3 New Choices in Retiree Health Care Coverage for Medicare-Eligible Retirees Starting Jan. 1, 2013, we ll replace our current Medicare Share and Medicare Supplement Plans with a Health Reimbursement Account (HRA). This means that Medicare-eligible retirees and their Medicare-eligible dependents will select their own private insurance plan, instead of enrolling in a Motorola Solutions-sponsored plan. You ll be able to choose from a wide selection of supplemental Medicare coverage offered by private insurance companies, which will help you personalize your Medicare benefits. We recognize that your health care coverage is important to you and your family. That s why we ve partnered with Extend Health, a leading Medicare coordinator, to support you. Extend Health s benefit advisors will help you select and enroll in a plan that meets your needs. And as you start using your private insurance plan in 2013, Extend Health s customer service team will be available to support you and answer questions. We designed this brochure to help you understand why we re making these changes, how they ll benefit you, and what you can do to make thoughtful, timely choices about your health care benefits. In the coming weeks, you ll receive more information from Extend Health including details that will help you prepare for these changes, evaluate your Medicare supplement plan options, and enroll in a plan that s right for you. 3

They ll mail it to your home beginning in late August. Attend an informational meeting to get answers to your questions (see page 7 for details). 6 What s Changing and Why We understand that your Motorola Solutions retiree medical benefits are an important part of maintaining your health and financial security. Like many other employers, we continue to look for ways to ensure that you have the health care coverage you need, at a cost you and we can afford. What s Changing Your coverage under the Motorola Solutions Medicare Share or Medicare Supplement Plan will end on Dec. 31, Beginning Jan. 1, 2013, we ll offer Medicare-eligible retirees and dependents a new way to access health care coverage one that gives you more plan choices and greater flexibility. You ll be able to choose from a wide selection of individual insurance plans (and dental and vision coverage if you need it), which will help you personalize your Medicare benefits. These plans will replace the post-65 retiree medical coverage we currently offer. And you ll be able to change your plan each year. On Jan. 4, 2011, Motorola, Inc. split into two independent, publicly traded companies. Even if you retired from Motorola, Inc., we consider you a Motorola Solutions retiree. You ll be able to work with Extend Health, a leading Medicare coordinator, to choose the type of private insurance plan that s best for you. We re offering Extend Health s phone support and enrollment services at no cost to you as a Medicare-eligible retiree or spouse/domestic partner. And as you use your private insurance plan, Extend Health s customer service team will be available to answer questions, provide claims support, assist you with reimbursements from your HRA, and much more. We ll continue to contribute toward your retiree health care benefits. We ll make an annual contribution to an HRA set up in your name. You can use your HRA to reimburse yourself for health care premiums and qualified expenses on a tax-free basis. We ll review our annual contribution each year. You ll choose and pay for your own private insurance plan. You won t be able to have your premiums deducted from your pension check, although you may be able to set up automated payments from your bank account to pay for your coverage. You must take action to have coverage! If you don t enroll in individual supplemental Medicare coverage through Extend Health between Oct. 8 and Dec. 31, 2012, Motorola Solutions won t contribute to an HRA for you. You can still elect coverage through Extend Health after Dec. 31, 2012, but you ll be responsible for the full cost of your supplemental Medicare coverage. 6
7 Why We re Making This Change We continually review our benefit programs to ensure that they re competitive and balance the needs of our employees, our retirees, and the company. We carefully consider our options and make changes as needed. Many companies have made changes to their retiree health care benefits, ranging from eliminating these benefits to changing the way they re delivered. At the same time, health care reform changes, tax requirements, and the availability of government subsidies have all impacted our benefits costs. With this change, we have an opportunity to provide you with more when it comes to your health care coverage: more coverage and cost choices, more flexibility in how you pay for coverage, and more help deciding which plan best meets your needs. By making this change, we ll be able to better control our own costs, which enhances the sustainability of our plan and helps our company succeed in the marketplace. And, we ll be able to give you access to more plan options and better value without sacrificing quality or coverage. Our partnership with Extend Health: Allows us to continue offering retiree health care options at a cost that you and our company can afford. Provides you with a number of options you can use to choose the best health care plan for your budget and health care needs (your Medicareeligible spouse/domestic partner can elect a different plan that meets his or her specific needs). Calling all retirees! Retiree meetings in September You re invited to attend informational meetings in Schaumburg, IL, and Phoenix, AZ. Motorola Solutions and Extend Health representatives will discuss these