ARTICLE I INTRODUCTION Awards Policy Book The National Association of Health Services Executives (NAHSE) presents acknowledgements, awards and scholarships to members and chapters for outstanding contributions to healthcare, local communities and NAHSE. Acknowledgements, awards and scholarships reinforce the mission of NAHSE and recognize the significant impact members make in advancing the NAHSE agenda. The following acknowledgements, awards and scholarships are bestowed by NAHSE: Acknowledgements NAHSE Hall of Fame NAHSE Service Pin Award Organization of the Year President s Award Chapter Awards Building Block Chapter of the Year Award Percy Allen, II, Chapter of the Year Award Program of the Year Individual Awards NAHSE s Image Award NAHSE s Promise Award Senior Healthcare Executive of the Year Young Healthcare Executive of the Year Scholarship Awards Ellis Bonner Scholarship Florence Gaynor Scholarship Haynes Rice Scholarship ARTICLE II NAHSE AWARDS OVERVIEW Section 1 Distribution and Eligibility Awards are optional and shall be given at the discretion of the President. Awards may be presented by the President or his/her designee. Current elected officers are ineligible to be nominated for NAHSE Awards with the exception of acknowledgements. In the case where a vacancy of a normally elected position is

appointed, and the appointment term overlaps with the awards timeline, that person shall also be ineligible. Section 2 Award Nominations All nominations and applications must be submitted in the means determined by the President or his/her designee by June 30 th at 11:59PM. These means will be communicated by March 1 st. Section 3 Selection Recipients will be selected by the NAHSE Awards & Scholarship Committee consisting of co-chairs appointed by the President and a diverse group of NAHSE members in good standing. Applications and nominations are to be rated based on the content of information submitted only- no outside information or general knowledge shall be considered. All selections will be made using an objective award review process to include a scoring tool and discussion. During the voting process, each award category shall include a No Confidence option. Award recipients shall only be communicated to the President and NAHSE National Office prior to the Annual Educational Conference. Section 4 Submission All applications and nominations will be submitted to the NAHSE National Office. ARTICLE III ACKNOWLEDGMENTS Section 1 Eligibility NAHSE members in good standing may submit a nomination for a NAHSE Acknowledgment. Section 2 NAHSE Hall of Fame Purpose: The award recognizes outstanding African-American healthcare professionals who have been active members of NAHSE for their lifetime contribution to healthcare, young healthcare professionals, health policy, advocacy and NAHSE. This award is granted periodically and is not meant to be an annual presentation. This award is strictly for those who have made extraordinary strides in the healthcare industry and been members of NAHSE for 30 years or more. 2

Selection: Recipients will be selected at the discretion of the President and the Executive Committee. The award will be presented during the Gala of the Annual Educational Conference, with the recipient receiving a plaque. Section 3 NAHSE Service Pin Purpose: The award recognizes African-American healthcare professionals who have been active members of NAHSE for five (5), ten (10), and twenty (20) years constantly striving to enhance and contribute to the field of healthcare, young healthcare professionals, and NAHSE. Selection: Recipients will be selected based on the receipt of complete, qualified applications. The award will be presented during the Thursday Host Chapter Event at the Annual Educational Conference, with the recipient receiving a lapel pin and certificate. The recipient s name will be included in the Annual Educational Conference Booklet. There is no limit to the amount of awards that can be given in a single year. a. Active membership for the service award sought. This does not need to be consecutive years, but total the amount of years needed for the award desired. b. Active involvement in NAHSE at the local and/or national level(s). c. Attendance to at least one NAHSE-related event (Annual Educational Conferences, NAHSE Leadership Meetings, etc.) for each year of service. Section 4 Organization of the Year Purpose: The award recognizes healthcare entities who partner with NAHSE providing resources and expertise to support the mission of NAHSE and the development of up-and-coming African-American healthcare executives. Selection: Recipients will be selected based on financial contribution to NAHSE. The award will be presented during the Gala of the Annual Educational Conference, with the recipient receiving a plaque. 3

