1 MEDVAMC Clinical Pharmacy Preceptors Oluwatoyin A. Arije, Pharm.D., is a clinical pharmacy specialist in Inpatient Surgery. She received her B.S. in Biology from Xavier University of Louisiana and her Pharm.D. from Belmont University College of Pharmacy. She completed a joint PGY-1 Pharmacy Residency program at LSU Health Sciences Center/ Xavier University of Louisiana College of Pharmacy in New Orleans, Louisiana. Dr. Arije is currently a member of the American Society of Health System Pharmacy, American College of Clinical Pharmacy, and Texas Society of Health System Pharmacy. Dr. Arije is also a clinical instructor at the Baylor College of Medicine. Richard M. Cadle, PharmD, BCPS (AQ-ID), FASHP is the Pharmacy Practice Residency (PGY1), Infectious Disease (PGY2), Primary Care Center of Excellence and VALOR Director and the Clinical Pharmacy Manager. He received his PharmD from the University of the Pacific College of Pharmacy in Stockton, California and then went on to complete a pharmacy practice residency at the VA Medical Center in Phoenix, Arizona. He serves on the HIV team and the Infection Control Committee. Dr. Cadle is also involved in multiple investigator initiated research protocols, and he serves on the manuscript review board for the Annals of Pharmacotherapy and the American Journal of Health-System Pharmacy. He is an Associate Professor of Medicine at Baylor College of Medicine. He is also a clinical assistant/associate professor at University of Texas, University of Houston, and Texas A&M Colleges of Pharmacy. He is an active member of Gulf Coast Society of Health System Pharmacists and a past president. He is the current Co-Chairman of the Education Council for the Texas Society Health System Pharmacists and the current at-large member of Gulf Coast Society of Health System Pharmacists Executive Council. Feibi Chi, PharmD, BCPS is a clinical pharmacy specialist for internal medicine. She received her PharmD from the University of Texas at Austin. She completed a PGY-1 Pharmacy Residency at the MEDVAMC and a PGY-2 Geriatrics specialty Residency at the Audie Murphy VA in San Antonio, Texas. She was previously a clinical pharmacy specialist on the nephrology team at the Methodist Hospital in Houston, Texas before joining the MEDVAMC team. Currently, Dr. Chi also serves as a clinical instructor at the Baylor College of Medicine and as the pharmacy practice residency coordinator.

2 Michael A. Gillette, PharmD, BCPS-AQ Cardiology, BCACP graduated from Nova Southeastern University (NSU) College of Pharmacy. He completed a PGY-1 Pharmacy Practice Residency as well as a 2-year Post- Doctoral Fellowship in Cardiology at the North Florida/South Georgia Veterans Health System in Gainesville, FL. Prior to coming to MEDVAMC, he was an Assistant Professor in the Pharmacy Practice Department at NSU. Dr. Gillette s clinical and research interests lie in the optimization of pharmacotherapy for the treatment of various cardiovascular diseases. He currently holds membership with the American College of Cardiology (ACC), American Society of Health-Systems Pharmacists (ASHP) and the American College of Clinical Pharmacy (ACCP). Outside of work, Dr. Gillette is an avid sports fan. Stephanie Bird, PharmD is a clinical pharmacy specialist in the surgical intensive care unit (SICU). She graduated from The University of Houston College of Pharmacy and completed her PGY-1 Pharmacy Practice Residency at The University of Chicago Medical Center. She stayed at The University of Chicago for her PGY-2 in critical care, during which time she served as chief resident. Dr. Bird s clinical interests include sedation, analgesia, and delirium management as well as infections in critically ill patients. She currently holds memberships with the Society of Critical Care Medicine (SCCM) and American College of Clinical Pharmacy (ACCP). Andrew Hunter, PharmD, BCPS graduated from The University of Texas at Austin College of Pharmacy and completed his PGY-1 Pharmacy Practice Residency at the MEDVAMC. He then completed a PGY-2 Infectious Diseases Pharmacy Residency at the Cleveland Clinic in Ohio. Dr. Hunter s clinical interests include antimicrobial stewardship and infections in immunocompromised patients. He currently holds membership with the American Society of Health-Systems Pharmacists (ASHP) and the Society of Infectious Diseases Pharmacists (SIDP). Outside of work, Dr. Hunter enjoys traveling, camping, cooking, and attending concerts.

