Introduction This questionnaire should be completed by the Program Director at your treatment facility with assistance and oversight from staff members knowledgeable about the resource use and costs for the treatment program as designated by the Program Director. The questionnaire is designed to collect resource use and cost information pertaining to your outpatient methadone treatment program for the previous fiscal year. Please complete Sections A through J of this questionnaire, following the detailed instructions provided. To complete the questionnaire, please use expenditure reports rather than budgets, because budgets do not always coincide with actual resource use. The information provided in this questionnaire, or through any other part of this study, will be held in confidence and will not be reported in a way that could directly identify you or your program.
