Income Tax Return Information Checklist for Company For the Year Ended 30 th June 2013 IMPORTANT NOTE WORKFLOW MANAGEMENT As your 2013 Tax Return is due for lodgment by the 15 th May 2014 (or as otherwise notified) please advise: a. if your Return is required urgently (the normal turnaround time is six to eight weeks). If the relevant information and documentation is not received by the due date, we may not be able to guarantee that the Tax Return is lodged in time. If you prepare & lodge your own quarterly BAS returns, please provide a copy of each of the BAS returns and we shall rely on the data that you lodged with the ATO to prepare your financials. Please note that we need the GST component of each expense so that we can work out the GST input tax credit claimed by the company. We also need bank statement balances of your business accounts at 30 June 2013. Motor Vehicle: Did you use your own car for Business purposes through the year: If yes then please provide a copy of the following: Log Book Method Business % use Please provide details of all expenses you incurred over the financial year including fuel, repairs or maintenance, registration & insurance, etc. Kilometres Method: You haven t kept a log book but use your car for business. Let us know how many Kilometres you would have travelled for business. The maximum the tax office allows you to claim using this method is 5000 Kilometres. Kilometres: Car engine size: (in litres e.g. 1.6 litres): Other Details required: If you have a loan for the vehicle please provide details of the interest you paid over the year and the cost of the car, if you have a hire purchase please provide a copy of the purchase contract. If you had a lease for your vehicle please provide figures of your lease payments 2. Distribution from other trusts/partnerships Do you receive distributions from other trusts/partnerships? If you received distribution from another private trust or partnership, please provide a copy of the Tax Return, if available. If you received distribution from a public unit trust, please provide a copy of the annual distribution summary. 3. Rent Do you receive rental income? If you do, please provide copies of the monthly rental statements or a copy of the annual rental summary, if available. Please supply settlement sheets and purchase documents if you acquired the property after 1 July 2012. If you have more than one property, please list all investment properties. 2

Dividends Please provide copies of dividend statements of income received: also note that if you are on the dividend reinvestment plan which means you don t physically get money to bank, the company uses that money to issue you more shares, therefore this is still your income and MUST go in your Return. Tax loss Do you have a loss to be brought forward from 2012? If yes, please advise the amount of carry-forward loss. 9. Superannuation: Have you made any contribution for your employees to a superannuation fund? If yes, please list the amount of the contributions made for each employee. 10. Other: Any other details of information which you are unsure of which category and would like us to be aware of: 3
11. Did you pay any expenses in cash? If so, please provide us with details. 12. Stock on hand: What is the value of your stock in hand at 30 June 2013? 13. Creditors: have you provided a FULL LIST of amounts owing BY YOU at 30 June 2013 including, name, expense, description and amount of EACH creditor (GST inclusive)? 14. Debtors: have you provided a FULL LIST of amount owing TO YOU at 30 June 2013 Including name and amount (GST inclusive)? 15. Assets: have you provided a list of any assets purchased or disposed of? 16. Hire purchase/lease agreements: have you provided us with a copy of any Hire Purchase Agreements for plant, vehicles, etc purchased during the 2013 year? 17. Bank Loan Documents: have you provided us with a copy of any bank loan documents from any loans or advances obtained during the 2011 financial year? 18. Wage records: if you employ staff have you provided us with copies of group certificates and PAYG Withholding Reconciliation Statement? If no, have you provided us with the gross, tax withheld and net amount of wages paid to each employee. The following format can be used. Employees Name Gross Wages Tax Withheld Net Wages 19. Business Activity Statements: if you have prepared and lodged your own Business Activity Statements, have you provided us with a copy of all your statements? 4
20. Balance Sheet - Equity: Details of any changes to shareholding. Details of loans from shareholders or partners. Details of any increase or decrease to reserves. 21. Additional Information: Loans, payments or debt forgiveness made to shareholders or associates of the shareholders, if private company. Also details of any Loan Agreements in place or made during the year. Auditor s report (if applicable). Copies of Instalment Activity Statements and/or Business Activity Statements lodged for the income year. Copies of minutes of company meetings. If you have any doubt about any income or expenses the company has received or incurred, please forward document. Any other information that you think is relevant. Please note that you may be required to forward some or all of the above documents to us for preparing your financial statements and Tax Return.
