Packet ELIGIBILITY Applicant must reside in Brevard County. Applicant must have completed at least ONE SEMESTER of college. Applicant must be committed to pursuing a college degree from an accredited college or university whose geographic location is defined in the scholarship description. Applicant should have a demonstrated financial need. Applicant must submit all required application documents by the deadline. Applicants should demonstrate a desire to advance their community through volunteerism. Scholarship awards will be up to 2 semesters, starting January 2015, with the second semester contingent on maintaining a 3.0 average. APPLICATION INFORMATION DEADLINE: Tuesday, November 11, 2014. Extensions will not be granted. Incomplete or late application materials will result in ineligibility. Please read the eligibility criteria for each scholarship in the packet carefully to determine whether you are eligible to apply. Applicants may not be eligible for every scholarship offered through the Community Foundation for Brevard. It is understood that scholarship checks will be forwarded directly to the institution of learning in which the recipient is enrolled. Scholarship packets must contain the following (please check the box indicating you have completed each item): APPLICATION Complete a separate application for each scholarship for which you are applying. Applications must be completed by the student only. RESUME UNOFFICIAL TRANSCRIPT(S) (one copy per application) FAFSA EFC # (Expected Family Contribution Number) or copy of current years Tax Return LETTERS OF RECOMMENDATION (3 as indicated in each scholarship) ESSAY TOPIC (Typed, single spaced, 12-pt. font). Please note: Essay must be completed by the applicant, in the applicant s own words. Award amounts are determined annually. The Community Foundation for Brevard cannot make any guarantees about the amount to be awarded or the number of awards to be given. Determination of which scholarship to be awarded will be based on the information provided on the application and at the sole discretion of the selection committee. Application must be submitted by Tuesday, November 11, 2014. In accordance with the Community Foundation for Brevard scholarship policy, no scholarship shall be awarded to any member of the Community Foundation's Board of Directors, any substantial contributor to the Community Foundation, any employee of the Community Foundation, or any other disqualified person as defined in IRC 4946(a) with respect to the Community Foundation, or, with respect to scholarships from a particular Fund, any Donor advisor or substantial contributor to such Fund or any member of a selection committee to such Fund, or for a purpose that is inconsistent with the purposes described in IRC 170(c)(2)(B).

Norma S. Annabal Scholarship A Fund of the Community Foundation for Brevard The Community Foundation for Brevard Scholarship awards are made possible by revenue generated from the Norma S. Annabal Scholarship Fund and will be awarded to college attendees who reside in Brevard County and attend Eastern Florida State College, Florida Institute of Technology, the University of Central Florida, or Rollins College. Upon completion of the first year, scholarship recipients may apply for a second year scholarship. Both the amount and number of scholarships will be recommended by a review committee with final approval by the Foundation s Board of Directors. If you are applying for the Norma S. Annabal Scholarship, please check off the items to ensure eligibility. This scholarship may be used only at Eastern Florida State College, Florida Institute of Technology, University of Central Florida, or Rollins College. Scholarship is to be used only for tuition, books, and lab fees. The applicant must have completed at least one semester of college. The applicant must demonstrate a need for financial assistance. Eligibility for this scholarship is based upon residency and preference will be given for those who attended a Brevard County high school or who have lived in Brevard for more than a year. The applicant must attach three letters of recommendation. Recommendations may not be submitted by family members. It is understood that all scholarship checks will be paid directly to the institution of learning in which the recipient is enrolled. Required Essay: Typed, single spaced, 12-pt. font. Ella Josephine Goodman Horn Fund Scholarship A Fund of the Community Foundation for Brevard The Community Foundation for Brevard Scholarship awards are made possible by revenue generated from the Ella Josephine Goodman Horn Fund and will be awarded to college attendees who reside in Brevard County and attend Eastern Florida State College, Florida Institute of Technology, the University of Central Florida, or Rollins College. Upon completion of the first year, scholarship recipients may apply for a second year scholarship. Both the amount and number of scholarships will be recommended by a review committee with final approval by the Foundation s Board of Directors. Upon completion of the first year, scholarship recipients may apply for a second year scholarship. Both the amount and number of scholarships will be recommended by a review committee with final approval by the Foundation s Board of Directors. If you are applying The Pablo Enriquez Memorial Scholarship, please check off the items to ensure eligibility. This scholarship may be used at any accredited college or university. Scholarship is to be used for tuition, books, lab fees and/or campus based room and board. The applicant must have completed at least ONE year of college with a GPA equivalent average of B or higher. The applicant must demonstrate a need for financial assistance. Eligibility for this scholarship is based upon residency and preference will be given for those who attended a Brevard County high school for four years and graduated. The applicant must attach three letters of recommendation. Recommendations may not be submitted by family members. It is understood that all scholarship checks will be paid directly to the institution of learning in which the recipient is enrolled. Required Essay: Typed, single spaced, 12-pt. font. The Geraldine L. Cabic Charitable Fund Scholarship A Fund of the Community Foundation for Brevard This Community Foundation for Brevard Scholarship award is made possible by revenue generated from The Geraldine L. Cabic Charitable Fund and will be awarded to college attendees who reside in Brevard County and attend Eastern Florida State College. Upon completion of the first year, scholarship recipients may apply for a second year scholarship. Both the amount and number of scholarships will be recommended by a review committee with final approval by the Foundation s Board of Directors. If you are applying for the Geraldine L. Cabic Charitable Fund Scholarship, please check off the items to ensure eligibility. This scholarship may be used only at Eastern Florida State College towards the College of Nursing or the Health Sciences Institute. Scholarship is to be used only for tuition, books, lab fees, room and board if said room and board is paid directly to the school. The applicant must have completed at least one semester of college. The applicant must demonstrate a need for financial assistance. Eligibility for this scholarship is based upon residency and preference will be given for those who attended a Brevard County high school or who have lived in Brevard for more than a year.
