Council on Aging Transportation COA currently contracts with 37 organizations to provide transportation services through various programs. There are currently 6 independent programs administered by COA which have transportation as a service option. For seniors who are unable to safely transport themselves and have limited options and support, Medical Transportation Service can help them get to a variety of medical appointments to maintain or improve their health. The appointments can include medical offices, physical/occupational/mental therapy services, and even dental services. Non-Medical Transportation provides assistance for seniors to destinations that are not medically related. The non-medical transportation service provided through Title III is used to provide trips to the grocery store, shopping, meals and access to community resources such as senior centers and adult day services. Non-Medical Transportation provided through PASSPORT is new and is available only to seniors who are enrolled in that program. PASSPORT services PASSPORT provides services such as home-delivered meals, personal care, home medical equipment and transportation to doctor's appointments, to name a few. PASSPORT clients also have a care manager who advocates for their needs, answers questions, and makes sure they receive the right services. Care managers have their clients' best interests at heart. Click here for more information. Who qualifies for PASSPORT? PASSPORT may be for you if you are: Age 60 or older Eligible for Medicaid (low-income) In need of skilled or intermediate care: hands-on help with dressing, bathing, toileting, grooming, eating or mobility.

How much does PASSPORT cost? Your care is free of charge, or set at an affordable level depending upon your income and assets. Elderly Service Program Services ESP provides services such as Meals on Wheels, personal care (bathing and grooming), home-making and emergency response devices, to name a few. ESP clients also have a care manager who advocates for their needs, answers questions, and makes sure they receive the right services. Care managers have their clients' best interests at heart. Who qualifies for Hamilton County ESP? You may qualify for ESP if you are: A Hamilton County resident Age 65 or older Impaired in everyday activities such as bathing, driving and preparing meals Who qualifies for Warren County ESP? You may qualify for ESP if you are: A Warren County resident Age 60 or older Impaired in everyday activities such as bathing, driving and preparing meals How much does ESP cost? Care through the Elderly Services Program is free of charge or set at an affordable level, depending on income and assets. Who qualifies for Clermont County ESP? Clermont Senior Services, Inc., a private, nonprofit 501(c)3 organization, is dedicated to serving older adults, age 60 and over, in Clermont County. Clermont Senior Services provides and coordinates in-home and community-based services, which enable older adults to live independently for as long as possible. Available services include: Adult Day Care Care Management Caregiver Support Home Care Home Repair Housing

Meals-on-Wheels Respite Care Transportation For more information about programs and services available through Clermont Senior Services, visit the agency's Web site, www.clermontseniors.com Transportation Clermont ESP Services Transportation is provided to medical appointments, adult day care, lifelong learning centers, and appointments for personal business and grocery shopping. Other personal appointments are accommodated as time allows. All vehicles are wheelchair accessible. Ability to provide service may be limited based on individual s need for assistance. Attendant may accompany the older person. Please call for appointments at least three days ahead. Service Area Clermont County all services. Brown and Hamilton Counties medical only. Eligibility: Age 60 and over. Resident of Clermont County. Request service at least 3 days ahead. Cost (suggested donation): Appointments in Clermont County: $2.00 roundtrip. Appointments in Hamilton County: $4.00 roundtrip Title III Services Eligibility Typically, any Ohioan age 60 or older, or any Ohioan who serves as a caregiver for an older Ohioan, is eligible for OAA services, provided he or she meets the eligibility requirements for specific programs. Due to limited funding, priority is often given to low-income, minority and rural elders

Services A variety of home and community-based services are provided by the Older Americans Act and related state and local funds. COA works with other agencies (home care agencies, transportation providers, senior centers, meal providers, etc.) to offer the following services to eligible older adults and caregivers in southwestern Ohio: (includes a range of services, but this paper just covers transportation options) Transportation: moves eligible individuals to and from community resources, services and activities through the use of a driver and motorized vehicle Other Local Transportation Options A comprehensive listing of transportation options for older adults is available in the Help4Seniors Resource Directory. Some of those (available by county) are: Clermont County Clermont Senior Services Transportation is provided to medical appointments, adult day care, lifelong learning centers, and appointments for personal business and grocery shopping. Other personal appointments are accommodated as time allows. Service is available throughout Clermont County and for medical appointments only in Hamilton and Brown counties. Service is available to Clermont County residents age 60 and older. All vehicles are wheelchair accessible. Please call for appointments at least three days ahead. CTC offers three fixed routes in addition to its Dial-A-Ride services. CTC operates 6 a.m.-6 p.m., Monday- Saturday. ADA Paratransit Service is available to CTC customers who have difficulty getting from their home to a bus stop on a Fixed, Shuttle, or Express route bus stop. Application required. Hamilton County Senior Transportation Options Sycamore Senior Center Sycamore Senior Center offers transportation Monday through Friday to the center as well as to medical appointments, chemotherapy and radiation treatments, therapy appointments and grocery shopping. A wheelchair accessible van is also available. Medical transportation appointments must be made at least two weeks in advance. Serves Arlington Heights, Blue Ash, Camp Dennison, Deer Park, Dillonvale, Evendale, Indian Hill, Kennedy Heights, Loveland, Madeira, Montgomery, Pleasant Ridge, Reading, Rossmoyne, Sharonville, Silverton, Sycamore Township, and Symmes Township. Greater Cincinnati area. By Phone: (513) 661-2777 Cincinnati Area Senior Services (CASS) For adults age 60 and older, transportation from various older adult communities to CASS senior centers, and from those centers to shopping and regularly-scheduled recreational outings. Weekly shopping and recreational trips also available. Serves Greater Cincinnati area. By Phone: (513) 721-0375 itngreatercincinnati Independent Transportation Network Trained volunteers help older adults remain independent by driving them to appointments, shopping or even out for lunch. A membership program with a pre-paid Personal Transportation Account, itn is a door-through-door service which includes help with steps and packages, heavy doors and walkers. Rides are available 24/7 for any purpose for people 60 years and older and visually impaired adults. Private automobiles and trained drivers. Warren County Public Transportation Options Warren County Transit Services (WCTS) Transportation is provided to advance scheduled destinations within Warren County on weekdays (Monday - Friday) from 6 a.m. to 6 p.m. Trips for medical reasons can be scheduled to some Middletown and Dayton areas. A limited amount of free tickets are available to residents who are 60 or older. Non-emergency transportation (NET) is a program that provides transportation for Medicaid-eligible (lowincome) recipients to health care providers who bill to Medicaid. The customer must be eligible for Medicaid on the date transportation is provided. Transportation is within Warren County and into all bordering counties. Operates from 8:00 a.m. to 4:30 p.m., Monday - Friday. Reservations required. Medical and non-medical transportation available.
