You ll receive a benefit booklet with details about your coverage. Value-added services are not part of the contract. Information To Know Guaranteed Coverage: The maximum amount of coverage available during your initial enrollment period with no medical information required. Eligibility: Eligibility: You are eligible if you are an active, full-time employee (except Campus Minister, part-time, seasonal, temporary or contract employees) working at least 30 hours per week. Special eligibility requirements may exist for step, foster, adopted, legal age or other child relationships. Additional information may be necessary to determine child eligibility. Additional eligibility requirements may apply. Your Benefits at a Glance Your Coverage Available in $10,000 increments. Minimum: $10,000 Maximum: Lesser of $500,000 or 5 times annual earnings Proof of good health is required for life insurance amounts greater than: If you are under age 70: $200,000 If you are age 70 and over: $10,000 Spouse Coverage Available in $5,000 increments. Minimum: $5,000 Maximum: 50% of employee coverage, not more than $100,000 Proof of good health is required for life insurance amounts greater than: If your spouse is under age 70: $50,000 If your spouse is age 70 or over: 10,000 Child Coverage You may elect coverage in the amount of: $5,000, or $10,000 Child benefits cannot exceed 50% of employee s coverage. Portability You may continue coverage for yourself and your covered dependents until age 70 if you cease to qualify as a member. The employee or spouse must enroll within 60 days from the date they cease to qualify as a member. Refer to your benefit booklet for maximum age requirements. Age Reductions 35% reduction of coverage on June 1 after reaching age 65, with an additional 15% reduction on the June 1 after reaching age 70. Coverage During Disability Accelerated Benefit Age reductions apply to the benefit amount after proof of good health. If you become totally disabled before age 60, coverage will continue and premium will be waived for you. You must be totally disabled for 9 months before the waiver begins. Coverage continues without premium payment until you recover or reach Social Security Normal Retirement Age, whichever occurs first. No benefits will be paid for any disability that results from: willful selfinjury or self-destruction, while sane or insane / war or act of war / voluntary participation in an assault, felony, criminal activity, insurrection, or riot. If you are terminally ill you can receive up to 80%, not to exceed $250,000, of your life coverage benefit in a lump sum as long as: 1 of 2

2 Coverage Outside United States Benefit Exclusion Your life expectancy is 24 months or less (as diagnosed by a physician). Your death benefit is at least $10,000. When you use the accelerated benefit, your death benefit is reduced by the accelerated benefit payment. There are possible tax consequences to receiving an accelerated benefit payment. You should contact your tax advisor for details. Receipt of accelerated benefits could also affect eligibility for public assistance. The charge for this benefit is included in your premium. Benefits will not be paid if you or your dependents are outside the United States for certain reasons for more than 12 months. Benefits are not paid for employees and spouses who commit suicide within the first 24 months of coverage (prior group voluntary life coverage applies towards the 24 month time period). Termination Your coverage doesn t terminate as long as you are an active employee paying premiums and your employer continues to sponsor the program. Coverage may be terminated with 31 days notice for a false or fraudulent claim. Individual Purchase Rights In termination situations, you, your spouse and your children may convert coverage to individual life coverage. Upon coverage termination your employer is required to inform you of your individual purchase rights to convert to an individual policy without proof of good health. The amount you can purchase varies depending on the termination situation. Claim Processing Principal Life makes claim administration easy and convenient for employers by offering an online life claim form. Once the form is complete, employers submit the information directly over a secure, confidential Web site, expediting the claim review process. The employer can choose to use the online form or a printable version that can be faxed or mailed. Along with the online claim form, Principal Life also provides Express Claim Processing for claims that meet certain criteria. Through the Express Claim Process, decisions are reached within five working days without the employer or beneficiary submitting paperwork. Claims/Beneficiary Information You can name anyone as your beneficiary except your company. You can also change beneficiaries at any time. Accidental Death & Dismemberment (AD&D) Coverage Accidental Death & Dismemberment Coverage pays a benefit equal to your group term life coverage amount when loss occurs within 365 days of an accident. Retiree coverage is not available. We pay the full benefit when you lose: your life both hands both feet sight of both eyes one hand and sight of one eye one foot and sight of one eye one hand and one foot We pay half the benefit when you lose: one hand one foot sight of one eye We pay one fourth of the benefit when you lose: the thumb and index finger on the same hand. Additional Benefits Included with AD&D Seatbelt/Airbag If you die in an automobile accident, AD&D pays an additional $10,000 if you were wearing a seatbelt or were protected by an airbag. Repatriation If you die at least 100 miles from your permanent residence, AD&D pays up to $2,000 for preparation and transportation of your body. Loss of Use/Paralysis AD&D pays a benefit as follows: 100% for quadriplegia; 50% for paraplegia, hemiplegia, loss of use of both hands or both feet, or loss of use of one hand and one foot; or 25% for loss of use of one arm, one leg, one hand or one foot. Loss of use means the total and irrevocable loss of voluntary movement for 12 consecutive months. Paralysis must be permanent, complete and irreversible. Loss of Speech and/or Hearing AD&D pays a benefit of 100% for loss of both speech and hearing; 50% for loss of speech or hearing; 25% for loss of hearing in one ear. Loss must be irrevocable and continue for 12 consecutive months. Exposure Exposure to the elements is considered an accidental injury if you incur a covered loss within one year of exposure resulting from an accidental injury. Disappearance AD&D will pay for loss of life if you disappear while you were a passenger in a conveyance involved in an accidental wrecking or sinking and your body is not found within one year of the accident. 2 of 3

