Search for a patient Click Main Menu>Search for a patient>patient Search You can search by any of the following: CHI number Surname DOB Postcode Diabetes Type Establishing a Contact If you are logged in to a secondary care domain rather than a GP practice then you will have to establish a contact before you can enter any data. Users in primary care are not required to do so. (A contact is the same as an Episode in the old SCI-DC Clincial system.) You can create a new contact for today s date simply by going to the Contact dropdown menu and clicking on the appropriate type by type e.g. Face to face, over the phone etc. 2
If you want to use a contact that has already been created on the system (e.g to amend data you entered previously) you can do so by going to Manage Contacts under the Contact dropdown menu. From there click on the icon on the far right, next to the contact you want to use 3
DSN Form. To record data go to the Forms dropdown menu and open the DSN form. Keep the pre-populated data or start with a blank form. Complete the DSN form and SAVE. 4

Patient Structured Education To add Structured Education advice, click on the left-hand tree at Patient Support >> Patient Education History >> Add A New Entry and complete the details. The completed Patient Education History will look like this: 6
Advice History Add a new Entry and complete the form as shown below and SAVE Travel Letter To print a Travel Letter go to Travel Letters in the Patient Support section on the navigation tree and choose the appropriate Travel Letter (insulin, tablets or GLP-1). The letter will open as a PDF document in a new window and can be printed from there. Navigation Tree PDF Document 7
Print a Letter to GP SPECIALIST CLINIC SUPPORT >> CLINIC LETTERS and choose letter type GP Letter or Paediatrics Letter IMPORT CLINICAL COMMENTS will add the free text you entered in the DSN form section 8
Generating the Letter Once you have ticked the compliance boxes, click on Generate Letter To view the Letter before printing, click on Click to View PDF 9
The Clinic Letter then opens in a new window. From there you can either save it to your hard drive for emailing or print it. Adding a New Patient In the event of a new patient, ENROL The Patient as below In the Main Menu, click on Patient List Management >> Enrol Patient (into Clinic) Enter the CHI (or other criteria available to you) and a search will bring up the patient if they are already on the national CHI database (which is most likely). 10
Then click on enrol on the right hand side of the search result to add the patient to your patient list on SCI-Diabetes. Patient Did Not Attend (DNA) When creating the Contact instead of Face to face or Telephone Contact etc, click on Manage Contacts Click on Add a New Entry 11
Fill in the RED BORDERED boxes as if the patient was attending, then add DID NOT ATTEND (DNA)/COULD NOT WAIT etc from the drop-down menu Check the Manage Contacts that the Contact has been recorded as Did Not Attend 12
Then go to Main Menu>>Search for a Patient>>>and start the next new patient entry 13