Section 5 President s Award Purpose: The award recognizes individuals who have supported and assisted the President during their term providing leadership and direction to NAHSE. This award is not meant to be an annual presentation. Selection: Recipients will be selected at the discretion of the President. The award will be presented during the President Luncheon at the Annual Educational Conference, with the recipient receiving a plaque and medallion. ARTICLE IV CHAPTER AWARDS Section 1 Eligibility NAHSE Chapters in good standing are strongly encouraged to submit a nomination or be nominated for a Chapter Award. Section 2 Building Block Chapter of the Year Purpose: The award recognizes and celebrates new NAHSE chapters who have developed strong foundations and infrastructure. This award seeks out new chapters who demonstrate exceptional achievement in the healthcare arena and contributions to African-American healthcare professionals and NAHSE. Pages: Nominations will be no longer than ten (10) pages [10-12 point font]. Covers and title pages are not included in the page count, but dividers are included. All nominations must contain page numbers. There is no page limit for the appendices. The award will be presented during the Leadership Luncheon of the Annual Educational Conference, with the recipient receiving a plaque and a $500 chapter development grant. a. Established chapter for less than five (5) years. b. Compliant with NAHSE Bylaws and policies. 1. Chapter President and/or designee must have attended the NAHSE Board Meetings for the past year. 2. Chapter President and/or members must have volunteered to serve on national committees within the past year. 3. Chapter President and Treasurer must have submitted the required quarterly reports for the past year. 4

This award was created in an effort to celebrate the high level of initiative and professionalism that exists in Chapter programming that reaches outside of the confines of the meeting room and touches individuals in local communities on a personal level. Pages: Nominations will be no longer than twenty (20) pages [10-12 point font]. Covers and title pages are not included in the page count, but dividers are included. A table of contents must be included. All nominations must contain page numbers. The award will be presented during the Leadership Luncheon of the Annual Educational Conference, with the recipient receiving a plaque and a $500 grant to support conference attendance the following year. The recipient will be expected to present on the 7

planning, development, and execution of the program as an educational session at the next Annual Educational Conference. a. Section One: Introduction and Overview 1. One-paragraph executive summary of the nomination. 2. Describe how the program originated. 3. Describe the educational/advocacy component of the program. 4. Describe whom the program was designed for and the individuals that benefited from the program. 5. Describe the goals and objectives of the program. 6. Describe the specific member/community need that this program attempted to address. 7. Describe what makes this program unique. b. Section Two: Content and Planning 1. Describe the who, what, when, where, why and how of the program. 2. Describe how members were involved in the conception, planning and overall presentation of the program. 3. Describe the planning process used in development of the program. 4. Describe how the program was marketed. Provide samples of advertising. 5. Describe the resources used in developing the program, including information on the following: a. Budget - provide a budget, with specific line items and funding sources. Describe how costs changed or how they were met. b. Supplies - list the supplies used and/or needed to implement the program. c. People - describe how many members were involved and their specific roles in the conception, planning and overall presentation of the program. d. Community Resources - describe the community resources that were used to plan and present the program. 6. Describe how this program affected members and/or the community. ARTICLE V INDIVIDUAL AWARDS Section 1 Eligibility NAHSE members and chapters in good standing are encouraged to submit nominations for NAHSE Individual Awards. Individuals may only be nominated for one (1) NAHSE Award during a nomination cycle. Members residing in areas without local chapters are eligible to submit nominations for this award. No recipient shall be nominated for the same award after it has been bestowed. Section 2 Pages Nominations will be no longer than twelve (12) pages [10-12 point font]. Covers and title pages are not included in the page count, but dividers are included. All nominations must contain page numbers. Section 3 NAHSE s Image Award Purpose: The award recognizes and celebrates the contributions and efforts of African-American healthcare professionals to the field of healthcare and significant hands-on involvement and achievement within NAHSE. The award will be presented during the President s Luncheon of the Annual Educational Conference, with the recipient receiving a plaque, and mentee role for the Ellis Bonner Scholarship recipient. a. Active membership for a minimum of ten (10) years. b. Minimum of fifteen (15) years progressive experience in a healthcare organization(s). c. Mid-level or senior executive leadership position in a healthcare organization(s) for a minimum of five (5) years. d. Demonstrated significant contribution(s) to NAHSE. e. Demonstrated commitment to the advancement and development of African-American healthcare professionals and 9

