MIAMI COUNTY Major Findings Miami County has Significant Coordination in Place Major purchaser contracts with one provider - MCTS Contracts are based on fully allocated costs with adjustments for changing situations Focus Populations (seniors, persons with disabilities and low income persons) can travel countywide from early morning to late evening on weekdays, with more limited service on weekends MCTS will provide trips anywhere in county (based on sufficient advance notice and capacity currently using 7 days advance notice) MCTS provides service from 5:00 am to 8:00 pm every weekday, from 6:00 am to 7:00 pm on Saturdays and from 8:00 am to 2:00 pm on Sundays 8

Focus Populations (seniors, persons with disabilities and low income persons) making trips in proximity to GDRTA fixed routes can travel from early morning until late at night The level of service (hours of operation and frequency of service) is higher in the more urbanized portions of the GDRTA service area providing greater travel opportunities for the focus populations living and traveling in those areas Persons with disabilities certified for Project Mobility can travel at the same times as the users of GDRTA fixed routes as long as their trip origin and destination are within the designated three-quarter mile corridor surrounding GDRTA fixed routes Focus Populations that do not fit into the above categories have limited transportation options unless they are seniors residing in specific communities providing transportation service, or they receive transportation as a client of a human service agency Focus Populations have very limited options to travel outside Montgomery County 13

PREBLE COUNTY Major Transportation Providers (2 or more vehicles in service daily) Preble County MRDD 15 vehicles 51,400 trips per year Provides client transportation countywide with occasional trips to Dayton as required Preble County Council on Aging 11 vehicles 11,700 trips per year Serves seniors who are county residents Provides Medical and Nutrition Transportation under contract with the Area Agency on Aging Service priority for medical trips eligibility screened to assure service to those who need it most Community Action Partnership 4 vehicles 4,900 trips per year Provides Headstart, medical and jobs trips in Preble County under contract to DJFS 14

PREBLE COUNTY Major Transportation Purchasers Preble County DJFS Contracts for Medicaid NEMT and Required Work-related transportation services Contract providers include Community Action Partnership and United Transportation Service Preble County Veterans Services Contracts for medical trips using United Transportation Service Preble County Mental Health and Recovery Board Purchases complete services, including transportation, from Samaritan Behavioral Health Major Findings Preble County has some Coordination in Place Two major purchasers contract with two providers Focus Populations (seniors, persons with disabilities and low income persons) have very limited travel options With no public transportation in place, the focus populations must depend entirely on being eligible for the available human service agency transportation services 15
