Further information This Guide to the ISCP is primarily designed for all Surgical Trainees (StRs, SpRs, FTSTAs) in all nine surgical specialties, whenever they take up their posts, all Consultants involved in training and all surgical Programme Directors, but it will also be of interest to and helpful for anyone interested in being trained for surgery responsible for supervising and supporting surgical trainees providing the environments & resources in hospitals in which that surgical training takes place. 3 What is the InterCollegiate Surgical Curriculum Programme (ISCP)? The ISCP is: An innovative, competence-based Curriculum across all nine surgical specialties a Web-based Training Management System to record progress through that Curriculum. Why was there a need for a new Curriculum? Surgical training required a complete overhaul to provide: a nationally standardised curriculum clearly defined standards assessments of competence for each stage with clear progression. The Surgical Colleges recognised this need and began work on reform in Why is the ISCP web-based? A web-based system provides many advantages: Convenience a site accessible to all providing a one stop shop for all the information trainees, trainers, managers, patients and the general public require on content, standards expected and assessment methods Efficient a shared site for trainees, trainers and Programme Directors to work in partnership on objectives, progression and assessment which can be easily updated Security a safe location for trainees to record and deposit evidence of training and their experience in preparation for Placement and Annual Reviews. Who has been involved in the development of the ISCP? The Curriculum and Website have been created by surgeons for surgeons with input from experts in the fields of education, quality assurance, IT systems management and patient experience. The Joint Committee of Surgical Training (JCST) has led the management of the ISCP with the: Surgical Colleges of Great Britain and Ireland All 9 Specialty Advisory Committees Surgical Trainee Associations ASIT and BOTA and the Trainee Forum Patient Liaison Groups Five Deaneries which have piloted the curriculum and system over two years Visitors to the website who have submitted comments and ideas. Over 500 surgeons at all levels have contributed to the development of the ISCP Development will continue via planned reviews, user feedback and formal evaluation. PMETB have approved the curriculum and the ISCP web-based system. Who has funded the ISCP? Following funding from the Department of Health, the Surgical Colleges of Great Britain and Ireland have worked in partnership to design and implement the ISCP. 3

5 If you are a Surgical Trainee The ISCP website provides a one stop shop to all the information, guidance and forms for assessment you require from August 2007 when the new curricula for all nine surgical specialties become mandatory. It is mandatory for new ST1, ST2, ST3 and FTSTA appointments to use the ISCP website. You will be driving your own training in partnership with your Assigned Educational Supervisor. It is in your own interests to be prepared for 1st August 2007, and from when you take up your post. Calman trainees are strongly recommended to register and use the ISCP website. Do these two things first: 1. If you have not yet looked at the ISCP website, do so now and read the guidance documents, syllabus for your particular specialty, assessment tools and downloadable forms. 2. If you have not yet registered on the ISCP website, do so now so that you can be ready to access the secure part of the site. 5 Important note: The secure area of the ISCP website, which will hold your portfolio including your Learning Agreement and Assessments, will go live once you have a Training Number, and when your local Programme Director (PD) has confirmed your acceptance on to the Training Programme. This may not happen until late July/early August, or in advance of taking up your post. And then do this: Sign the Health & Probity Statement Complete the Self-assessment and Personal Development Plan forms Prepare for your first meeting with your Assigned Educational Supervisor (Trainer) when you will agree your personal Learning Agreement for your Placement. It s irrelevant whether you like the idea of the ISCP or not the fact is it s here and we have all got to use it. So the best thing is to be proactive get in there and get to know it. Personally I think it s great logbook and portfolio all kept online. Trainee, Wessex Deanery It s up to you, the Trainee, to take the initiative. Get to know the Syllabus and Assessment Tools. Be really prepared for that first meeting. Trainee, Eastern Deanery

