The Hoarding Project is hosting a weekend retreat opportunity for people who hoard/clutter who wish to incorporate art/creativity into their healing. Update: Don Pace & Jennifer Sampson presentation at Washington State Environmental Health Association Conference on May 12 th b. Discussion re: Matt Paxton presentation in Bellevue (May 6 th ) Terina Bainter provided summary of Matt Paxton s presentation to the group, including main points of education and potential collaboration with task force in the future. c. Fundraiser Date: September 27, 2014 Terina Bainter updated group on Task Force Flapjack fundraiser to be held on September 27, 2014 at the Federal Way Applebees from 8:00am to 10:00am. 12 Volunteers from the task force are needed to serve Task force members are asked to sell tickets to the event: $10/each; task force can sell up to 400 tickets to the event. Discussed raffle donations 5. 11:40-11:50: Case Consultation a. Consulted on 2 hoarding cases from attendees in the group 6. 11:50-12:10: Working Groups a. Education/Outreach and Community Intervention: Jennifer Sampson Group discussed feedback on version #2 of resource guide. 5 Home & Community Services Mission To promote, plan, develop and provide long-term care services responsive to the needs of persons with disabilities and the elderly To assist low-income individuals and families to obtain appropriate services To maximize independence, dignity, and quality of life. To work with our partner agencies to ensure a client focused service delivery system.

9 HCS Programs COPES Community Options Program Entry System. In addition to personal care COPES can provide: PERS Personal Emergency Response system Home delivered meals Behavioral consultation Client training including consultation with nutritionists, pharmacists, behavioral specialists, nursing services, etc. Environmental modifications Durable Medical Equipment Adult Day Health Nursing Services

10 HCS Programs MPC -Medicaid Personal Care State Plan program which provides personal care services in both in home and residential setting SDCP Specialized Dementia Care Program provides specialized dementia care services for clients in residential settings

11 HCS Programs New Freedom provides an array of services including personal care, equipment, durable medical equipment. Client is assigned a Budget versus residential rate or in home hours. PACE Program of All-Inclusive Care of the Elderly. Provided by Providence ElderPlace. Client receives specialized managed care services coordinated by Providence.

12 HCS Programs RCL Roads to Community Living a program available for clients who have been institutionalized in a Skilled Nursing Facility or Hospital for a least 90 days. In addition to personal care, services available under RCL include; Community Choice Guide Environmental Modifications / Durable Medical Equipment Nursing Services Adult Day Health Service Animal Client Training (nutrition, behavioral, pharmacy/medications, budgeting Vehicle adaptations Items / services necessary for community living Home delivered meals

13 HCS Services Washington Roads Expansion provides services comparable to RCL. Includes services for institutional clients, in addition to clients residing in residential and in home settings. Must be both functionally and financially eligible for HCS services. Includes services such as: Personal care services Community Choice Guides Items and services for community stablilitzation Behavioral consultation / services OT /PT services Specialized Medical equipment Life skills consultation

14 HCS Programs Duals Managed Care (Medicare/Medicaid) Rollout January 2015 Family Caregiver Support Program administered by the AAA (Area Agency on Aging) provides services family member and clients who do not currently on Medicaid LTC services

15 A look at what happens to the client from request for services to service The first step: Intake authorization: A referral is received from the community, Senior I & A, or a hospital by phone, fax, mail or in person The referral information is then input into the CARE (Comprehensive Assessment Reporting Evaluation Tool) system The referral is assigned to a Social Services worker as appropriate (in-home, residential, hospital or nursing home)

16 The second step: Information Gathering: The assigned worker conducts the CARE assessment CARE stands for Comprehensive Assessment Reporting Evaluation The CARE assessment is a computerized assessment tool used to determine a client s functional eligibility for services using the following gathered information: ü Medical information such as diagnosis, medication, treatments, medication management, pain, skin issues, foot care. ü Personal care, that is activities of daily living such as mobility, bathing, eating, toileting, dressing, transferring ü Cognitive status such as memory, decision making, comprehension, behaviors.

17 Financial eligibility is being determined concurrently with Functional Eligibility. Services cannot be set up until Eligibility has been determined for both. Some Programs allow Fast Track setting up services prior to financial determination (MPC / COPES)

18 The third step: Setting up services after eligibility has been determined: Depending on client preference and eligibility; services may be set-up in one of the following settings: Residential Settings such as: Nursing Facility Boarding Home such as Assisted Living, Adult Residential Care, Enhanced Adult Residential Care Adult Family Home

19 In Home Care Choice of Provider type: Individual Provider Home Care Agency

20 The Fourth Step: Move the case on for case management. In-Home Cases are transferred onto to the contracted Area Agency on Aging Case Management Partner. In King County: ADS: City of Seattle Aging & Disability Services ACRS: Asian Counseling & Referral Services CISC: Chinese Information and Services Center ECN: Evergreen Healthcare Residential and Nursing Facility Cases are reassigned to the Home & Community Services case manager for that facility.

21 Other Services We May Provide Protective Services in cases of suspected abuse such as physical, emotional, neglect, financial exploitation or self neglect. Who investigates? Adult Protective Services (APS) Complaint Resolution Unit (CRU): handles investigations of facility staff against residents Hospital Complaint Line

22 Information & Referral We provide clients with information regarding other community resources that may assist with a need. Some examples may be: Volunteer Chore Services: for clients who are not functionally eligible for core services but need some help with housekeeping or preparing a meal or just wants someone to come visit. Pet supplies: often times clients have pets and don t have the money to maintain the pet. There are charitable organizations in the community that will provide pet supplies for people with low income.
