The OSF Preferred Skilled Nursing Network is a select group of skilled nursing facilities including nursing homes that meet stringent quality and safety standards set forth by the Centers for Medicaid & Medicare (CMS) and OSF HealthCare. These facilities undergo bi-annual reviews to ensure the highest level of care is maintained. These standards include: H 4 or 5 Star Overall Rating from CMS H 3, 4 or 5 Star Quality Rating from CMS H RNs in-house 24/7 H Ability to start an IV 24/7 H Able to admit a patient within a two-hour window By choosing an OSF preferred facility, you and your family will have the reassurance of knowing that an OSF physician will continue coordinating your care, providing a seamless care transition that can be helpful during times of stress, injury or sickness.