3 Regina A. Issac, PharmD, BCPS is the clinical pharmacy specialist for spinal cord injury. She received her BS in Biomedical Science from Texas A&M University in College Station, Texas and her PharmD from the University of Houston College of Pharmacy. She then completed a PGY-1 Pharmacy Practice Residency at the MEDVAMC. Dr. Issac currently serves on the wound care and fall risk committees. She holds academic appointments at the University of Texas and University of Houston Colleges of Pharmacy and is a clinical instructor at the Baylor College of Medicine. Jenny Kwong, PharmD, BCPS is clinical pharmacy specialist in internal medicine. She graduated with highest honors from the University of Texas College of Pharmacy and was recipient of the Eli Lilly & Company Award for exemplary academic performance. Dr. Kwong serves on the Veterans Affairs Learning Opportunity Residency (VALOR) program advisory board, which designs and coordinates program content for pharmacy students in the program. She is also a clinical instructor at the Baylor College of Medicine. Janeca N. Malveaux, PharmD, BCPS, CDE is a clinical pharmacy specialist in primary care. She received her PharmD from the University of Louisiana at Monroe College of Pharmacy. She is a former resident of the MEDVAMC pharmacy residency program with an emphasis in primary care. Dr. Malveaux is involved with the diabetes / endocrinology clinic, various formulary management projects, and research projects. She is an active member of the American College of Clinical Pharmacy and American Association of Diabetes Educators. Dr. Malveaux currently holds academic appointments as clinical assistant professor at the University at Texas and the University of Houston Colleges of Pharmacy and is a clinical instructor at the Baylor College of Medicine.

4 Angelena McBayne, PharmD is a clinical pharmacy specialist in Internal Medicine. She received her PharmD from Texas Southern University. After completing a PGY1 Residency at the Central Arkansas Veterans Healthcare System, she practiced as a clinical pharmacy specialist for the Adult Day Healthcare, Dual Diagnosis Unit and Specialty Clinics. She serves on several committees at the local and VISN level. Dr. McBayne is also a clinical instructor at the Baylor College of Medicine. Nicole L. McMaster-Baxter, PharmD, MS, BCPS is the Inpatient Clinical Pharmacy Program Manager. She received her Pharm.D. and Certificate in Aging Studies at the University of Missouri-Kansas City. Dr. McMaster then completed an ASHP-accredited geriatrics specialty residency at the San Antonio VA. She spent two additional years there as a clinical pharmacy specialist in geriatrics and primary care and completed a Masters of Science in Clinical Investigation through the UTHSCSA. She practiced in Internal Medicine for seven years at the MEDVAMC before assuming her current position. Dr. McMaster currently holds academic appointments as clinical assistant professor at the University of Texas and University of Houston Colleges of Pharmacy and is a clinical instructor at the Baylor College of Medicine. In 2010, she received the TSHP Lewis S. Smith Pharmacist of the Year award. John E. Mbue, PharmD, BCOP is a Hematology & Oncology Clinical Pharmacy Specialist. He received his Doctor of Pharmacy Degree from The University of Missouri-Kansas City School of Pharmacy and a Certificate in Health Informatics from the University of Texas Health Science Center Houston, School of Biomedical Informatics. Dr. Mbue then completed an ASHP-accredited hematology and oncology residency at Walter Reed Army Medical Center, Washington D. C. He practiced in Hematology and Oncology Service at Brooke Army Medical Center in San Antonio Texas for seven years before assuming his current position at MEDVAMC. He enjoys traveling, movies, soccer, and watching his kids play basketball.