7 Limitations and Exclusions for Accidental Death & Dismemberment Coverage does not include payment for more than the benefit stated in the schedule for losses resulting from a single accident. Benefits are not paid for losses resulting from: Willful self-injury or self-destruction while sane or insane. Disease or treatment of disease or complications following the surgical treatment of disease. Voluntary participation in an assault, felony, criminal activity, insurrection, or riot. Participation in flying, ballooning, parachuting, parasailing, bungee jumping, or other aeronautic activities, except as a passenger on a commercial aircraft or as a passenger or crew member on a company owned or leased aircraft on company business. War or act of war. The use of alcohol if, at the time of the injury, your alcohol concentration exceeds the legal limit allowed by the jurisdiction where the injury occurs. Your operation of a motor vehicle or motor boat if, at the time of the injury, the alcohol concentration exceeds the legal limit allowed by the jurisdiction where the injury occurs. Duty as a member of a military organization. Your use of any drug, narcotic, or hallucinogen not prescribed by a licensed physician. 9 Policyholder: GLAHIT - Schreiner University Long-Term Disability (LTD) Coverage Effective Date: 3/1/2014 This is a summary of your long-term disability coverage from Principal Life Insurance Company. Long-term disability coverage is designed to replace a portion of your income when you are disabled because of injury or illness. Eligibility: You are eligible if you are an active, full-time employee (except Campus Minister, part-time, seasonal, temporary or contract employees) working at least 30 hours per week. Your Benefits at a Glance Qualifying as Disabled You may qualify as disabled during the elimination period and own occupation period if because of sickness, injury or pregnancy, one of the following applies: You cannot perform the majority of the substantial and material duties of your own occupation. You are performing the duties of your own occupation on a modified basis and lose at least 20% of the income you earned before becoming disabled. You are performing the duties of any other occupation and lose at least 20% of the income you earned before becoming disabled. After completing the own occupation period, you may qualify as disabled if because of sickness, injury or pregnancy, one of the following applies: You cannot perform the majority of the substantial and material duties of any occupation for which you are or may become qualified based on your education, training or experience. You are performing the substantial and material duties of your own or any other occupation on a modified basis and lose at least 20% of the income you earned before becoming disabled. When Benefits Begin The monthly income you earned before becoming disabled is referred to as predisability earnings. LTD benefits begin after you have been disabled for 90 days. This is called the elimination period. It can be satisfied with days of total or partial disability. Benefits if Not Working If you recover and return to work during the elimination period and become disabled again, you may not have to satisfy a new elimination period. If you become disabled again, your elimination period will pick up at the point where it was left off when you recovered. You have a period twice as long as the elimination period to satisfy the required number of days of disability. When you are unable to work in any capacity during the benefit payment period, your monthly benefit equals your primary monthly benefit, less income from other sources. Your primary monthly benefit is equal to 60% of your earnings before becoming disabled, but will not exceed $7,000. Benefits if Working Your monthly benefit will not be less than the minimum monthly benefit of the greater of 10% of your primary monthly benefit or $100. If you are able to work while disabled, you may still be eligible to receive a disability benefit. If you are working during the benefit payment period, your monthly benefit for the 12 month work incentive period is the lesser of: 100% of the indexed earnings you received before becoming disabled, 1 of 5

