Welcome Center. It s time again to Take the Wheel as you head out on your benefits enrollment journey. BorgWarner s annual open enrollment is an excellent opportunity to think through your upcoming healthcare needs, review your current benefit coverage and decide whether you should make any changes to your benefit coverage elections to better support your needs and budget. At BorgWarner, we believe that a healthy workforce is a more productive workforce, and healthy employees are happier employees. Our benefits provide you and your family with access to programs and services that promote healthy lifestyles and provide preventive care such as screenings, physicals, immunizations, and well-child care. As an employee, you are also eligible to use the services of our on-site Wellness Clinic located in Plant 2. If you haven t been in yet this year, call 266-5018 to make an appointment with the nurse for a wellness visit or to find out more about programs and services available to all employees. NOVEMBER 15, 2014 All employees must log in and make elections prior to the deadline, even if you want to keep the same coverage or if you wish to waive coverage. 2015 Enrollment Points of Interest Navigating the Road Ahead Online Enrollment for all Employees Once again this year, all employees will make their benefit elections ONLINE through Workday. The online system improves efficiency and enhances the security and privacy of employee information. The online environment also supports our corporate objective to be more environmentally-friendly and green. Remember, all employees must log in and elect benefits each year before the open enrollment deadline. If you need help, please come to one of our Workday help sessions or see one of your Human Resources Representatives. 2015 Employee Contributions for Health Plans The employee rates for the CIGNA plan are actually lower for 2015! The employee rates for the Teamsters Blue Shield PPO stayed the same, and the CIGNA Choice Health Fund will remain free as long as you elect to complete both activities for the premium surcharge discounts. For more information, see the chart on page 3 of this booklet. New! Prescription Annual Out of Pocket Maximum (CIGNA plans only) New for 2015 Prescription Drug program for both CIGNA health plans will now include an annual Out of Pocket Maximum (OOPM) of $3600 single/$7200 family for prescriptions filled at an in-network pharmacy. Once you meet this limit, the Plan pays 100% of any eligible prescription drug costs for the remainder of the year. (There is still no limit/maximum for prescriptions filled at an out-of-network pharmacy.) CHOOSE WISELY The plan elections you make during open enrollment are effective beginning 1/1/15 until 12/31/15. Your next opportunity to make changes will be during annual enrollment next year, unless you experience a qualifying event/ family status change. A qualifying event, such as a birth or marriage, allows you to make changes within 30 days of the event. See your HR Benefits Representative for more information.

2015 Healthcare Option Highlights Once again in 2015, you have three (3) medical plans to choose from, each with different coverage level options. All offer coverage for in-network care, preventive care and prescription drugs. The main differences between the plans are the payroll contributions, premium surcharges/discounts offered, deductibles, coinsurance and whether they offer a Health Reimbursement Account. There is a slight decrease in employee contribution amounts for the CIGNA PPO plan for 2015. The CIGNA Choice Health Fund and Teamsters BlueShield plan employee contribution amounts remain unchanged for 2015. Rewarding Healthy Behaviors Wellness Incentive Programs (CIGNA Healthcare Medical Plans Only does not apply to Teamsters Blue Shield Plan) Discount Options Available to Avoid Monthly Surcharges: To reward employees for taking positive steps toward a healthier lifestyle, BorgWarner will offer two incentives that offset monthly premium surcharges for employees enrolled in the CIGNA healthcare medical plans. Participation is voluntary the employee decides whether to complete both, one or neither of the discount incentives. Each activity earns you $50 monthly, for a combined discount of $100, off your monthly medical premium surcharges when you are enrolled in either of the BorgWarner Health Plans. The discount applies only to the employee. If the employee does not complete the activity before the deadline to qualify for the discount, the $50 monthly surcharge will apply. Discount #1: CIGNA Health Risk Questionnaire (HRQ): If you elect to take the HRQ, you must complete the CIGNA HRQ online at www.mycigna.com by November 30 th 2014 to avoid paying the $50 surcharge beginning in January 2015. If you do not complete the CIGNA HRQ online by November 30 th, you will be charged a $50 monthly surcharge beginning with your first paycheck in January of 2015. CIGNA will send a notification report to BorgWarner indicating employees who have completed the HRQ so that HRQ surcharges can be properly administered. Individual HRQ results are NOT shared with BorgWarner. Discount #2: Tobacco Cessation: Tobacco users who elect the Tobacco Cessation Pledge must complete a BorgWarner approved tobacco cessation program by February 28, 2015. Programs can be completed in one of two ways: 1. By phone through CIGNA Quit today. Call 1-800-237-2904 to talk to a CIGNA health coach and set up a program with goals customized to your needs. 2. Attend the on-site Tobacco Cessation Support Group held weekly in TC. Call 266-5018 to find out more information and to sign up. You will not be charged the Tobacco Cessation Premium as long as you complete one of the programs by February 28, 2015. If you do not complete a program before the deadline, you will be charged the $50 surcharge beginning with the first payroll in April 2015. Contact the on-site Wellness Clinic today at 266-5018 We offer an on-site program for employees that will qualify for the Tobacco Cessation discount, as well as information on other local programs and on-going support. We can also do a biometric screening to give you current cholesterol and blood pressure data you can use to complete the HRQ, and we offer help completing the HRQ if you need it. 2015 Prescription Drug Plans When you elect to enroll in one of the healthcare plans, your plan will include Prescription Drug coverage. You are automatically enrolled in prescription plan coverage based on the healthcare plan you choose. Plan coverage and copay structure will remain the same for 2015, with the exception of the new Out-of-Pocket Maximum (OOPM) for CIGNA plans. If you are enrolled in a CIGNA healthcare plan, your CIGNA medical card will also serve as your prescription card. Under the CIGNA prescription drug program, effective 1/1/15 there is a limit (called the Out-of-Pocket maximum) to the amount you would have to pay toward prescription drug costs if they are filled at an in-network pharmacy. The limit for 2015 is $3600 single/$7,200 family, Once you meet this limit, the Plan pays 100% of any eligible in-network prescription drug costs for the remainder of the year. This limit does not apply to any prescription filled at an out-of-network pharmacy. Remember, pharmacy costs do not apply to your medical deductible. CIGNA Retail Network Pharmacy Use a retail pharmacy any time you need a prescription filled right away. Simply show your CIGNA medical ID card at a network pharmacy, and pay your portion of the cost. The pharmacist will file your claim for you. CIGNA Tel-Drug Mail-order Service As an alternative to refilling your prescriptions at a retail pharmacy location, you have the option of filling prescriptions through a convenient and cost-effective mail-order program offered by CIGNA Tel- Drug. For added convenience, the prescription is delivered right to your home. For more information on the prescription drug coverage, including a drug cost comparison tool, visit www.mycigna.com ******************************************************************************************************************** If you are enrolled in the Teamsters BlueShield healthcare plan, you will automatically be enrolled in OptumRx for your prescription needs. Optum Rx Retail Network Pharmacy Use a retail pharmacy any time you need a prescription filled right away. Simply show your OptumRx ID card at a network pharmacy, and pay your portion of the cost (do not use your BlueShield ID card for pharmacy services). The pharmacist will file your claim for you. Most national pharmbowbowacies are in the OptumRx network, including Rite-Aid, Walgreen s, and CVS. Optum Rx Mail-order Service If you use a maintenance drug for high blood pressure, for example you are required to use the convenient mailorder service. Order up to a 90-day supply for one co-pay, and your drugs are delivered directly to your home for free. You can order refills over the phone or online. For Teamsters BlueShield PPO plan participants, the mail service pharmacy is handled through OptumRx by calling 1-800-788-4863. Customer Service representatives are available 24 hours a day, 7 days a week. Mandatory Mail Order Service The Teamster s BlueShield Prescription Plan through Optum Rx requires mandatory mail order service for all maintenance prescriptions You can also always call the Teamsters Health and Welfare Office at 1-518-437-9837 with questions.