5 Lisa J. Miller, PharmD, BCPP, CGP is the clinical pharmacy specialist in psychiatry. She received her BS in Pharmacy from University of the Sciences at Philadelphia and her PharmD from Medical College of South Carolina. Dr. Miller completed a pharmacy residency in psychiatry as well as geriatrics at the Veterans Affairs Medical Center in Gainesville, Florida. She holds academic appointments as clinical assistant professor at the University at Texas and University of Houston Colleges of Pharmacy and is a clinical instructor at the Baylor College of Medicine. She is also a founding member of the College of Psychiatric and Neurologic Pharmacists and serves on the editorial advisory board for the Annals of Pharmacotherapy. Brian G. Mitchell, PharmD, BCPS, BCPP is originally from Houston, Texas. He was a previous Michael E. DeBakey Veterans Affairs VALOR Pharmacy student in He received his Doctor of Pharmacy degree in 2009 from Texas Southern University College of Pharmacy and Health Sciences. Dr. Mitchell completed his Pharmacy Practice Residency at the North Texas Veterans Affairs in and his Psychiatric Pharmacy Specialty Residency with Parkland Health & Hospital System and Texas Tech University Health Sciences Center - School of Pharmacy in His practice interests include depression, posttraumatic stress disorder, schizophrenia, substance abuse and bipolar disorder. He holds academic appointments as Assistant Professor in the Psychiatry & Behavior Sciences Department at Baylor College of Medicine and is a Clinical Adjunct Professor at Texas Southern University College of Pharmacy & Health Sciences. Dr. Mitchell is currently a member of the American Society of Health System Pharmacy, American College of Clinical Pharmacy, College of Psychiatric and Neurologic Pharmacists, and Texas Society of Health System Pharmacy. Dr. Mitchell serves as a peerreviewer for the Journal of Pharmacy Practice. Jetavia J. Moody, PharmD is a clinical pharmacy specialist in Geriatrics, Long Term Care and Palliative Care. She graduated from Texas Southern University College of Pharmacy and Health Sciences and completed her PGY-1 Pharmacy Practice Residency at the Central Arkansas Veterans Healthcare System. Dr. Moody also has experience in the outpatient setting at the MEDVAMC as well as the community setting. She currently serves on the Medication Safety Subcommittee, Pain Committee and the Peer Review Committee. She holds academic appointments at Texas Southern University College of Pharmacy and Health Sciences and Texas A&M Rangel College of Pharmacy and is clinical instructor at Baylor College of Medicine.

6 Katie Morneau, PharmD, BCPS is a Clinical Pharmacy Specialist in inpatient surgery with the neurosurgery and vascular surgery teams. She received her B.S. in Biochemistry from St. Mary s University in 2006 and completed her PharmD at the University of Maryland School of Pharmacy in Baltimore in She went on to a PGY1 Pharmacy Practice residency at Methodist University Hospital in Memphis, TN and completed a PGY2 Critical Care residency at UT MD Anderson Cancer Center in Houston, TX. She practiced as the transplant/surgery clinical pharmacy specialist for 2 years at MEDVAMC before transitioning to her current position. She holds academic appointments as a clinical assistant professor at UT College of Pharmacy and a clinical instructor at the Baylor College of Medicine. She holds memberships in the American Society of Health-System Pharmacists, Society of Critical Care Medicine, and American College of Clinical Pharmacy. She currently serves as Director of the Gulf Coast Society of Health-System Pharmacists and a member of the ASHP New Practitioners Forum Resident Advancement Advisory Group. David M. Newland, Pharm.D., M.S., BCPS, is the Clinical Pharmacy Specialist in Solid Organ Transplantation. He received dual B.S. degrees in Neuroscience and Biology from Indiana University-Bloomington and earned a M.S. degree in Biology from Purdue University. Dr. Newland received his Pharm.D. from Pacific University School of Pharmacy in Hillsboro, Oregon. He completed a PGY1 Pharmacy Practice Residency at the VA Puget Sound Health Care System in Seattle, Washington and subsequently completed a PGY2 Pharmacy Specialty Residency in Solid Organ Transplantation at University Health System/University Transplant Center in San Antonio, Texas. During his PGY2 training, Dr. Newland was winner of the 2015 Texas Society of Health-System Pharmacists (TSHP) poster competition in the PGY2 category for his major research project titled Implementation of a Pharmacist-Run Post-Transplant Diabetes Clinic. Dr. Newland is an active member of American Society