1 2016 Green Home Tour Entry Form TOUR DATES April 30 - May 1 AND May 7-8, 2016 This form MUST be complete! Each house must have an entry form. Each house must be certified through, or enrolled in the approved, third party certification program of NGBS or LEED (as determined by the GHBT EC or Home Innovation Research Labs). The Builder must agree to furnish proof of certification when achieved. Failure by the builder to complete the certification process identified with the entry will make the builder ineligible to enter any other Green Tour marketing events presented by GHBT during the following 12 months. Entry deadlines: December 18, 2015 (Early Bird entry) $ ($300 deposit, balance due by 3/1/15) January 15, 2016 (Regular entry) $1, ($350 deposit, balance due by 3/1/15) February 12, 2016 (Final last call entry) $1, ($400 deposit, balance due by 3/1/15) Above dates are the due date for the entry form and fee. 2 2016 Green Home Tour Entry Form Page 2 Due dates for Tour forms and collateral materials: Completed Tour entry form and fee; December 18, 2015 (Early Bird) January 15, 2016 (Regular entry) February 12, 2016 (Final last call entry) Deadlines: February 12: 1. Certification Documents. Documentation that house is enrolled/ certified in an approved Green Certification program. (NGBS or LEED) 2. Floorplan. The floorplan submitted by the builder will be simplified for inclusion in the tour book. Please indicate any changes (such as a flipped plan or finishing/ deletion of a room) when the floorplan is submitted. Please submit the floorplan electronically if possible to julie@hbadoc.com 3. House Description. Supply no more than 10 concise bullet points that describe the green features or benefits of your entry. If possible, indicate the reason/ benefit for a particular feature (e.g., for better indoor environmental quality). Please submit your bullets via to julie@hbadoc.com 4. Directions. THIS IS IMPORTANT!!! Provide directions from a major thoroughfare to your home. If your home is located in a neighborhood, please include specific directions to the home from the entrance to the neighborhood. Please do NOT include phrases like follow the directional signs. Please submit your directions with GPS coordinates via to julie@hbadoc.com. February 19: 1. Professionally-drawn elevation OR high-resolution photo. A high-resolution photo (1 MB file size) should be submitted if the house will be at a reasonable stage of completion by February 19, Otherwise, the builder must submit a professionally-drawn front elevation of the house. The HBA has artists we can recommend if needed (please coordinate this with the artist at least 2 weeks prior to this deadline!). Low-resolution files are not suitable for printing and will not be accepted. The publisher retains the right to reject and request a resubmission. 2. Certificate of Insurance. Each builder on the Tour is required to provide a copy of your liability insurance showing that it is current (for the EVENT DATES April 30-May 1, May 7-8) and listing the HBA of Durham, Orange & Chatham Counties as additional insured. Your insurance company can or fax this form directly to the HBA (Julie@hbadoc.com or fax ). The HBADOC address is 20 W. Colony Place, Suite 180, Durham, NC (your insurance company will need our address for the process). FEES (total due shown below may be paid in installments): Early Bird Entry Fee (by 5 PM, December 18, 2015) $ 995 Regular Entry Fee (by 5 PM, January 15, 2016) $1,195 Final Entry Fee (by 5 PM. February 12, 2016) $1,395 Important! You must have enrolled the home in or have received certification from an approved third party verified, green certification program by 2/19/16. Failure to meet this deadline will result in withdrawal of the entry and a $500 administrative fee. This is my commitment and entry fee for a home in the 2016 Green Home Tour presented by the Green Home Builders of the Triangle (a joint Council of the Home Builders Association of Durham, Orange, and Chatham Counties and the Home Builders Association of Raleigh-Wake Co). I understand I must be a current member of the HBADOC or HBARWC, AND a current member of the Green Home Builders of the Triangle council in order to enter this home on the tour. I will furnish all required materials by the deadlines indicated in the entry form. The house will be open during all advertised hours. During the tour hours, I will ensure that the entry will be staffed by a knowledgeable person who can explain the green features of the home. I understand that my house may be under construction, but it must be at least framed to enter. SECTION II. For any materials or notices that will be sent by US Mail, use the following address: ENTRY AFFIRMATION Important - the Builder and the HBA s (HBADOC and HBARWC) agree to indemnify and hold each other harmless from and against all claims, losses, costs, damages, expenses and liabilities for bodily injury, death, or property damage arising out of or in connection with any and all occurrences on or about the home, caused in whole or in part by the other. Important - the Builder agrees to furnish a Certificate of Insurance documenting your General Liability insurance coverage of your business and Tour entry. The insurance certificate should list the HBA as an additional insured during Tour dates. You should request this from your current insurance carrier. Please have the Certificate of Insurance ed to julie@hbadoc.com or fax to (919) BUILDER AGREEMENT This entry is the result of an original design and I have obtained the legal right to use the design from the copyright owner and to the design as represented in the structure, plans, specifications and drawings for the house. I further represent that I have not pledged, assigned, or encumbered the rights to the design and that such design does not violate any copyright, property, or common law rights of others. I hereby indemnify and hold harmless HBADOC and HBARWC from any and all claims of third parties, including, but not limited to, copyright infringement or conversion, which may be asserted as a result of the inclusion of the house in the Tour or the inclusion of the drawings or floor plans of the house in any publication, including on the Tour page, of the HBA publicizing or in conjunction with the 2016 Green Home Tour. If an Act of God prevents the scheduled events from being observed, I agree that the Green Home Tour committee shall have the authority to re-schedule the Tour dates at its sole discretion. I agree to help market the Green Tour on all social media outlets I use leading up to and during the event.