A Recap of Your Other Benefits for 2015 The coverage levels and contribution rates for the plans listed below remain the same for 2015. Dental/Vision BorgWarner offers a combined dental and vision plan, including options for employee and dependent coverage. Your dental benefits cover routine exams, fluoride treatments, X-rays, fillings, extractions, crowns, bridges and braces. You can visit any dentist you choose. Preventive care is covered at 100% with no deductible. Coverage for other services begins after you meet your annual deductible. Your vision benefits provide coverage for one exam per calendar year, and either one pair of eyeglasses (frames and lenses) or contacts once every 24 months. Like your medical benefits, coverage is higher when you visit an in-network provider. To locate a CIGNA Vision Network provider, visit www.mycigna.com or call CIGNA Vision Member Services at 1-877-478-7557. Flexible Spending Accounts (FSA) Flexible Spending Accounts (FSAs) are a great way to help you budget for expenses. There are two types of accounts: Health Care FSA and Dependent Care FSA. Keep in mind, the two FSAs are separate, so you may enroll in one, both or none of them. You decide how much to contribute to an FSA each year within the limits set by the IRS. Your contributions are deducted from your paychecks throughout the year before taxes are withheld. This makes savings convenient. Plus it reduces your taxable income, saving you on income taxes. When you have an eligible expense during the year, file a claim to get reimbursed from your FSA. To find out what qualifies as an eligible expense, visit www.irs.gov/publications/p502. Long Term Disability Insurance Optional 70% Buy-up Coverage All BorgWarner employees are automatically enrolled in basic Long Term Disability Insurance (LTD60), administered by CIGNA. Coverage begins after you ve been disabled for more than six months and meet the insurer s definition of disabled. Basic coverage pays 60% of your current salary up to $5,000 per month until you reach normal retirement age. In addition, optional coverage is available to employees who wish to buy-up to 70% disability coverage (LTD70). If elected, LTD70 pays 70% of your current salary (including a combination of wages and an average of 3 years of employee incentive plan payouts as calculated on Jan 1 of each year for the previous year) up to $5,000 per month until you reach normal retirement age. The employee pays a monthly premium for this plan if elected. Long-term disability benefits will be reduced by the amount of other income you may receive such as Social Security or workers compensation. Basic Life Insurance and Optional Life Insurance Plans Your Basic Life Insurance benefit, administered by MetLife, offers financial protection for your survivors if you die while you re an active employee of the company. BorgWarner pays the full cost of this benefit. You can also purchase additional life insurance for you, your spouse and your dependent children through MetLife. There is no open enrollment period for life insurance benefits. Contact MetLife at any time to apply for additional coverage or make changes to your current coverage.

Key Dates to Remember: This brochure is designed to highlight the online enrollment process and to help make your enrollment trip quicker and easier. This is your once-a-year opportunity to ensure that you and your family have the benefit coverage you need. All employees must complete 2015 enrollment online through Workday by November 15, 2014, even if you want to keep the same coverage. How to Enroll: 1. Don t forget to make an election for each coverage you want (medical, dental/vision, etc). If you want to waive health insurance, elect the CIGNA no medical EAP only coverage for employee. Review and confirm your dependent information. If you are adding a new dependent who is not currently covered under a BorgWarner plan, you will need to provide documentation to HR (Marriage License, Birth Certificate, etc) in order to complete the enrollment process.
