43 Student Information Technology Services Non-Disclosure/Confidentiality Agreement I understand that I may be granted access to confidential data, information, and records (hereinafter "Confidential Information") whether written or maintained in computer systems (hereinafter "Computer Systems"). My access is limited to my need to know for the purpose of performing the duties as a Student Information Technology Services (hereinafter SITS ) student worker or student volunteer. Access to certain information and or specific computer systems may have additional security requirements beyond those contained within this agreement. Confidential Information includes, but is not limited to, Social Security numbers, confidential personnel records, confidential medical records (records covered by federal law under the "Health Insurance Portability and Accountability Act of 1996"), student education records (records covered by federal law under the "Family Educational Rights and Privacy Act of 1974 ) and credit card information (records covered by Payment Card Industry Data Security Standard). All information acquired by the SITS volunteer/worker in their capacity as a SITS volunteer/worker shall be deemed confidential and protected from access, disclosure or use other than in compliance with official duties as a SITS volunteer/worker. By my signature below, I acknowledge that I have been advised of, understand, and agree to all the policies outlined in the "Acceptable Use Policy" including any policies and legal requirements regarding the distribution of copyrighted material, including unauthorized peer-to-peer file sharing or other activities, as well as Privacy Policies, as well as any pertinent sections of the Student Handbook, and any relevant Human Resources Policies. For the location of the Student Handbook and relevant Human Resources Policies, see table at the end of the document. In addition, I understand that I am required to adhere to the following statements:

46 Student Information Technology Services Student Technology Repair Release of Liability Waiver The technology repair service provided by Lower Columbia College s SITS Help Desk is provided free of charge, performed on a first come, first served basis; repair service is only available to current LCC students. This waiver must be signed and agreed upon in order to receive service. I certify that I am personally the owner of the technology/equipment that is being presented for service and that I am at least 18 years of age. Business owned technology or equipment is not eligible for service. Equipment can be dropped off or picked up only during the hours of 1:00 PM 4:00 PM on Tuesday, Wednesday or Thursday, when the help desk is open and an authorized agent is there to accept or release the property from and to its owner. There is no guaranteed turnaround time for repair or diagnostic services. LCC SITS Help Desk reserves the right to refuse service to anyone or to perform service on equipment if such repair is deemed to be beyond the scope of our repair capabilities. LCC SITS Help Desk reserves the right to refuse installation of hardware and/or software if staff believes that such installation may cause damage or create problems once installed. LCC SITS Help Desk will not supply any operating system or application software to restore the equipment back to working condition. It is the responsibility of the owner submitting the equipment to provide all needed software and software product keys as needed. LCC SITS Help Desk will not purchase or provide any parts or components needed to repair the equipment. Purchase of replacement parts is the sole responsibility of the equipment owner.
47 Student Information Technology Services Student Technology Repair Release of Liability Waiver No verbal or written warranty, expressed or implied, is offered on the services provided by the LCC SITS Help Desk. Work performed by LCC SITS Help Desk may void existing equipment warranties and, in the process of working on computer equipment, there is a potential for data loss. YOU ARE RESPONSIBLE FOR BACKING UP YOUR DATA PRIOR TO SUBMITTING EQUIPMENT FOR SERVICE. If, in the process of working on technology/equipment, material is discovered which indicates a possible violation of LCC policy or state or federal law, the information will be forwarded to the appropriate department or law enforcement agency. I acknowledge that services will be provided by students in a learning capacity I release any and all expectation of privacy to the data on the device. I, (print name) understand that as a result of submitting my personal equipment for repair, diagnosis may reveal the need to purchase new components that I am responsible to purchase, or that I can decline continued attempts to repair the equipment. Further, I agree to release, indemnify, and hold harmless Lower Columbia College, its students, faculty and staff, from liability for any claims or damages of any kind or description that may arise from any work performed on my equipment, even if caused by negligence of LCC staff, faculty or students.