of Transplantation (AST), American College of Clinical Pharmacy (ACCP), and American Society of Health-System Pharmacists (ASHP). His clinical interests include cystic fibrosis, cardiothoracic transplantation, and contributing to the paucity of data in cardiovascular risk reduction, including optimal diabetes management, in transplant recipients. Kristina Ngo, PharmD, BCPS is a clinical pharmacy specialist in Home Based Primary Care (HBPC). She received her PharmD from The University of Texas at Austin College of Pharmacy and the University of Texas Health Science Center-San Antonio. She was previously a clinical pharmacy specialist in anticoagulation clinic and also has experience in both the inpatient and outpatient settings at the MEDVAMC. She holds academic appointments as clinical assistant professor at the University of Texas and is a clinical instructor at the Baylor College of Medicine.

7 Jennifer D. Nguyen, PharmD, BCPS, is a clinical pharmacy specialist in primary care. She received her PharmD from the University of Texas College of Pharmacy and then went on to complete a PGY-1 Pharmacy Practice Residency at the MEDVAMC. Dr. Nguyen is actively involved in GCSHP and has served as GCSHP president and treasurer. She is currently a co-editor of the MEDVAMC pharmacy newsletter RX-Press. She holds academic appointments as clinical assistant professor at the University of Texas and is a clinical instructor at the Baylor College of Medicine. In 2011, she received the University of Texas College of Pharmacy Houston/Galveston Region outstanding clinical preceptor award. James Papadatos, PharmD is a clinical pharmacy specialist in primary care. He received his PharmD from the University of Maryland. After graduation, he completed a PGY-1 Pharmacy Practice Residency and a PGY-2 Specialty Residency in Ambulatory Care at the VA Maryland Health Care System in Baltimore. Dr. Papadatos past research projects include: a medication use evaluation of pioglitazone in patients with type 2 diabetes and evaluation of a pharmacist-managed erythropoietin stimulating gent clinic. Caroline N. Pham, Pharm.D., BCPS is a clinical pharmacy specialist in internal medicine. She received her Pharm.D. from the University of Houston College of Pharmacy. She completed a PGY-1 Pharmacy Residency at the MEDVAMC and a PGY-2 Internal Medicine specialty Residency at the University Medical Center Brackenridge (UMCB) Seton Medical Center in Austin, Texas. She was previously a clinical pharmacy specialist working with various interdisciplinary services at Memorial Hermann Northwest in Houston, Texas before joining the MEDVAMC clinical pharmacy team. Dr. Pham is also a clinical instructor at the Baylor College of Medicine.

8 Rosemin Pirmohamed, PharmD is a clinical pharmacy specialist in Prime care. Dr. Pirmohamed received her PharmD from Mercer University College of Pharmacy and Health Sciences in Atlanta, Georgia. She then completed an ambulatory care residency at the Stratton VA Medical Center in Albany, New York. Dr. Pirmohamed currently practices in the Out-Patient Anticoagulation Clinic. She is the PGY-2 Ambulatory Care Residency Program Coordinator and educational liaison for the fourth year experiential rotations for the Colleges of Pharmacy at University of Texas, University of Houston, Texas Southern School of Pharmacy and Texas A&M. She holds academic appointments as a clinical assistant professor at the University of Houston and University of Texas Colleges of Pharmacy and is a clinical instructor at the Baylor College of Medicine. In 2014, she received the University of Texas College of Pharmacy Houston/Galveston Region outstanding Ambulatory Care Preceptor award. Kimthy D. Phung, PharmD is clinical pharmacy specialist in primary care. She received a BS in Biochemistry and Psychology from the University of California at San Diego and a PharmD from the University of California at San Francisco. She completed a pharmacy practice residency at Kaiser Permanente Health Systems in Anaheim, California. In 2009, she received the UT Clinical Preceptor of the Year award. Shaila Sheth, Pharm.D., BCPS is a clinical pharmacy specialist in Internal Medicine. She received her Pharm.D. from the University of North Carolina Eshelman School of Pharmacy, Chapel Hill and completed a PGY-1 Pharmacy Residency at the University of Virginia. She was previously a clinical pharmacy specialist in Internal Medicine at Beaumont Health System, Michigan before joining the MEDVAMC clinical pharmacy team. Dr. Sheth is also a clinical instructor at the Baylor College of Medicine and her clinical interests are infectious diseases and inpatient diabetes management.

