3 Appointment Transportation Options For Assistance to find transportation myride2 ( ) This call is toll-free, and multilingual. Myride2 offers help to seniors and adults with disabilities in Oakland and Macomb counties to find transportation. They help coordinate your transportation options with one call. They do not offer rides, but do help you locate a transportation provider and make arrangements. There is no charge for their coordinating services, but the actual transportation provider may charge for their services. Myride2 is a service of the Area Agency on Aging 1-B. Wheelchair-Accessible Van Services Ambu Trans Weekday: $27 pick up plus $2.50 per mile Weekend: $37 pick up plus $3.00 per mile Angel Healthcare Express $32 one way, $2.50 per mile Auburn Hills Senior Services Open to age 65+ or disabled residents of Auburn Hills. Travels within five mile radius of Auburn Hills for medical appointments. 24 hours advance notice. Give-A-Lift Prices vary Hour Transportation $35 pick up plus $2.75 per mile Jewish Family Service

North Oakland Transportation Authority Free; open to age 60+ residents of Addison Township, Leonard, Lake Orion, Orion Township, Oxford and Oxford Township. Some weekend service available. Call for service area information and hours. Disabled riders under 60 need healthcare provider authorization. Website forms must be completed to pre-register and be interviewed before first ride. Escorts allowed but must be registered with the dispatch office. OPC (Older Person s Commission) Rochester-based (call for reservations Monday Friday 9 am 2:30 pm) $3 one-way suggested contribution, $2.50 for escorts. Limited to Rochester, Rochester Hills and Oakland Township residents age 60+, and destinations within Rochester, Rochester Hills, and Oakland Township. Reddi Ride $35 pick up for wheelchair; $25 for ambulatory, plus $3.00 per loaded mile. $40 hourly rate to wait. $.50/unloaded mile over 20 miles from base. Two days notice preferred. Wheelchair rental available at $10. Premium rates on Saturdays, Sundays, and from 6 p.m. to 6 a.m. Holiday transportation available. Negotiable rates for long-term clients. Rochester Area Neighborhood House Transportation Must live within Rochester, Rochester Hills, Auburn Hills east of I-75, Oakland Township, Addison Township and be low- or moderate-income. 48 hours notice required. Call for reservations Monday Friday 8:30 am to 3 pm. Closed Mondays and weekends. No set charge; donations accepted. Will travel up to 15 miles. Transportation available first-come first-served Tuesday through Friday between 7 a.m. and 3 p.m. Star $39 pick up plus $2.00 per mile 4
5 Transport Care Services (based in Rochester Hills) From 7 am - 5 pm: pick-up fee of $30 per one-way wheelchair transport (ambulatory patient $20), plus $3 per mile. Holidays, weekends, and afterhours 5:01 pm 7 am: pick-up fee of $40 per one-way wheelchair transport (ambulatory $30), plus $3 per mile. One way or round trip available, and up to three friends or family members can ride along for free. Services primarily Oakland and Macomb counties, but can go elsewhere. Short notice is acceptable; at least one hour, and 24 hour notice preferred. Has own wheelchairs available. Open 365 days a year, 24 hours daily. Universal $45 pick up plus $2.00 per mile Worry Free Transportation Cars and wheelchair vans. Some insurances accepted hours notice. ZCD Transportation On-call line: $35 pick up plus $3.00 per mile 5
6 Volunteer Transportation Providers Some organizations provide transportation services by recruiting and screening volunteer drivers. There is generally no charge, but donations are usually accepted. Check when calling. Macomb County Interfaith Volunteer Caregivers Matches volunteers of all ages with older or disabled adults for transportation assistance. Donations accepted. Calls accepted Monday Thursday, 9 a.m. to 4 p.m. Macomb Family Services Romeo Shelby Troy People Concerned Volunteer Drivers Open to Troy residents only. Free transportation provided by volunteers who drive their own vehicles to take seniors for medical appointments. Scheduling depends on volunteer availability. Troy Medi-Go Plus Phone: Troy residents only. This door-to-door small bus service provides transportation for Troy residents age 60 and older and to disabled residents of all ages to medical offices in Troy and the surrounding area, Monday-Friday from 8:30 a.m. to 4:30 p.m. Call for details and reservation information. Donations accepted. Medicaid-Beneficiary Appointment Transportation LogistiCare (Deaf or hearing-impaired call ) For Medicaid beneficiaries living in Wayne, Oakland, Macomb counties who have no other way to get a ride to their medical appointment. Call at least two business days before you need a ride. If your doctor confirms that the appointment is urgent, such as when you are being released from the hospital, you may call for a ride right away. Have your Medicaid ID, and name/address/phone of your medical provider ready when calling. 6

Crittenton Home Care: Transportation/Medical Escort Services Private duty home care service offers transportation and medical escort to appointments and procedures/tests. Employees bonded, insured, background checked, and trained. Services Oakland, Macomb, and surrounding counties. Call for rates and scheduling. Home Instead Senior Care: Incidental Transportation Services Birmingham office: Clarkston office: Shelby Twp office: Private home care agency offers companion service which can include incident transportation for clients to and from medical appointments and tests. Three hour minimum charge, with 24 hour notice preferred. Can be one time only, or regularly scheduled service. Available 7 days a week. Services include helping the client get ready for the appointment, transporting and accompanying, taking notes if desired, returning home and staying with client until settled back at home. Call for prices. Someone Waiting, Inc Based in Sterling Heights, but services metro area. Specializes in waiting for patient during appointment or procedure. Licensed and insured. Will drive client to/from physician office, hospital, and other locations.
