1 WHAT IS LONG- TERM CARE? Long- term care is a range of services and supports you may need to meet your health or personal needs over a long period of time. Most long- term care is not medical care, but rather assistance with the basic personal tasks of everyday life, sometimes called Activities of Daily Living, such as: Bathing Dressing Using the toilet Transferring (to or from bed or chair) Caring for incontinence Eating WHAT ARE NURSING HOMES? Nursing homes, also called skilled nursing facilities (SNF), provide a range of services, including nursing care, 24- hour supervision, and assistance with Activities of Daily Living. They also offer rehabilitation services such as physical, occupational, and speech therapy. You may only need nursing home services for a short period of time for recovery or rehabilitation after a serious illness or operation. Or you may need longer stays because of chronic physical health problems or cognitive problems (e.g., memory loss) that make it necessary for you to have 24- hour care or supervision. WHO NEEDS LONG- TERM CARE? You need long- term care when you are not able to complete personal care or other daily activities on your own. This is most often the result of a chronic illness or disability. In some cases, the illness or disability may include memory loss, confusion, or disorientation, sometimes called cognitive impairment. 2 PAYING FOR CARE: MEDICARE: Under certain limited conditions, Medicare will pay some nursing home costs for Medicare beneficiaries who require skilled nursing or rehabilitation services. To be covered, you must receive the services from a Medicare certified skilled nursing home after a qualifying hospital minimum stay of 3 days for a related illness or injury. You will need to verify certification with each facility. MEDICAID: A State and Federal program that will pay most nursing home costs for people with limited income and assets. Medicaid will pay only for nursing home care provided in a facility certified by the government to provide service to Medicaid recipients. You will need to verify certification with each facility. Please check Medicaid eligibility with the Nebraska Medicaid office by calling (Omaha) or online at About half of all nursing home residents pay nursing home costs out of their own savings. After these savings and other resources are spent, many people who stay in nursing homes for long periods eventually become eligible for Medicaid. MANAGED CARE: This plan will not help pay for care unless the nursing home has a contract with the plan. If the home is approved by your plan, learn if the plan also monitors the home for quality of nursing care. MEDICARE SUPPLEMENTAL INSURANCE: This is private insurance. It's often called Medigap because it helps pay for gaps in Medicare coverage such as deductibles and co- insurances. Most Medigap plans will help pay for skilled nursing care, but only when that care is covered by Medicare. Some people use employer group health plans or long- term care insurance to help cover nursing home costs. LONG- TERM CARE INSURANCE: This is a private policy. The benefits and costs of these plans vary widely. For more information on these plans, contact the National Association of Insurance Commissioners (NAIC). It represents state health insurance regulators and has a free publication called "A Shopper's Guide to Long- Term Care Insurance." COUNSELING AND ASSISTANCE: SHIIP (Senior Health Insurance Information Program) has counselors who might be able to answer your questions about how to pay for nursing home care, the coverage you may already have, or whether there are any government programs that will help with your expenses. 3 QUESTIONS TO ASK WHEN TOURING A NURSING HOME Touring nursing home facilities in person is an essential part of finding the right facility for your loved one. Many people recommend making multiple visits during your decision making process. For your first visit, make an appointment with the admissions director to tour on a weekday. Late morning and midday hours are usually the best times to get a feeling for the day- to- day operations of a home. Watch out for homes with bad smells, residents who are strapped into wheelchairs and staff who seem to ignore the residents. One of the most important things you can do is to talk to the residents and ask how they're doing. Above all, trust your instincts; first impressions matter. WHAT TO ASK: As you visit the nursing home, be sure to ask the admissions director a lot of questions. Don't worry about taking up their time. Verify any information you were told over the phone. Is the facility a non- profit or for- profit? What types of care do they offer? Is the facility certified by Medicaid and Medicare? What is the average length of stay? What are the qualifications of the staff in the therapy department? Ask what makes this nursing home different from others. Does the facility include family members in resident care plans? Ask for recent state inspection survey results. The report should be dated within the last 9-15 months. Compare the number of deficiencies cited to the state average. If the facility has received a citation in a particular service area, be sure to ask questions about this area. Beware of choosing a facility with a very high number of deficiencies compared to other facilities in the area and the state average.