9 Hannah Sulik, PharmD, BCPS is a clinical pharmacy specialist in geriatrics. She received her PharmD from the University of Texas College of Pharmacy. Dr. Sulik completed a PGY-1 pharmacy practice residency at the Central Arkansas VAMC in Little Rock, AR and PGY-2 geriatric pharmacy residency at the South Texas VAMC in San Antonio, TX. She practiced as a clinical pharmacy specialist in geriatrics at the Salem VAMC in Salem, Virginia prior to her current position. Tomi L. Swain-Alexander, PharmD, BCPS, CDE is a clinical pharmacy specialist in primary care and Hepatitis C. She received her BS in Chemistry from Southern University A&M College in Baton Rouge, Louisiana and her PharmD from Xavier University of Louisiana in New Orleans, Louisiana. Dr. Swain-Alexander completed a PGY-1 Pharmacy Practice Residency at Baptist Memorial Hospital in Memphis, Tennessee and a PGY-2 specialty residency in Ambulatory Care at the Ralph H. Johnson VA Medical Center in Charleston, South Carolina. She currently serves on the VALOR program advisory board. She holds an academic appointment at the University at Texas College of Pharmacy and is a clinical instructor at the Baylor College of Medicine. Anne M. Tucker, PharmD, BCNSP is a Clinical Associate Professor at the University of Houston College of Pharmacy and a Clinical Pharmacy Specialist in nutrition support at the MED VAMC. She serves as a residency preceptor for nutrition support elective rotations. Dr. Tucker received her B.S. in Chemistry with a Biochemistry emphasis from the University of Arkansas at Fayetteville and her Pharm.D. from the University of Arkansas for Medical Sciences in Little Rock where she also completed an ASHP-accredited PGY-1 Pharmacy Practice Residency. She holds memberships and leadership appointments in the American Society of Parenteral and Enteral Nutrition (A.S.P.E.N.), Board of Pharmacy Specialists Nutrition Support Specialty Council, Society of Critical Care Medicine (SCCM), and American Society of Health-System Pharmacists (ASHP). Dr. Tucker is a member of the MED VAMC Interdisciplinary Nutrition Support Team recognized as an A.S.P.E.N. Nutrition Support Team of Distinction.

