Deadline Date: March 2, 2015 FOOTHILLS SURGICAL TECHNOLOGY CONSORTIUM APPLICANT INFORMATION FOR FALL 2015 Isothermal Community College, McDowell Technical Community College, and Cleveland Community College together as Foothills Surgical Technology Consortium, offer a one-year diploma program in Surgical Technology. The Surgical Technology curriculum prepares individuals to assist in the care of the surgical client in the operating room and to function as a member of the surgical team. Graduates of this program will be eligible to apply to take the National Board of Surgical Technology and Surgical Assisting Certification Examination for Surgical Technology. Employment opportunities include: inpatient/outpatient surgery centers, labor/delivery units, dialysis units/facilities, physician s offices, and central supply processing units. Prospective Surgical Technology applicants residing in Rutherford/Polk, McDowell, and Cleveland counties must apply to the college serving their county. Other applicants may apply to the college of their convenience. Priority will be given to service area applicants first, then other North Carolina residents, then out of state residents. * Interested students should attend a Health Sciences Information Session. DEADLINE: The deadline for Fall 2015 enrollment is Monday, March 2, 2015. The program last one academic year (Fall 2015, Spring 2016, and Summer 2016), while accepting 10 students per year. ISOTHERMAL COMMUNITY COLLEGE ADMISSION REQUIREMENTS: 1. Complete and submit an online application for admission to Isothermal Community College declaring Surgical Technology (A55280S) as the major. Current and returning students may complete a Change of Major form in admissions to declare Surgical Technology (A55280S) as a major. 2. Submit official transcript of high school education or GED scores to the admissions office. 3. Submit official transcript(s) from all colleges attended to the admissions office. Courses with a grade of C or better will be considered for transfer. 4. Complete the college placement test (exemptions are possible with SAT/ACT scores or college level English/Math courses). Must have approval through the Admissions office. There is no charge for the test. SURGICAL TECHNOLOGY ADMISSIONS REQUIREMENTS: 1. Complete all developmental courses required as a result of placement testing with a grade of C or better. Required courses (unless exempt) include: Must have completed before deadline date. MAT 060 or DMA 010 & 020 & 030 Satisfactory completion or exemption (per transcript) ENG 095 or DRE 098 Satisfactory completion or exemption (per transcript) 2. The applicant must have a cumulative 2.0 GPA in most recent high school/college/ged course work. A grade of C or higher must be maintained in all curriculum courses taken before or after program acceptance to avoid forfeiting your space in the program. 3. Must review Surgical Technology textbooks and video, and then sign the Surgical Technology textbook/video review page. The review page must be returned to the Surgical Technology Advisor in the Advising Center before the deadline date, Monday, March 2, 2015. 4. Submit 3 Reference Forms (not from relatives or close friends) in sealed envelopes with the individual s signature across the back. Return all reference at the same time to the Advisor. Forms are included with this packet. The applicant is responsible for ensuring requirements 1-4 have been met and all materials have been received by Monday, MARCH 2, 2015 by 4:00 p.m. All admission criteria must be met to be eligible to take the PSB-Health Occupations Aptitude Exam. Completion of requirements does not guarantee admission. IMPORTANT DEADLINE DATES & ACTIVITIES: 9. Eligible applicants will be notified by either letter and/or email to report for the PSB- Health Occupations Aptitude Exam on Wednesday, April 15, 2015. A picture ID is required for identification purposes. There will be a charge for the exam of $20.00. The PSB website is www.psbtests.com. 10. Letters to attend a group session which will consist of the Director of the Foothills Surgical Technology Consortium program and the advisor at Cleveland Community College on Wednesday, May 6, 2015 from 10:00 a.m. to 12 noon. 10. Notifications of acceptances will be sent either by email or mailed letter by, Monday, June 15, 2015. 11. New Student Orientation will be Tuesday, July 14, 2015 from 10:00 a.m. to 11 the Health Science building on the Cleveland Community College campus in room 5213. Students will be fitted for uniforms from 11:00a.m. to 12:00 p.m.. Students must be prepared to pay half of the clinical supply costs on orientation day. Students enrolled during the summer session in general education courses required for the Surgical Technology program should provide a form completed by faculty teaching these courses. (Forms are available through your advisor in the Advising Center.) REQUIREMENTS AFTER ACCEPTANCE INTO THE FOOTHILLS SURGICAL TECHNOLOGY CONSORTIUM: 1. All Students accepted to the Surgical Technology program must submit a NCCCS Student Medical Form (provided by Advisor) and proof of adult, infant, and child CPR certification for the healthcare provider by October 1, 2015. The health form must be completed by a licensed healthcare provider. This form includes immunization records. These forms are due to the program director on the first day of class, Monday, August 17, 2015, fall semester, 2015. The accepted applicants status is considered pending until these completed items is submitted. Page 3 of 5
2. Students should be aware that upon admission to a program with a clinical component criminal background checks and or drug testing are required by the clinical facilities used for clinical sites. Criminal background checks are handled through a designated third party vendor. Isothermal Community College is an open door college and will not refuse admission to anyone based on a criminal record. However, the clinical site reserves the right to refuse someone with a criminal record, positive drug test, and/or visible tattoo the use of its facility. If a student does not complete the clinical portion, he or she will not be able to successfully complete the Surgical Technology program. 3. Ten (10) students will be accepted into the Surgical Technology program Fall Semester, 2015. Final admission will be based on: a. Review of the applicant s academic record. b. PSB-Health Occupations Aptitude test results. c. Group Session with Director and Advisor of Surgical Technology. d. Overall total points of the selection rubric. e. Review of Reference forms. f. Documentation of physical and emotional health that would provide evidence indicative of the applicant s ability to provide safe care to the public. g. Completion of required immunizations. h. Completion of CPR Healthcare provider certification. Any offer of acceptance is considered provisional until all required materials above are completed and received by the required time.