changes and answer your questions. These meetings are open to retirees age 65 and older, as well as their guests. In Schaumburg: Meetings will be held Sept. 12 at 9 a.m., 1 p.m., and 3 p.m. Central time at the Renaissance Schaumburg. In Phoenix: Meetings will be held Sept. 13 at 9 a.m., 1 p.m., and 3 p.m. Mountain time at the Phoenix Airport Marriott. Please call (800) to RSVP. Every retiree s situation is different, and you can rest assured that employees and fellow retirees will receive information that s applicable to them. Who This Change Affects This change affects you if: You re currently covered under our Medicare Share or Medicare Supplement Plan OR AND You re eligible for coverage but you ve not yet elected it (meaning that you ve deferred your coverage) You re a Medicare-eligible retiree; You re a Medicare-eligible spouse/domestic partner of a retiree; or You re a surviving Medicare-eligible spouse/domestic partner If this applies to you, you can enroll in an individual medical and/or prescription drug plan by Dec. 31, If you don t enroll through Extend Health by the deadline, Motorola Solutions won t contribute to an HRA for you. You can still elect coverage through Extend Health later, but you ll be responsible for the full cost of your supplemental Medicare coverage. Your Medicare-eligible spouse/domestic partner will be able to enroll separately in a new plan. He or she doesn t need to enroll in the same plan as you you can each choose and enroll in the plan(s) that are best for your individual medical, dental, vision, and prescription drug needs. Motorola Solutions will make an annual contribution to an HRA for you and your Medicare-eligible spouse/domestic partner as long as each of you remains enrolled in coverage through Extend Health. If Your Dependent Isn t Yet Eligible for Medicare The changes described in this brochure don t apply to retirees, spouses, domestic partners, and dependents who aren t yet Medicare-eligible. At this time, your spouse/domestic partner who isn t eligible for Medicare doesn t have to take action. Your non-medicare-eligible spouse/domestic partner and dependents may continue coverage through Motorola Solutions existing pre-65 medical and/or dental coverage. Once your spouse/domestic partner becomes eligible for Medicare and enrolls in Medicare Parts A and B, he or she will have access to the new individual program, including enrollment support offered through Extend Health. 8

We re offering Extend Health s phone support and enrollment services at no cost to eligible Motorola Solutions retirees and spouses/domestic partners. An Extend Health benefit advisor will work with you to review the individual plans that are available in your geographic area. Your benefit advisor will help you choose and enroll in the private insurance plan for 2013 that best serves your needs and fits your budget. Your benefit advisor will also work with your Medicare-eligible spouse/domestic partner to help him or her make the best choices as well. Licensed and experienced Extend Health benefit advisors will be available to help you every step of the way by: Educating you about the private insurance plans available to you. Explaining the difference between various medical, prescription drug, dental, and vision features and costs for each plan. Helping you make informed and confident enrollment decisions with personalized phone support. 10 How Extend Health Will Help You Beginning in the fall, Extend Health will help you enroll in the private insurance plan that best fits your needs. There are three steps to this process: Learn, Evaluate and Enroll. Extend Health will fully support you through each of these steps. 1 Learn In late August/early September, Extend Health will send you a Getting Started Guide. The guide will tell you how to prepare for enrollment and will give you important information about scheduling an appointment to speak with an Extend Health benefit advisor to help you with the process. Then, in October, Extend Health will send you an Enrollment Guide with instructions on how to evaluate and enroll in the private insurance plan that s right for you. The Enrollment Guide will include comparisons of plan options, information on eligibility, and additional information about working with Extend Health. 2 Evaluate Using the Getting Started Guide, the Enrollment Guide, and Extend Health s online tools, you can review the options available to you before speaking with a benefit advisor. During your scheduled call-in time, you ll provide medical background and other basic information to an Extend Health benefit advisor, on a confidential basis, and you ll learn how your background and specific situation influence your enrollment decisions. You can also complete this information online before you meet with your benefit advisor. It s a good idea to complete this step so your advisor has all the information he or she needs to help you find the best plan for your particular situation. The information you provide will be completely confidential and will not be shared outside Extend Health or with Motorola Solutions. Your benefit advisor will make recommendations based on this information to help you determine which options make sense for you. You ll be able to compare your options and decide what level of coverage best meets your health care needs and budget. 3 Enroll Your Extend Health benefit advisor will guide you through the enrollment process and help you apply for and enroll in the supplemental Medicare coverage you choose. Your benefit advisor will ensure that you make informed and confident decisions. You ll have expert support throughout the entire process. 10
