Our Commitment To Excellence A Message from our Leadership It is our privilege to serve as the Executive Leadership of your Pinellas County CA ospitals and to have this opportunity to share with you the partnerships our hospitals provide to the community. Your CA Pinellas County hospitals, including Edward White ospital, Largo Medical Center, Northside ospital and St. Petersburg General ospital have a significant impact on Pinellas County, providing more than 3,680 people with employment and more than $15.6 million in taxes. We are proud to have more than a thousand of the area s most respected physicians on our medical staffs. We treated more than 117,000 patients in our emergency departments, where outstanding efforts by our teams continue to improve quality and operations. Our hospitals average ER wait times are posted live on our websites. We also invite residents to text message ER to 23000 and respond with a local zip code to receive the CA Pinellas County hospital average emergency room wait times. While the ER volumes are growing, so is the number of patients without adequate health insurance or the financial resources to pay for this care. We continue to treat a large percentage of uninsured patients in the communities we serve. Providing high quality care remains at the core of our mission, and we were honored to have Edward White ospital and Largo Medical Center named in 2011 as 2010 Top Performers for Key Quality Measures by The Joint Commission. They joined an elite group of 405 hospitals nationwide which are industry leaders in using evidencebased care processes that are closely linked to positive patient outcomes. Northside ospital was the first West Florida Division facility to earn state designation as a Comprehensive Stroke Center. The hospital provides the latest advanced technology and neuroradiology services for stroke patients. St. Petersburg General ospital became an accredited community cancer program by the American College of Surgeons Commission on Cancer (ACOS CoC). Good patient care requires having the right information at the right time and we continue to support our physicians by providing them secure access to electronic health information, fetal monitors and heart rhythms on their smart phones and other hand held devises. We are investing heavily in information technology to provide quicker and easier access to health information. At the same time, we are ensuring that we are in compliance with Meaningful Use (MU) criteria as we add new features to our Electronic ealth Record (ER) system, hcare, and work with other health care systems and providers in the communities we serve. Our hospitals have done an outstanding job focusing on quality and operations, with all CA Pinellas County hospitals receiving many national and state clinical outcome and achievement recognitions. Indigent Patient Education CA West Florida ospitals of Pinellas County are committed to helping uninsured patients qualify for local and state health benefit programs. Each hospital offers onsite benefit education and Medicaid enrollment assistance. Going Green As an active member of Practice Greenhealth, an industry leader in sustainable, ecofriendly practices, CA hospitals are involved with green initiatives both at the facility level and in many cases in the greater community. It is part of our commitment to have a comprehensive environmental strategy in concert with our mission of the care and improvement of human life. Social Media CA Pinellas County ospitals now have a social media presence to engage our patients and the communities we serve as fully as possible. Volunteering Countless hours of volunteering were performed by members of the CA Pinellas County ospital family last year. From providing medical care on mission trips to serving as the voice for children through programs such as Guardian Ad Litem and Autism advocacy, our staff, volunteers and physicians give back to our communities, both in and outside of our facilities. urricane Preparedness CA ospitals of Pinellas County have a comprehensive hurricane preparedness plan to ensure that our patients and the staff who care for them are safe during a storm. Our hospitals have arrangements in place with vendors from around the state and nation to provide support, from on-site security if needed to medical supplies, additional generators, food, fuel, emergency transportation, building supplies and construction crews who can be dispatched immediately to make needed repairs to keep our hospitals operational. Several days worth of food and supplies all arrive at each facility well in advance of a storm, to ensure the hospital is self-sustaining. Transfer processes and plans are in place with our network of hospitals from around the country, should patients need to be relocated. 4 Giving Back To Our Community

All of our hospitals are Joint Commission Accredited and Accredited Chest Pain Centers and Certified Advanced Primary Stroke Centers. Our Commitment To Excellence Quality & Patient Safety Putting Patients First is the foundation of our culture and passion for quality health care. Many of the initiatives we implement serve as national models for the industry today. The CA West Florida hospitals have achieved industry leading performance on public Clinical Measures, such as the CMS Core Measures for Congestive eart Failure, Pneumonia, Acute Myocardial Infarction, and Surgical Care Improvement Project. Our hospitals are thoroughly engaged in programs and initiatives which promote and optimize Patient Satisfaction, Clinical Efficiency programs and Quality outcomes. AIM for Zero AIM for Zero is an example of CA s commitment to providing excellence in healthcare through the consistent application of evidence-based interventions in the elimination of Central Line-Associated Blood Stream Infections (CLA-BSI). CA s goal for this initiative is zero, as we know from the medical literature that these infections are increasingly proven to be avoidable. The letter A in AIM stands for antimicrobial stewardship which embraces the effective and efficient utilization of antibiotics. Overuse of broad spectrum antibiotics is not only needlessly expensive, but it breeds resistant infections. The letter I refers to a checklist of interventions that has been demonstrated as preventing CLA-BSI at the point of Insertion. M stands for vigilance in maintaining and removing central lines at the earliest appropriate moment. Culture is critical to high performance health care. AIM for Zero includes behavioral expectations that encourage any team member to speak up to assure that the required interventions are followed. The personal behavior choices caregivers make each day affect their ability to protect patients from preventable, and potentially life-threatening, infections. Through exemplary leadership and support, CA is committed to offering the premier performance patients expect and deserve. Broselow System for Acute Pediatric Administration of Drugs in the ER All of our hospital emergency departments have installed the Broselow System to ensure the proper dosing and administration of drugs to children. Pediatric dosing can be difficult, especially in emergency situations, and the Broselow System allows medical professionals to instantly access proper dosing, improve the speed and accuracy in acute preparation and administration of drugs, reduce medical errors and standardizes pediatric dosing so that we can provide our young patients with high quality emergency care, safely. Electronic Bar Coding Medication System (BCMA) CA led a national effort to reduce medication errors by placing this system in all of our hospitals several years ago. Bar codes on medications are scanned at the patient s bedside, providing our clinical staff with an effective tool to ensure that the right patient receives the right medication at the right time through the right route. Electronic Bar Coding Transfusion System (BCTA) We expanded bar coding for transfusion products in all of our hospitals as a result of the medication safety successes. Prior to administering any blood product, the unit is scanned to ensure the patient is receiving the right transfusion product at the right time through the right route. hcare Electronic ealth Record (ER) The West Florida Division has invested heavily in its Electronic ealth Record (ER) initiative for many years. With ER, access to patient health information is simplified and patient safety and quality improved. Our hospitals have brought numerous clinical components for the ER on-line through major investments in technology. As of March 2010, physicians are able to document and enter patient orders on-line and remotely from their offices. This streamlines the ordering process and ensures accurate and timely approach to patient care. As more physician practices use Electronic Medical Record (EMR) systems, there is an increased need to integrate data from a variety of clinical systems, including hospital systems. Linking physician practices to our hospital databases is being rolled out through the hcare ub interface. This allows physician offices real-time access to patient clinical data from the hospital. Effective clinical data exchange can provide necessary information to physicians to make timely decisions and reduce duplicate testing and unnecessary hospitalizations. 6 Quality & Patient Safety

