Business Travel Accident at a Glance The Teradyne Business Travel Accident (BTA) Insurance Plan covers you when you're traveling on company business. Any benefits you may receive from this plan are in addition to any benefits provided through the Group Life Insurance Plan or other programs. You're automatically covered if you are a regular employee. Teradyne pays the entire cost of the plan. Who's Eligible for Business Travel Accident You are automatically eligible for coverage through this plan if you are: 1. Active full-time U.S. and International Employees working 20 or more hours per week (excluding those International Employees who reside and are employed in Europe) of the Policyholder who are not in any other class 2. Active part-time U.S. and International Employees working 20 or more hours per week (excluding those International Employees who reside and are employed in Europe) of the Policyholder who are not in any other class 3. All Eligible Spouses and Eligible Dependent Children, who are traveling with an Employee on the business, authorization and expense of the Policyholder, who are not in any other Class 4. All LitePoint active employees listed on file with the Policyholder Coverage begins on the date you start work and continues for as long as you remain employed by Teradyne. Business Travel Accident Coverage 24 Hour Business Travel Accident 24-Hour Business Travel Accident (BTA) coverage applies to all circumstances arising from and occurring while you are traveling on assignment by or at the direction of Teradyne. Your spouse and dependent children traveling with you are also covered, provided the trip is authorized and at the expense of Teradyne. Coverage begins at the actual start of a trip; whether it is your home or regular Teradyne office location, whichever occurs last. Coverage ends immediately upon return to your home or regular Teradyne office location, whichever occurs first. Teradyne s BTA Insurance Plan contains numerous coverage enhancements such as extraordinary commutation on premises, bomb scare, hijacking, exposure to elements and disappearance. Travel Guard Assist Twenty-four hours, seven days a week, you can reach toll free emergency telephone assistance service while traveling at one of the following numbers in the event of a travel-related emergency: From within the U.S., Canada, Puerto Rico or U.S. Virgin Islands 877.244.6871 From anywhere else call collect to: +1 715.346.0859 In the event of a travel-related emergency Special assistance in replacing lost or stolen travel documents, including passports. Emergency funds transfer Special assistance in locating the nearest, most appropriate medical care. Verification of insurance coverages facilitating entry and admissions into hospitals and other medical care providers. Assistance in establishing contact with family, personal physician and employer as appropriate. Special assistance in the coordination of direct claims payment. Management, arrangement and coordination of emergency medical transportation and evacuation as necessary. Translation services and referral to local interpreters as necessary.
Knowledgeable legal referral assistance. Courtesy assistance in securing incidental aid and other travel-related services. Coordination of securing bail bonds and other legal instruments. BTA Benefit Amounts You may receive benefits from this plan if you have an accident while traveling on business for Teradyne. The benefit you may receive is based on your regularly scheduled hours and the coverage level for the loss you suffer. Benefits for Employees Who Work 20 or More Hours per Week Your full benefit amount is five times your basic annual earnings at the time of the accident, rounded to the nearest $1,000. The minimum benefit is $150,000, and the maximum is $1 million. Basic annual earnings mean model compensation. It includes targeted commission earnings or a model variable compensation; however, it does not include overtime pay, shift differentials, commissions or incentive payments. LitePoint employees basic annual earnings will be based on base salary. The maximum benefit amount payable is $20,000,000 per aircraft accident. If an aircraft accident results in benefit amounts which when totaled exceeds $20,000,000; the benefit will be divided proportionally. Benefits for Employees Who Work Up to 20 Hours per Week Your full benefit amount and maximum benefit is $100,000. War Risk Maximum If more than one employee suffers a loss as a result of declared or undeclared war or act of terrorism, anywhere in the world except the United States, Iran, Iraq, or Afghanistan maximum benefit amount payable is $10,000,000. If an accident results in benefit amounts which when totaled exceeds $10,000,000; the benefit will be divided proportionally. Full Benefits If the result of a business travel accident is one of the following losses, you (or your beneficiary) may receive the corresponding benefit amount: Accidental Loss of: Percent of Benefit Amount Life 100%* Both Hands or Both Feet 100% Sight of Both Eyes 100% One Hand and One Foot 100% One Hand and the Sight of One Eye 100% Speech and Hearing in Both Ears 100% One Hand or One Foot 50% Sight of One Eye 50% Speech or Hearing in Both Ears 50% Thumb and Index Finger of Same Hand 25% * An additional 10% of the principle sum, maximum $50,000, will be paid in the event of accidental death where you are driving or operating a private passenger automobile and wearing a seat belt. An additional 10% of the principle sum, maximum $25,000, will be paid if the seat belt benefit is payable and if the insured is positioned in a seat protected by a properly functioning original factory installed airbag. Under certain circumstances, such as loss of a limb, loss of use of a limb, loss of speech of hearing or lapsing into a coma, Teradyne s BTA Insurance Plan contains reduced benefits. Medical Evacuation and Repatriation In the event of an accidental bodily injury, illness or disease, emergency transportation will be provided from the location where the injury or illness occurred to the nearest hospital where appropriate medical
