Integrated Health Service Plan (IHSP) Action Items The CE LHIN, in establishing system performance indicators, will develop elder sensitive measures that encourage high quality and safe care of the seniors population. The CE LHIN and its community partners will further the concept of a local Quality Conference that would disseminate best practice approaches and facilitate knowledge exchange related to quality and safety for seniors. To enhance quality of care through opportunities for sharing resources and coordinating functions Wait time task group to facilitate inter-facility working partnerships that will improve access to provincial targets Conduct a critical care health human resources needs assessment and explore coordinated recruitment Facilitate, as needed, the timely approvals and implementation of the Community Health Centres currently under development in the area. Obtain current information on where telemedicine equipment currently resides, its capacities and the network of human resources that are providing services through this medium. Explore coordinate regional training schedule for the use of telemedicine/video-conferencing CE LHIN Core - Telemedicine CE LHIN Core - Telemedicine Links to Active Projects Assess capacity for stroke rehab Rehab Continuum. (A statement of commitment by consumer/caregiver and service provider's which outlines the behavioural modifications/activities that are recommended) Host a Transportation Summit that will bring together consumers, caregivers and providers to identify actions for improving transportation services, thus improving access to services. Once completed, the Central East LHIN will review the evaluation of the Medication Management project through the former Scarborough Community Care Access Centre to determine future opportunities for this model. Interdisciplinary Teams. Explore opportunities to augment or partner with Primary Care Providers (e.g. Family Health Teams, CHCs, OTHERS) in the provision of preventative, maintenance and restorative services/programs Establish a FLS Primary Care Hub Establish HHR Working Group Provider education on management of complex, multiple CD's Coordinated effective method to collect/share information. across programs Client information travels with them to various chronic disease providers Identify barriers and opportunities to form interdisciplinary care teams for people with chronic disease Knowledge Exchange - CQI and Standards.