CA West Florida Pinellas County Medical Education We re also bringing future physicians to our communities through medical training programs. The Graduate Medical Education program of Largo Medical Center is the third largest osteopathic teaching program in the South and has been designated as a Statutory Teaching ospital by the Agency for ealth Care Administration (ACA). There are twelve residency programs (a thirteenth will begin in 2012) and more than 118 residents in the program this current academic year at Largo Medical Center. Both St. Petersburg General ospital and Northside ospital also provide Traditional Rotating Internship, with St. Petersburg General offering a Family Practice Residency and Northside offering an Internal Medicine Residency and Cardiology Fellowship. Medical Research Sarah Cannon Research Institute (SCRI) of Nashville, one of the largest global clinical research programs, has signed an agreement with The Tampa Bay eart Institute at Northside ospital in St. Petersburg, FL, as a cardiovascular clinical research strategic site. SCRI s rapidly expanding cardiovascular research network enables hospitals and physician practices to develop their research programs and provides the opportunity for patients to consider participation in clinical trials in their own community. Our hospitals also work with physician partners who are engaged in research and clinical trial initiatives in multiple specialty areas. MRSA ABC s A new strain of antibiotic-resistant staph, or MRSA, is spread easily and is much more difficult to treat than ordinary staph bacteria. The growing prevalence of MRSA in our communities and in our hospitals is creating new challenges for the healthcare community. In response, CA hospitals across the nation, under the leadership of Dr. Jonathan B. Perlin, CA s chief medical officer, have implemented more aggressive infection prevention protocols to tackle this issue. CA facilities have also been provided with a proactive education campaign for patients, visitors and staff to prevent infections from occurring and spreading. We see MRSA as a national challenge affecting hospitals and communities alike, and would like to share our materials as a way of contributing to the education and more importantly the eradication of MRSA. Our Commitment To Excellence Nation s First Infant Screening Program We were the first U.S. healthcare system to voluntarily perform a simple blood test to screen newborns for elevated levels of bilirubin that triggers jaundice. By identifying newborns at risk for Kernicterus, which causes permanent brain damage and physical impairment, we can treat them immediately and prevent this serious health threat. Flu Season Preparation Influenza remains the number one cause of vaccine-preventable death in the United States. Infected health workers can infect others and we want to eliminate that risk. Any death from a disease that is vaccine-preventable is one too many. CA employees have received national recognition for their commitment to patient safety for their efforts in supporting CA s flu immunization policy. This flu season our goal is the same a commitment to 100 percent patient safety. The way we will achieve this goal is through vaccination of health workers with direct patient contact or wearing a mask when vaccine is not possible. Responsible Imaging CA West Florida developed and implemented an innovative program called Responsible Imaging to enhance medical imaging safety through state-of-the-art monitoring, use of best practices and extensive education of staff, physicians and patients. The program was developed and piloted at Largo Medical Center and Regional Medical Center Bayonet Point with LANDAUER, a global leader- in radiation science and services. With increasing attention on the long-term cumulative effect of radiation exposure, we are excited to be a leader in responsible medical imaging practices to ensure the lowest dose of exposure for our patients. Vitals Now The West Florida Division was selected to pilot a new technology called Vitals Now this enables the automatic capture of patient vitals right from the bedside into our main Clinical System. This enhances clinical decision making for our care givers as patient vitals are now readily available in the patient s record. 8 Quality & Patient Safety / Specialized Care

Offering one of only two interventional neuroradiology programs in Pinellas County, Northside ospital is now able to extend the treatment window for stroke patients from three hours to eight hours. Our Commitment To Excellence Behavioral ealth Largo Medical Center offers behavioral health services where patients can receive the specialized care they need in a dedicated setting, while having full access to the medical services of a hospital. They provide a 33-bed Inpatient Unit, a 14-bed Voluntary Inpatient Unit, a 24-our Referral Line as well as both Inpatient and Outpatient Electroconvulsive Therapy (ECT). Services have recently expanded to include a 7-bed behavioral health emergency room to meet the needs of patients who experiencing a mental health crisis. St. Petersburg General has expanded their services by opening a Medical Stabilization Service for Substance Abuse. Comprehensive Cancer Care Cancer is the second leading cause of death in this country, exceeded only by heart disease. Our hospitals have committed to quality care by striving to meet and exceed the many standards required for the American College of Surgeons Commission on Cancer (ACoS CoC) Accreditation.