11 Medicare and Your Coverage Choices Medicare benefits are broken into several parts. Before you enroll, it helps to understand how these parts work together so you can decide which choices will best meet your needs and budget. Here s a brief summary to help you understand the parts of Medicare and the decisions you may make. What You Get Part A and Part B Original Medicare consists of Part A and Part B. When you qualify for Medicare due to age or disability, you automatically receive Part A and become eligible for Part B. Part A Provides you with inpatient care and covers hospital stays, home health care, stays in skilled nursing facilities, and hospice care. Part B Provides you with outpatient care and covers doctor s fees and other medical services not requiring hospitalization. You have to choose to enroll in Part B. What You Choose When you select supplemental Medicare coverage, you ll have a choice of the following types of plans: Medicare Supplemental Insurance Plans Help pay some or all of the medical costs not covered by Medicare Parts A and B, such as coinsurance, deductibles, and copayments. + Medicare Part D Plans Cover a portion of your prescription drug costs. Vision and dental plans You also have the option to enroll in individual dental and vision plans through Extend Health. Talk to a benefit advisor to learn more about your options. With these types of plans, you can use the doctors and hospitals of your choice. OR Medicare Advantage (Medicare Part C) Plans Cover most of the same costs as Medicare Parts A and B and often include coverage for prescription drugs. Medicare Advantage plan options vary depending on where you live. With this type of plan, you may need to choose doctors and hospitals in the plan s network. 11
12 Important Note: Motorola Solutions will only make a contribution to your HRA if you enroll in supplemental Medicare coverage through Extend Health. If you decide to elect supplemental Medicare coverage on your own without Extend Health s support, you won t be eligible for an HRA. How the HRA Works A Health Reimbursement Account (HRA) is a Motorola Solutions-funded account that you can use to reimburse yourself for medical, dental, and/or vision plan premiums and qualified health care expenses on a tax-free basis. When you enroll in supplemental Medicare coverage through Extend Health, Extend Health will establish an HRA for you and your Medicare-eligible spouse/domestic partner and manage the account on your behalf. If you don t spend all of your HRA funds in the year they re contributed, the funds will be rolled over so you can use them in a future year. Expenses eligible for reimbursement from your HRA Premiums for Medicare coverage and Medicare supplement plans Out-of-pocket medical expenses like deductibles, copayments, and your share of coinsurance Out-of-pocket prescription drug expenses Dental and vision plan premiums Dental and vision out-of-pocket expenses For a complete list of HRA-eligible expenses, go to publications/p502.index.html or call the IRS at (800) and refer to publication 502. Getting reimbursed from your HRA 1. You pay premiums and expenses. You pay the full premiums directly to the insurance provider. You can ask Extend Health about the auto reimbursement option for premiums. You also pay your provider any required out-of-pocket expenses. 2. You submit out-of-pocket expenses. You submit your claim to Extend Health for your premiums (if you don t elect auto reimbursement) and out-of-pocket health care expenses. 3. Extend Health reimburses you. Extend Health administers your account and will reimburse you from your HRA until all available funds are exhausted. How much will Motorola Solutions contribute to your HRA? Please see the enclosed cover letter for the amount of Motorola Solutions contribution to your HRA for
13 Catastrophic HRA provides a safety net Motorola Solutions is also establishing a catastrophic HRA that will be shared by all members and provide an additional safety net for high-cost prescription drug claims. Prescription costs in excess of $4,750 may be eligible for reimbursement. Extend Health will mail you an HRA Guide with more information in late December. Watch for more HRA details from Extend Health After you enroll in supplemental Medicare coverage through Extend Health during this fall s enrollment period, Extend Health will mail you an HRA Welcome Kit in late December that will help you access and manage your HRA, as well as provide you with claims and processing information. The guide also includes details on how to set up direct deposit of your HRA reimbursement to help you get your money back faster. 13

Calendar of Important Dates and Events Late August Extend Health mails the Getting Started Guide to your home. YOUR ROLE Review the guide to help you prepare for annual enrollment. Complete the pre-enrollment worksheet that s included in the guide. Call Extend Health at (855) or visit to begin preparing for the enrollment process and schedule an enrollment appointment prior to the end of September. MID-SEPTEMBER Extend Health holds informational meetings. YOUR ROLE Review the meeting schedule on page 7. Once your spouse/domestic partner becomes eligible for Medicare and enrolls in Medicare Parts A and B, he or she will have access to the new individual program, including enrollment support offered through Extend Health. 16
