1 March 2016 (To be reviewed March 2018) Application guidance and policy for run through training pilots General information and pre application steps The GMC will have approved an uncoupled specialty training programme. When looking to add run through training as a route of entry for a specialty, the following information must be considered and sent to the GMC as a curriculum change request (the curriculum change request form should not be completed for these requests). The following general principles apply to all run through training pilots:- They are to be conducted in parallel with the uncoupled training programme The indicative duration of training will remain the same The early years run through training programme will remain the same as the core training programme in content and assessment (i.e. no changes to curricula competences or outcomes) An equality and diversity analysis and consideration of the public sector equality duty must form part of the request. A curriculum change will not be considered if this information is missing. Evidence to support the pilot As part of all submissions to the GMC, Colleges * must include evidence to support the application. The information needed must include: 1 The rationale for the change/ adding the run through route. * College refers to Medical Royall Colleges and Faculties

2 2 Evidence of support for the change from the overarching training body (i.e. a single specialty cannot seek this request in isolation, they need support from the parent training body for that specialty e.g. if a surgical specialty then JCST, if a psychiatry specialty then the College Education and Training Committee) 3 Evidence of support for the change from deaneries/letbs (via the Lead Dean for the specialty) as they will need to manage doctors in training on two different routes and the additional costs relating to extensions to training etc. 4 Evidence of support for the change from the four UK nations as they agree the funding and would need to support the full duration of the training programme and they need to approve any changes to national recruitment practices. 5 In your submission please indicate all specialties that use the core (early years) training programme as an entry route (please also include those that are not under the College submitting the application). Where your core training programme is an entry into a specialty managed by another College, please provide evidence that you have informed that College of your proposed changes and include comment as to whether they will accept doctors in training who have completed the run through training and that they will be amending their curriculum if the request is approved (see paras 17-19) 6 Details of how long it is proposed the pilot would be for. It is important to recognise that a report / evaluation of success of the pilot would need to be provided to give full approval and this would need to be timed to allow for recruitment to continue; as a guide this should be submitted to the GMC after the normal August training start date so that a decision can be communicated before the next recruitment round in the autumn. We can discuss timelines with you on a case by case basis 7 A transition plan of how those already in core training who want to move into run through training would be moved (or not). Please see paras for further details about manging the transition. 8 Information outlining how entry into ST3 (ST4, if there is a three year core programme) would be managed to ensure fairness to applicants coming from core training. 9 Details of how those in run through training who want to move into another specialty that has the core programme as a route of entry, will be managed. Please see paras for further details about manging the transition. 10 Plans around assessment/arcps in the light of the removal of the recruitment into ST3/ST4 hurdle. 2

3 11 Information around the use of College exams and how they impact progression for the run through route is needed i.e. uncoupled training programmes require College * examinations as an entry requirement (or an exit from core training); the submission will need to clarify if doctors in training are permitted to continue in training whilst they resit examinations, or if will they remain at the same point until they are passed. 12 Details of how the pilot will be evaluated and reported back to the GMC. Programme approval If a run through training pilot is approved, appropriate programme approval will need to be sought. 13 A run through programme is a separate programme to an uncoupled one and therefore it will be necessary for every Deanery/LETB that is going to offer run through to apply for approval of the run through programme BEFORE doctors in training start on the programme. 14 A list of Deaneries/LETBs participating in the pilot will need to be submitted to the GMC as part of the request. This list cannot change during the pilot without reference back to the GMC. 15 The College may provide overarching support for the programmes so that any approval submissions do not need to include separate College support. 16 The GMC will publish approval of the pilot and the amended curricula on its website inform Deaneries/LETBs that it has approved their participation in the pilot and remind them to apply for approval of programmes prior to any doctor in training being recruited inform the National Training Survey team of the new programme Doctors in training completion of training route CCT / CESR (CP) 17 Should a doctor in training appointed onto the pilot run through programme subsequently not wish to continue in that specialty, but enter one of the specialties 3

4 that has the core training route of entry, then they would not be eligible for a CCT in that new specialty. To be eligible for a CCT all of the training must have been prospectively approved for that specialty and if run through is not part of the second specialty curriculum, then it is not approved. These doctors would be eligible for a CESR (CP). 18 An example of this might be where a doctor in training who completes ST1 & ST2 for run through Cardiothoracic surgery but rather than remain in Cardiothoracic surgery, applies to enter ST3 of General surgery. Although the run through training programme ST1 & ST2 for Cardiothoracic surgery is the same in content and assessment as the core training programme CST1 & CST2 it has not been included within the General surgery curriculum and therefore the doctor in training is not eligible for a CCT. The doctor is however eligible for a CESR(CP) as they have gained competences in unapproved (for that specialty) training. 19 Full details about CCT, CESR (CP) and other routes to the GMC Specialist and GP register are available via the GMC website. Transition 20 When applying to conduct a run through pilot, Colleges must submit a transition plan (see para 7). 21 It is important that doctors in training are made aware of your proposals and how it impacts on them; these details must be clearly articulated and readily available. 22 The GMC requires the College to produce clear FAQs / guidance to cover those considering entering the specialty / core training and for those already in the core training. Ceasing pilot 23 If you wish to cease the pilot at any point including at the original pilot end date, you will need to provide us with a rationale for this decision and include support/endorsement from the four UK nations and Lead Dean. 24 It is not possible to cease the pilot whilst there are doctors in training on the programme, as such the College will either need to provide a transition plan as to how they will move doctors in training who are on the run through programme to the un coupled programme OR they could freeze recruitment to the pilot to prepare for cessation. The GMC will require evidence from every participating Deanery/LETB that there are no doctors in training on the run through programme. The College will need to provide this as part of the cessation request. 4

5 25 Include with your request, details of the changes that will need to be made to your curricula, including those that you have amended to enable acceptance of the pilot run through training. Where other Colleges have indicated they would accept the run through pilot (see para 5) these Colleges will need to be notified of the cessation of the pilot and also provide detail of curricula changes required. 26 Once we have confirmed the cessation of the pilot we will notify all Deaneries/LETBs that this is no longer an acceptable route and your amended curriculum will need to be published as will any amended curricula from Colleges referred to in para It is the Colleges responsibility to notify those undertaking recruitment to ensure that the route is not used going forward and person specifications etc. are updated. 28 The GMC will remove details of the pilot and the include amended curricula (removing reference to run through route) on its website inform Deaneries/LETBs that it has ceased approval of the pilot inform the National Training Survey team of the cessation of the run through programme. Reporting and evaluation of the pilot 29 When initially seeking approval of the pilot, the College will have proposed how the pilot would be evaluated and reported back to the GMC (see para 12). 30 In addition to the results at the end of the pilot, Colleges are also asked to provide an update in the Annual Specialty Report (ASR). Full implementation of pilot 31 Where the College, Deaneries/LETBs or four UK nations consider the pilot to be successful and wish to apply to the GMC for the run through route to be fully implemented, the following information needs to be supplied: An evaluation of the pilot to reflect the original request / justification i.e. if the justification indicated that the new route was envisaged to increase retention / recruitment, then evidence that this has succeeded. Numbers / percentages of doctors in training who sought to transfer to the run through route. 5

6 Feedback from doctors in training and trainers. 32 The GMC will consider the evaluation and justification for the permanence of the pilot and make a decision. If the decision is to reject the application, then the pilot will cease (follow the process see paras 23-28). If the decision is to approve the application, the GMC will: remove the separate details of the pilot from its website inform Deaneries/LETBs that it has given full approval of the run through programme inform the National Training Survey team of the full implementation of the run through programme.