10 less income from other sources, less current earnings; or Your primary monthly benefit, less income from other sources. Your Benefit Duration After the work incentive period, your monthly benefit equals your primary monthly benefit, less income from other sources and multiplied by your income loss percentage. Your age at the time of disability affects the length of time you are eligible to receive disability benefits. If you become disabled before reaching age 65, your disability benefits are payable until the later of: The date you reach Social Security normal retirement age: or 36 months after your benefit payment period begins. If you become disabled at or after age 65, your disability benefits are payable until the later of: The date you reach Social Security normal retirement age; or The date the benefit payment period reaches the number of months shown below. Rehabilitation Services and Benefits Rehabilitation Services While disabled, you may qualify to participate in a rehabilitation plan. Our rehabilitation staff works with you, your physician(s) and your employer to create an individual rehabilitation plan to assist you in returning to work. Return to Work Child Care Benefit Mandatory Rehabilitation If you are not disabled, but have a condition that could prevent you from performing the substantial and material duties of your own occupation, preventive rehabilitation services may be offered. This benefit helps you remain on the job or return to work by helping you cover the cost of child care expenses. We will reimburse you for 100% of your child care expenses, up to $350 per month for up to twelve months. Return to work child care benefits begin after work incentive benefit payments end. This provision indicates that, if appropriate, you may be required to participate in an individual rehabilitation plan. Any expenses associated with the rehabilitation plan will be paid for by Principal Life. If you do not comply with the rehabilitation plan without good cause, your disability benefits may cease. 2 of 5

11 Additional Benefits Survivor Benefit Continuation of Coverage for Sabbatical A survivor benefit is a lump sum payment issued to your survivors, should you die while receiving disability benefits. The benefit payment is equal to three times your primary monthly benefit. If you stop working due to a sabbatical, your disability coverage can be continued with payment of premium for up to one year. Limitations of Benefits Limitations Treatment of Mental Health Conditions and Drug and Alcohol Abuse Conditions No benefits will be paid for disabilities resulting from: Willful self injury, while sane or insane War or an act of war Participation in an assault or felony A new or continuing disability that begins after your benefit payment period has ended, but you have not returned to active work A disability is considered due to alcohol, drug or chemical abuse, dependency or addiction or a mental health condition if the disability is caused by one of these condition(s) and not by other disabling conditions. Maximum benefit payment periods for: Mental health conditions 24 months Alcohol, drug or chemical abuse conditions 24 months The benefit payment period listed above is a lifetime maximum for all periods of disability. All disabilities from conditions with the same maximum benefit payment period contribute towards one lifetime maximum. Preexisting Conditions However, if at the end of the benefit duration, you are confined in a hospital or any other type of facility providing treatment for any of these conditions, the benefit payment period may be extended to include the time period you are confined for treatment. A preexisting condition is a sickness or injury, including all related conditions and complications, or pregnancy for which you: Received medical treatment, consultation, care or service; or Were prescribed or took prescription medications in the three months prior to your effective date under this policy. Benefits will not be paid for disabilities resulting from preexisting conditions unless, when you become disabled, you have been actively at work for one full day after completing the earlier of: Three consecutive months of coverage under the policy in which you received no treatment, including prescription medication, for the disabling condition; or 12 consecutive months of coverage under the policy. Preexisting condition exclusions also apply to benefit increases due to: Policy amendments Changes in earnings of 25% or greater Terms you should know: Income from Other Sources Income you receive from other sources can be deducted from your primary monthly benefit amount. Other sources include: All retirement or disability benefits that you and your dependents receive or could have received, from Social Security or other government agencies Workers Compensation benefits Income from state disability plans Payments from policies that provide coverage for time away from work, if paid in part by or deducted from payroll by the policyholder Income from other group disability policies Disability or retirement benefits, paid by pension plans sponsored by the policyholder 3 of 5

12 Income received from no-fault auto laws Renewal commissions received from the policyholder Severance pay All payments for the month that the member receives under state unemployment laws Other income sources do not include: Individual disability coverage Profit sharing plans Thrift savings plans Nonqualified deferred compensation plans 401(k) plans Individual retirement accounts (IRA) Stock ownership plans Keogh (HR-10) plans Any cost of living increases paid in connection with other sources of income Social Security or pension plan payments that were being received prior to the current disability Any income the member receives for services rendered prior to the member s date of disability Modified Basis You are considered to be working on a modified basis if you are working on either a part-time basis or you are able to perform some, but not all, of the substantial and material duties of an occupation on a full-time basis. Monthly Payment Limit The benefits paid to you are reduced if your total income exceeds 100% of your predisability earnings. Total income includes: Your normal benefit payable Additional benefits payable under your policy Return to work earnings Other Income Sources defined in the policy Sick pay, salary continuance and PTO Own Occupation The occupation you are routinely performing when disability begins. Own occupation does not mean the specific tasks or job you are performing for the policyholder or at a specific location. Own Occupation Period The period of time you must be unable to perform the duties of your own occupation. Your own occupation period is the first 2 years of the benefit payment period. Predisability Earnings The monthly earnings you receive before becoming disabled as defined in the policy. Primary Monthly Benefit - Your primary monthly benefit is equal to 60% of your earnings before becoming disabled, but will not exceed $7,000. Substantial and Material Duties The essential tasks generally required by employers from those engaged in a particular occupation that cannot be modified or omitted.
