 11 month employees will receive 11 sick leave days and 12 month employees will receive 12 sick leave day per school year. For partial years of employment, this allocation is prorated. New employees may transfer up to (90) days of sick leave accumulated in another school division in Virginia or in an agency of the state government. Unused sick days are accumulated to a maximum of 90 days. Sick leave may be used for personal illness, illness in the immediate family, or bereavement leave for death in the immediate family. See WPS School Board Policy GCBD-2 Sick Leave and Sick Leave Bank for more information. Sick Leave Bank The Sick Leave Bank benefit is designed to provide pay to those participating employees out for extended illness who have exhausted all of their personal paid leave. Participation in the Sick Leave Bank is for all full time employees who are members of Plan 1 or 2 of the VRS. Bus Drivers, Bus Aides, Cafeteria Workers, and part time Custodians are not eligible for participation in the sick leave bank. Eligible employees participate in the bank by making an initial contribution of two (2) days from their accumulated sick leave allocation. Employees will not be required to contribute more days to the sick leave bank until all sick leave days in the bank have been exhausted. The sick leave bank does not cover the first twenty (20) consecutive days of a disability/sickness. The waiting period must be covered by the participating member s accrued sick leave or leave without pay. After members return to work, the waiting period must again be satisfied before qualifying to use the sick leave bank again. However, if members suffer a relapse within thirty (30) calendar days due to the original disability/sickness, they do not have to satisfy another waiting period. A participating member may use up to a maximum of ninety (90) days from the bank in a rolling 12-month period measured backward from the date the medical leave began. The member must have exhausted all of his/her paid leave days before using days from the bank. See WPS School Board Policy GCBD-2 Sick Leave and Sick Leave Bank for more information. The sick leave bank is not available to new employees who are in the VRS Hybrid Plan as they have short/long term disability. Short and Long Term Disability Employees participating in the VRS Hybrid retirement program will be a member in the short term and long term disability programs. The cost of this benefit program is funded by Winchester Public Schools. Jury or Civil Duty Leave Employees called for jury duty or subpoenaed to give testimony as a witness during the term of their contract or agreement with the Winchester Public Schools may take a leave of absence for the time required to serve on a jury or as a witness for their required presence at the courthouse. Employees shall be paid at their regular per diem rate minus the monies received for their services (for jury duty). Verification of the call to jury duty or supoena and the pay received shall be required prior to employees receiving payment from the school division for their absence. 9