to the advancement and/or inclusion of African- Americans in healthcare leadership. f. Active involvement in NAHSE at the local and/or national level(s). g. Involvement in advocacy and policy development. h. Active in community organizations. Section 4 NAHSE s Promise Award Purpose: The award recognizes and celebrates the contributions and efforts of young members who have had significant handson involvement in advancing the mission of NAHSE. The award will be presented during the President s Luncheon of the Annual Educational Conference, with the recipient receiving a plaque. a. NAHSE member under the age of 35. b. Active membership for at least three (3) years. c. Less than seven (7) years management experience in a healthcare organization(s). d. Demonstrated significant contribution(s) to NAHSE. e. Active involvement in NAHSE at the local and/or national level(s). f. Active in community organizations. Section 5 Senior Health Services Executive of the Year Purpose: The award recognizes and celebrates the major contributions and efforts of seasoned African-American healthcare professionals to the field of healthcare and NAHSE. The award will be presented during the President s Luncheon of the Annual Educational Conference, with the recipient receiving a plaque, medallion, and mentee role for the Florence Gaynor Scholarship recipient. a. NAHSE member over the age of 40. b. Active membership for a minimum of ten (10) years. c. Minimum of fifteen (15) years progressive experience in a healthcare organization(s). d. Senior executive leadership position in a healthcare organization(s) for a minimum of five (5) years. 10

e. Demonstrated leadership and vision. f. Demonstrated commitment to the advancement and development of African-American healthcare professionals and to the advancement and/or inclusion of African-Americans in healthcare leadership. g. Active involvement in NAHSE at the local and/or national level(s). h. Demonstrated commitment to the development and career of young healthcare professionals. i. Major contribution(s) to healthcare. j. Involvement in advocacy and policy development. k. Active in community organizations. Section 6 Young Health Services Executive of the Year Purpose: The award recognizes and celebrates the major contributions and efforts of young African-American healthcare professionals to the field of healthcare and NAHSE. The award will be presented at the President s Luncheon of the Annual Educational Conference, with the recipient receiving a plaque, medallion, and mentee role for the Haynes Rice Scholarship recipient. a. NAHSE member under the age of 40. b. Active membership for a minimum of five (5) years. c. Minimum of seven (7) years progressive management experience in a healthcare organization(s). d. Active involvement in NAHSE at the local and/or national level(s). e. Demonstrated commitment to the development and career of young healthcare professionals. f. Major contribution(s) to healthcare. g. Involvement in advocacy and public policy. h. Active in community organizations. ARTICLE VI SCHOLARSHIP AWARDS Section 1 Eligibility NAHSE members in good standing may submit an application for a NAHSE Scholarship. Applicants must be African-American graduate students enrolled in a Commission on the Accreditation of Healthcare Management Education (CAHME), Council on 11

Education for Public Health (CEPH), Association to Advance Collegiate Schools of Business (AACSB), or American Association of Colleges of Nursing (AACN) accredited graduate program, pursuing a master or doctorate degree, and majoring in healthcare administration or a related field. Applicant must be matriculating during the current academic school year. Section 2 Application Requirements: a. Cover Letter b. Application Portfolio Checklist c. Scholarship Application Form d. Personal Photo (3 x 5 head shot in black and white) Section 3 Ellis Bonner Scholarship Purpose: The award is designed to defray the cost of tuition for outstanding non-traditional graduate students who are striving to become future leaders in healthcare management and NAHSE. Non-traditional, for the purpose of this award, shall be defined as, an individual who works full-time while enrolled and/or has dependents other than a spouse. The award will be presented during the Gala of the Annual Educational Conference, with the recipient receiving $2,500 and mentorship from the NAHSE s Image Award recipient. a. Non-traditional graduate student. b. Demonstrate financial need. c. Minimum grade point average (GPA) of 2.5 for undergraduate and 3.0 for graduate on a 4.0 scale. d. Personal and career goals, commitment and interest in healthcare management. e. Distinguished leadership f. Participation in professional development opportunities. g. Section 4 Florence Gaynor Scholarship Purpose: The award is designed to defray the cost of tuition for outstanding female graduate students who are striving to become future leaders in healthcare management and NAHSE. The award will be presented during the Gala of the Annual Educational Conference, with the recipient receiving $2,500 and mentorship from the Senior Health Services Executive of the Year Award recipient. a. Female graduate student. b. Demonstrate financial need. c. Minimum grade point average (GPA) of 2.5 for undergraduate and 3.0 for graduate on a 4.0 scale. d. Personal and career goals, commitment and interest in healthcare management. e. Participation in professional development opportunities. f. Participation in the local community. Section 5 Haynes Rice Scholarship Purpose: The award is designed to defray the cost of tuition for outstanding graduate students who are striving to become future leaders in healthcare management and NAHSE. The award will be presented during the Gala of the Annual Educational Conference, with the recipient receiving $2,500 and mentorship from the Young Health Services Executive of the Year Award recipient. a. ARTICLE VII CHANGES IN AWARD POLICY The Award and Scholarship Committee may not make a change in award criteria without submitting a written request to the President and obtaining approval. Roy L.
