9 12 Check radio button for Education Institutionn only. 13 Click Page 9

10 14 Verify you chose the state of IL and then Click Page 10

11 15 Enter the information EXACTLY as it appears here in the fields to the left. 16 Verify you entered the information correctly and then Click Page 11

12 17 Check radio button for Other only. Enter Education Supplies in the Describe field. Enter E in the Illinois State Exemption Number field 18 Verify you entered the information correctly and then Click Page 12

13 19 Verify you entered the information correctly and then Clickk located at the bottom of the page. Page 13

14 20 Sign your name (as your name appears on your Procurement Card) in the box here using your mouse/cursor Tip: you may use the Clear button to re sign your name as needed before you save/continue. 21 Click Page 14

15 22 Read the Terms & Conditions and Check the box next to You must first agree to our Terms & Conditions Click Page 15

16 24 Click here to review Certification Status & Confirmation Page (see nextt page). Page 16

18 26 Choose Add a Credit or Debit Card Note: You may continue with set up without waiting for the ATEP enrollment to complete. Page 18

20 28 Enter your Procurement Card information here. 29 Verify you entered the information correctly and then Click Page 20

21 30 Enter your billing information which should match what is listed forr your Procurement Card account. 31 Verify you entered the information correctly and then Click Page 21

22 32 Verify informationn at left and then Click Your Account and Choose Your Account from the drop down options. Page 22

24 Here is where your billing address / info will show. 34 Here is where your billing information will be shown. If you need to add a separate department Ship To address, Click Important: Set up process complete. You may log out. You will need to waitt up to 24 hours for your ATEP status to change from Pending to Active. See page 17 for reference. After a new address is added, you will see it here/at left. Page 24