17 Will I have many plans to choose from when I enroll? Extend Health has partnered with more than 70 health insurers and offers a wide selection of plans from both national and regional insurance companies. This means that even if you travel, you can choose an insurance company who provides coverage wherever you are in the United States. Your Extend Health benefit advisor will provide more detailed information about the Medicare plan types and explain the actual plans offered in detail. Will my current or past health status affect the plans or rates that are available to me? No. Because you re changing from employer-based group health coverage to individual coverage, no restrictions are placed on your coverage, and there won t be a penalty reflected in your premiums. Will I be able to select a plan that provides the same coverage as my current plan? Individual Medicare plans work differently than group health insurance plans. However, Extend Health works with the top national and regional insurance companies to ensure that you ll have quality plan options. Benefit advisors will help you find a plan that best suits your needs. In many cases, you ll be able to find a plan that s better than your current plan, because it can be more tailored to your specific needs. What can I expect to pay for my new plan? What you ll pay depends on the type of plan that you select. Extend Health s research shows that many people will continue to pay about the same as they have under group coverage with their former employer. But some may pay more and others will pay less. As a general rule, Medicare Advantage plans tend to have lower premiums but require copayments for services. Medigap and Medicare Supplement plans tend to have higher premiums with low or no copayments for services. Your Extend Health benefit advisor will work with you to understand the costs and benefits of the different coverage options available to you. Can I have the cost of coverage I elect for 2013 deducted from my pension check? No. However, most insurance companies give you several billing options for ongoing payments: direct billing, electronic funds transfer from your checking account, or automatic deduction from your Social Security check. Can I continue to see my current doctor? Extend Health knows how important it is to be able to continue to see your current doctor. We recommend talking to your doctor prior to contacting Extend Health, to determine which insurance company plans he or she accepts. This will make your enrollment call with your Extend Health benefit advisor more efficient. I still have questions about the Extend Health transition. Who can I call? It s a good idea to first review this brochure and the Extend Health Getting Started Guide before calling a benefit advisor at Extend Health. 18 If I don t like the plan I enrolled in, can I change my election next year? Yes. Every year you ll have an enrollment period when you can learn about other health care plans and consider enrolling in a different plan. However, during these annual enrollment periods, your current medical conditions may limit the plans available to you. You re encouraged to call Extend Health if you have any questions. Medicare-eligible retirees, Medicare-eligible spouses/domestic partners, and Medicare-eligible survivors are eligible as long as they enroll in supplemental Medicare coverage offered through Extend Health by Dec. 31, If you don t enroll by Dec. 31, 2012, or you decide to elect individual coverage on your own without Extend Health s support, you won t be eligible for an HRA in What expenses are eligible for reimbursement from an HRA? The following is a partial list of eligible expenses: Premiums for Medicare coverage and Medicare supplement plans Out-of-pocket medical expenses like deductibles, copayments, and your share of coinsurance Out-of-pocket prescription drug expenses Dental and vision plan premiums Dental and vision out-of-pocket expenses For a complete list of HRA-eligible expenses, go to or call the IRS at (800) and refer to publication 502. How will my eligible health care expenses be reimbursed from my HRA? You ll be able to request reimbursement directly from Extend Health. You ll find the details in the HRA Guide Extend Health mails to your home in late December. Is there anything I need to do right now in preparation for my retiree group health care benefits changing effective Jan. 1, 2013? Please read this brochure carefully and watch for more detailed information in the mail. Refer to the What s Next section on page 14 of this brochure for the timing of upcoming mailings. It s also a good idea to start thinking now about your coverage needs for 2013 so you ll have a head start when you re getting ready to speak to your benefit advisor. You may also want to discuss this change with a family member or someone who helps you make health care decisions. When does the enrollment period start? You can begin to enroll as early as Oct. 8. Extend Health will send its Enrollment Guide in October, with details about how and when to enroll. How will my 2012 claims be handled if they haven t been paid by Dec. 31, 2012? Any eligible medical expenses incurred on or before Dec. 31, 2012, can be submitted to Anthem Blue Cross and Blue Shield (BCBS) within one year of the date the services were received. Remember, your Motorola Solutions retiree coverage is secondary to Medicare, so medical claims have to be submitted to Medicare before they re sent to Anthem BCBS. If I decide not to enroll by Dec. 31, 2012, or choose to enroll in an individual insurance plan without Extend Health s support, will I still qualify for the HRA if I enroll through Extend Health in a future year? If you currently have Motorola Solutions Share or Supplement Plan coverage and you don t enroll through Extend Health by Dec. 31, 2012, you won t be eligible for our HRA at any time in the future. However, if you have deferred electing your retiree health care coverage, you may enroll at any time in the future through Extend Health to be eligible for our HRA. 18
