Please complete the answer sheet before contacting CSAO for a Review and Exam date. You are required to successfully pass the evaluation before completing the exam. A list of dates for the Home Study Review and Exam at CSAO s head office is available on our website or from our customer service. Outside of the Greater Toronto area, please contact your local CSAO Field Consultant to discuss registration (see contact list attached).