10 Matthew A. Wanat, PharmD, BCPS is a Clinical Assistant Professor at the University of Houston College of Pharmacy and a Clinical Pharmacy Specialist in the Cardiac ICU at the Michael E. DeBakey VA Medical Center. He received his Doctor of Pharmacy from Northeastern University in Boston, MA. Dr.Wanat completed a PGY1 Pharmacy Practice Residency and PGY2 Critical Care Pharmacy Residency at Houston Methodist Hospital. Dr. Wanat precepts students and residents in the Cardiac ICU at MEDVAMC and on academia rotations at the University of Houston. Dr. Wanat is also a Clinical Instructor with Baylor College of Medicine. Dr. Wanat sits on the Board of Directors for the Society of Critical Care Medicine (SCCM) Texas Chapter, and received the Excellence in Leadership Award from SCCM in He has several peer-reviewed publications in the areas of anticoagulation, sedation and the scholarship of teaching. Dr. Wanat is a peer-reviewer for several journals including the American Journal of Pharmacy Education (AJPE), and is on the editorial board for the Texas Society of Health System Pharmacists Journal. Dr. Wanat is an item writer for the NAPLEX, Board of Pharmaceutical Specialties and Society of Critical Care Medicine exams. Dr. Wanat s teaching has been recognized by the college with the Rho Chi Teaching Excellence Award and the Faculty Excellence in Teaching Award. In his free time, Dr. Wanat is an avid sports fan and enjoys traveling. Sonya R. Wilmer, PharmD, BCPS, VHA-CM is the Ambulatory Care Pharmacy Practice Residency Program Director and a clinical pharmacy specialist in primary care. She received her BS in Chemistry from Dillard University in New Orleans, Louisiana and her PharmD from Mercer University College of Pharmacy and Health Sciences in Atlanta, Georgia. She completed a residency through Children's National Medical Center and George Washington University Medical Center in Washington, DC and is a graduate of the ASHP Pharmacy Leadership Academy. Dr. Wilmer is a member of ASHP, GCSHP and American College of Clinical Pharmacy. She holds appointments as clinical assistant professor at UH and TSU Colleges of Pharmacy and is a clinical instructor at the Baylor College of Medicine. Currently she is the Chairperson of the VISN 16 Pharmacy Technician Professional Standards Board. Primary Care Community Based Outpatient Clinics (CBOCs) Preceptors Yavonne Q. Martinez, PharmD, MPH, BCPS graduated from The University of Texas at Austin College of Pharmacy and the University of Texas Health Science Center-San Antonio. She completed her Primary Care Specialty Residency at The University of Mississippi Medical Center and obtained her Master of Public Health-Community Health Practice Module from The University of Texas Health Science Center at Houston School of Public Health. Dr. Martinez s clinical interests are management of chronic disease states: diabetes mellitus, hypertension and hypercholesterolemia in the rural communities. She is currently a member of the American Pharmacists Association and the Lake Houston

11 Pharmacy Association. Her primary interests involve spending quality time with her family. Scott McDowell, Pharm.D., BCPS, CDE is a clinical pharmacy specialist in primary care. Dr. McDowell is a board certified pharmacotherapy specialist and is a board certified diabetes educator. Dr. McDowell graduated with his Pharm.D. from the University of Georgia College of Pharmacy with honors and received residency training from the University of Houston College of Pharmacy/Health Spring and Stanford Hospital and Clinics. Dr. McDowell is a reviewer of the Journal of Managed Care and Specialty Pharmacy, is on the Grants Review Committee of the American Pharmacists Association, and has served as an item writer for the Board of Pharmaceutical Specialties Pharmacotherapy exam. Dr. McDowell is currently a coach in the Healthy Outcomes through Patient Empowerment (HOPE) study being conducted by Baylor College of Medicine and the Michael E. DeBakey VA Medical Center. Dr. McDowell is a member of the Gulf Coast Society of Health- System Pharmacists. In his spare time, he enjoys spending time with his wife, going to church, eating at new restaurants, painting, running, and playing percussion. Jessica McMahan, PharmD, BCACP, CDE is a clinical pharmacy specialist in primary care at Lake Jackson CBOC. She received her BA degree from Duke University and her PharmD from the University of Wyoming College of Pharmacy. Prior to coming to the Houston VA in 2013, Dr McMahan spent 5 years at Landstuhl Regional Medical Center in Germany for the US Army. Cindy Nguyen, PharmD, BCPS is a clinical pharmacist specialist in primary care for the Katy Outpatient Clinic. She received her BS in Biology and Chemistry at Newman University and PharmD from The University of Michigan College of Pharmacy. She completed her PGY-1 Pharmacy Practice Residency with Kaiser Permanente Orange County California. She currently holds membership with the American Society of Health-Systems Pharmacists (ASHP), the American College of Clinical Pharmacy and American Pharmacists Association.