6 6 Using the ISCP means that I can agree and record the necessary information, usually straight after any education activity. My Trainee has the responsibility of ensuring all forms are completed so I know it is done and properly recorded! Educational Supervisor, London Deanery If you are an Assigned Educational Supervisor (AES)* *A Consultant becomes an AES once the Programme Director has confirmed him/her as the Educational Supervisor for one or more Trainees in a Placement. The ISCP website provides a one stop shop to all the information, guidance and forms for assessment you require from August 2007 when the new curricula for all nine surgical specialties become mandatory. You will be driving training in partnership with your Trainee/s. It is in your own and your Trainees interests to be prepared for 1st August Your local Programme Director will confirm Trainees into the Training Programme and then match you with a Trainee. Your local Programme Director will select areas from the Syllabus for the Trainee to cover in the Placement and create what are known as Global Objectives. Do these two things first: 1. If you have not yet looked at the ISCP website, do so now and read the guidance documents, syllabus for the 9 surgical specialties, assessment tools and downloadable forms. 2. If you have not yet registered on the ISCP website, do so now so that you can be ready, as well as your Trainee/s, to access the secure part of the site. And then do this: Prepare for your first meeting with your Trainee/s when you will agree the objectives for the Placement (see the Learning Agreement template document on the website). Important notes: (1) It is the Trainee s responsibility to ensure the on-line form is completed. The AES adds comments and countersigns, which validates the information. (2) The secure area will go live once you have registered, been allocated Trainee/s and received a Password. This may not happen until late July/ early August, or until trainees take up their appointments. If you are a Programme Director (PD) The ISCP website provides a one stop shop to all the information, guidance and forms for assessment you require from August 2007 when the new curricula for all nine surgical specialties become mandatory. As Programme Director, you are the Gatekeeper to the ISCP system for all Trainees and Assigned Educational Supervisors. You confirm or validate each Trainee into the Programme. You confirm the Trainee s Placement Educational Supervisor at which stage the Consultant becomes the Assigned Educational Supervisor to the Trainee. You select areas from the Syllabus for each Trainee to cover in the Placement and create what are known as Global Objectives. There is investment of time in getting to grips with the ISCP the Curriculum and the web-based training system - but that investment is well rewarded. The new system provides a level of transparency and clarity for surgical training that we have never had before. Programme Director, North-West Deanery

7 What if? In light of recent medical training system concerns, we know you want as much technical information about the ISCP system as possible. Here it is. If you have further questions or concerns, please contact the ISCP Helpdesk. The ISCP system works and has been thoroughly tested over time ISCP has been on-line for 3 years with an on-line interactive portfolio available since Sept 05. It has been piloted extensively; feedback from users has resulted in updates and new versions. Consultant surgeons and trainees across specialties and deaneries have tested and contributed to improvements in the system s functionality. The core functions of the website are now at a steady-state in preparation for the launch nationwide. Only additional functionality will be added. The ISCP system can cope with high demand There were 84,000 visits in the last five months with over 645,000 pages viewed. The present line to the internet (of 2Mbps) is being improved with a new connection which will offer a much higher bandwidth (10Mps scalable to 100Mbps) and a fallover (2Mbps backup link) in case one of the lines has a problem. The website has been technically optimised. It uses current, well supported technologies including.net and SQL Server. In addition, the database is being upgraded to the latest (2005) version. The ISCP system is secure There is regular vulnerability scanning of the internet presence. Intrusion and Penetration testing of the specific website is taking place prior to the system going live in August. Best practice data protection principles are in place and the ISCP Data Governance group (including a Caldicott guardian) ensures data protection rules are followed at all times. External advice has been taken on internal risk assessment. A full backup of data to tape takes place nightly, a log backup happens every 30 minutes (so the system may be restored to the last half hour, and therefore to the last few transactions), and a back-up takes place before any back-end changes to the database so the system can be restored. All secure areas are encrypted via 128 bit SSL and all passwords are additionally encrypted as well as the database access code. The development code is protected so cannot be corrupted (e.g. injected) and all elements/pages of the website are independently protected. The server security is regularly updated; an additional firewall has been installed. All data on the ISCP system is password protected protect your password! Password protected access is available to the Trainee, AES and PD. AESs and PDs will have access to the agreements and assessments of all the trainees they are supervising. All user accounts have strict permissions in place from top down, meaning that no user can see details without first being given specific access. What if my password becomes known to others? Contact the Helpdesk immediately. They will issue you with a new password. What if the website cannot be accessed? Check first with your local NHS provider that there are no local problems. Report problems to the ISCP Helpdesk by or by phone. What if I have an idea to improve the ISCP website? your ideas to the Helpdesk a programme of planned review of the website is in place. 7

8 Do you need more information? First - log on to and read the Guidance documents. Once you start using the website, you will find this summary information falls into place. Second - refer to the Stepby-Step guide to the ISCP available in three versions (for Trainee, AES and PD) to download from the website. Copies are being distributed to Trainees and all those involved in delivering or supervising training. Further copies are available from your Programme Director, local School of Surgery or the ISCP Helpdesk. Third call or the ISCP Helpdesk. They will redirect your query if they do not know the answer. Some questions may need to be answered locally. During July and August, the capacity of the Helpdesk is being increased to deal with heightened demand. your query if you can when the lines are busy. Lunchtimes will be the busiest times. Please log on to the website and register NOW so that you can test your access and avoid potential problems during the busiest times in July and August.
