As a student enrolled at Baker College, I realize that my admission into the professional portion of the Radiologic Technology Program is contingent upon the completion of the specific prerequisite courses with a minimum GPA of 2.7 in each course. I recognize, acknowledge, and understand admission into the professional portion of the Radiologic Technology Program is contingent upon a competitive score on a numerical point system, and the availability of clinical affiliates. The admission criteria are subject to changes from year to year. Providing inaccurate or incomplete information on any application form or attachment voids the entire application. Furthermore, I understand that the clinical components of the Radiologic Technology Program are offered at various clinical affiliations, which most likely will not be within commuting distance of Baker College. BAKER COLLEGE RADIOGRAPHY ACKNOWLEDGEMENT FORM I understand and accept the following statements and information: 1. The prerequisite courses for the Radiologic Technology program at Baker College apply as follows: A grade point of 2.7 GP or better in each course is required for application eligibility. SCI 101C MED103 SCI 102C ENG 102 2. SCI 211 (pathophysiology) must be completed with a grade point of 2.7 or better before starting the program in the fall quarter. A test-out or grade of 2.7 or higher in MTH 111 and ENG 101 must also be earned prior to starting in the fall. 3. Criminal convictions may prevent me from being placed for necessary clinical experiences, which would make program completion impossible. 4. Arrests and/or criminal convictions that occur while enrolled in the program must be reported immediately to the program director or clinical coordinator. 5. I am willing to complete clinical courses at any program-approved healthcare provider site in the state of Michigan. For many students, this requires temporary relocation. 6. I have read and understand the Baker College Radiologic Technology Program Overview packet that is currently available in the faculty wing reception area at the school. 7. I will need to own a computer with Microsoft Office installed for the second year of the program. Internet access will also be required. 8. These points are based upon number of credits in each course, and whether or not the course is double weighted. Please circle the points earned in each course to track your progress. A student who earns at least 40 points in this category may be able to earn enough additional points on the HESI entrance exam to be selected for admission. The maximum possible points = 50. HESI Entrance Exam An exam will be administered by the program during week nine of spring quarter. The test focuses on anatomy and physiology and math. Because there are no points earned from your math course grade, the math section is worth twice as much as the A&P section of the exam. A weighted composite score of at least 75% earns points. The maximum possible points in this category = 45. (Example: A score of 100% = 45 points). Use the table below to list the points that correspond to your HESI test score.