treatment can be obtained and or repatriation. The medical evacuation and repatriation must be ordered by a physician, who certifies that the transportation and medical treatment are necessary and appropriate and approved by Travel Guard Assist. In the event of your death, repatriation benefits will also cover necessary expenses for embalming, cremation, and transportation of your remains back to your residence. The maximum benefit amount for medical evacuation and repatriation is up to $250,000. Out of Country Medical Accident and Sickness Medical In the event of an accidental bodily injury or Emergency Sickness within 30 days of the date of the accident, and requires treatment by a Physician, our coverage will pay the Usual and Customary Charges incurred for Covered Medical Services received due to that Injury up to $100,000 per Insured Person for all Injuries caused by the same accident. This benefit is payable for such charges incurred and within 26 weeks after the date of accident causing that Injury or Sickness. Access of this benefit would be through Travel Guard Assist. Security Evacuation Expense In the event of the need for a security evacuation due to certain events taking place in the country you are traveling (provided it is a covered country) such as hostile political or military actions, verifiable physical attacks upon you, or even Natural Disasters, our coverage will provide $100,000 of expenses needed to cover an evacuation to the nearest place of safety where both access to transportation home and temporary lodging is available. Expenses would include food, lodging, and physical protection while traveling to the nearest place of safety. Access of this benefit would be through Travel Guard Assist. Family Loss of Life Coverage If a business travel accident results in loss of life of your spouse and/or unmarried dependent children traveling with you as authorized and at the expense of Teradyne, you or (or your beneficiary) may receive the corresponding benefit amount: Family Member: Benefit Amount Spouse $50,000.00 Each Dependent Child $25,000.00 BTA Exclusions Business Travel Accident Plan coverage does not apply to accidents occurring in the following situations: Aircraft Owned, Lease Or Operated This coverage does not apply to an accident occurring while you are in, entering or exiting any aircraft owned, leased or operated by you or Teradyne. Aircraft Pilot Or Crew This coverage does not apply to an accident occurring while you are in, entering, or exiting any aircraft while acting or training as a pilot or crew member; except if you are temporarily performing pilot or crew functions in a life threatening emergency. Armed Services This coverage does not apply to loss caused by full time duty in the armed forces, National Guard, or organized reserve corps of any country or international authority except while on short-term National Guard or reserve duty for regularly scheduled training purposes. Disease Or Illness This coverage does not apply to loss caused by or resulting from your emotional trauma, mental or physical illness, disease, pregnancy, childbirth or miscarriage, bacterial or viral infection, or bodily malfunctions.
You will be covered if a bacterial infection is caused by an accident or from accidental consumption of a substance contaminated by bacteria. Suicide or Intentional Injury This coverage does not apply to suicide, attempted suicide or loss that is intentionally self-inflicted. Drugs or other Intoxicants This coverage does not apply while the insured person is under the influence of intoxicants or drugs unless taken under the direct advice of and as specified by a physician. Medical or Surgical Treatment This AD&D coverage does not apply if the loss is a result from medical or surgical treatment of sickness, disease, mental incapacity or bodily infirmity whether the loss results directly or indirectly from the treatment. Stroke etc. This coverage does not apply in the event of a loss resulting directly from stroke or cerebrovascular accident or event; cardiovascular accident or event; myocardial infarction or heart attack; coronary thrombosis or aneurysm. How to File a BTA Claim To file a claim, you or your beneficiary should contact: the Local Human Resource department for the office or; Human Resources Service Center, MS NR-700-2-1 700 Riverpark Drive North Reading, MA 01864 978-370-3041 Our Human Resource Department/Service Center will provide the proper claim forms and/or assist you in notifying the insurance company. Proof of Loss Written proof of loss must be furnished to the company within 90 days after the date of the loss, or is soon as is reasonably possible. If the loss is one for which this Policy requires continuing eligibility for periodic benefit payments, subsequent written proofs of eligibility must be furnished at such intervals as the company may reasonably require. Claim Payments For all claims with monthly benefits, you or your beneficiary will receive the applicable benefit amount no less frequently than monthly during the continuance period. At the end of this period, the carrier will pay any remaining balance of the benefit amount immediately. All payments are subject to receipt of complete Proof of Loss. For all other claims, you or your beneficiary will receive the benefit immediately upon receipt of due written proof of loss. Beneficiary Your loss of life benefits will be paid to the beneficiary you have designated. If your beneficiary is deceased, or you have not elected a beneficiary, the benefit will be paid in the following order: Your spouse; In equal shares to your surviving children; In equal shares to your surviving parents, In equal shares to your surviving bothers and sisters;
To your estate. All other benefits amounts will be paid to you. Terms of Contract Business Travel Accident Plan benefits are subject to the terms of the contract between Teradyne, Inc. and the insurance carrier, National Union Fire Insurance Company of Pittsburgh, Pa. Teradyne has no responsibility to pay benefits beyond those that can be provided by the contract terms.
