The issue of foreign educaed nursing sudens has achieved naionwide aenion because he applicaion process requesing licensure varies sae-osae. This makes i relaively easy o become licensed in some saes and very difficul in ohers. Monana had hundreds of foreign educaed applican inquiries over he pas few monhs and is working closely wih numerous agencies o ensure ha only qualified applicans are approved for NCLEX esing or licensure by endorsemen. Sandra Dickenson Joins he BON saff Sandra M. Dickenson, Sandi, acceped a emporary one year posiion as Execuive Direcor for he Monana Board of Nursing on January 18, 2005. Sandi obained her BA in Nursing from Carroll College in 1987 and her MS as a CNS in Maernal-Child Nursing from VCU Medical College of Virginia in 1997 wih a secondary area in Educaion. She joined he USAF in 1987 and has acceped assignmens ranging from Med-Surg, LDRP and PEDS o Case Manager, Fligh Nurse, hospice, and firs surgical assisan. Her mos recen posiions were as a Nurse Manager of he MACDILL Women s Healh Clinic in Florida and as a Uni Supervisor of he KADENA Family Pracice Clinics serving 14,000+ miliary paiens in Japan. She was employed as adjunc faculy in Virginia for he ADN (LPN o RN) program and currenly holds ANCC cerificaion as an Informaics Nurse. The BON welcomes her applicaion of educaion and experience o Monana s nursing regulaion and governance. Message from Execuive Direcor Sandi Dickenson Reflecing over he pas five monhs, I ponder where o begin. Exciemen swep over me ha firs day as I enered he room where he BON was convening on January 18h. My commimen o serving such a varied audience came wih lile working knowledge of he deails. Wihin eigh days I was on a plane o he annual Federal Associaion of Regulaory Boards (FARB) joined by Board member Kahy Hayden. The nex morning Kahy lierally bumped ino one of he gues speakers as she crossed he sree! Her jovial personaliy earned us he pleasure of prime seas (nex o him) and a personal uorial of he presened maerial much appreciaed as he moderaor sared us off wih a es! I reurned o direc my firs nursing program sie visi he following week. Every nursing program in Monana ha leads o iniial nursing licensure mus obain BON approval. The iniial sie visi process involved he BON hiring an Educaional Consulan o visi he campus wih he ED and evaluae (survey) he proposed program. Approved nursing programs coninue o have surveys, known as sie visis, on a regular basis by he BON. I spen he following hree weeks seing up my work area, responding o pracice quesions and oriening as a sae employee. In March I was again on a plane, his ime headed for he Mid-Annual meeing of he Naional Council of Sae Boards of Nursing (NCSBN). We had a errific session ha firs day as he EDs were grouped wih heir BON Presidens in a variey of learning aciviies designed o help us undersand our role in governance. NCSBN provided an invaluable seing where Presiden Karen Pollingon, Member Cynhia Pike and I could nework wih oher sae BONs for informaion on a wide variey of nursing concerns. We soon learned ha MT is no alone in addressing issues such as expanding pracice, deermining compeency, how he mobiliy of nursing pracice across sae lines impacs regulaion and he recogniion of APRN program graduaes. I reurned o cach up on my office work, mee wih subcommiee ask forces and prepare for he April BON meeing. In my spare ED ime I handled he Pracice Manager duies due o an ongoing posiion vacancy. May Day quickly arrived and I headed o Malmsrom AFB in Grea Falls where I served 15 days of acive duy as a Reservis in he USAF. The value of keeping one hand in clinical nursing ruly assiss my undersanding of Nursing Pracice: how regulaion affecs pracice, how pracice rends effec BON Rule changes and how Rule changes affecs pracice. Having seled in a bi over he pas six weeks, I am awed by he dedicaed specialy nurses who voluneer heir knowledge and experise o ensure ha Monana Nursing Pracice remains he VERY BEST. I have me legislaors, adminisraors, educaors, clinicians, members of various healh care boards, healh professionals and deparmen saff who serve selflessly on ask forces and commiees. Many ravel repeaedly over long disances and spend counless hours in meeings in order o assis he BON in our goal of safe nursing pracice. My hanks o every one of you! HB279 esablished he concep of pharmaceuical care, auhorized elecronic ransmission of prescripion informaion, enabled board regisraion and discipline of pharmacy echnicians and auhorized collaboraive pracice, defining i as...he pracice of pharmacy by a pharmacis who has agreed o work in conjuncion wih one or more prescribers, on a volunary basis and under proocol, and who may perform cerain paien care funcions under cerain specified condiions or limiaions auhorized by he prescriber (MCA 37-7-101(4)). The bill also amended he definiion of he pracice of pharmacy o include adminisering drugs and devices pursuan o a collaboraive pracice agreemen... (MCA 37-7-101(26b)) and iniiaing or modifying drug herapy in accordance wih collaboraive pharmacy pracice agreemens esablished and approved by healh care faciliies or volunary agreemens wih prescribers (MCA 37-7-101(26e)). Pharmaciss herefore have dependen prescripive auhoriy, as opposed o independen prescripive auhoriy. Pharmaciss canno iniiae or modify drug herapy wihou having a valid collaboraive pracice proocol in place. The rulemaking, noicing and public commen process was hen begun. Collaboraive pracice agreemens mus be reviewed, revised if necessary and renewed annually. MCA 124.74.503, Adminisraion of Vaccines by Pharmaciss, can also be found on he board s websie above. Pharmaciss can vaccinae only pursuan o a collaboraive pracice proocol, and may only vaccinae paiens 18 years of age or older. They mus have successfully compleed a raining course provided by he Ceners for Disease Conrol or oher approved provider, mus hold a curren basic CPR cerificae, follow esablished procedures and mainain a curren copy of CDC s Epidemiology and Prevenion of Vaccine- Prevenable Diseases. The Board of Pharmacy presenly has less han 20 collaboraive pracice proocols on file. Many of hese are vaccine proocols, and several anicoagulaion proocols are in place as well. Only one oupaien pain proocol is presenly acive in he sae, and a las coun 3 pharmaciss had applied for and received heir mid-level DEA license. The DEA will only gran mid-level licenses o pharmaciss acively involved in collaboraive pracice ha will be called upon o wrie prescripions for conrolled subsances. The Board of Pharmacy feels ha collaboraive pracice is a poenially useful ool ha can be of use in he provision of healh care o he ciizens of our rural sae.
How License Probaion affecs your Nursing Pracice by Lori Ballinger Boh on he naional and sae level, approximaely 1% of all regisered nurses and 1.5% of all licensed pracical nurses have been subjec o disciplinary acion by a licensing board. The violaions range from coninuing o pracice wih an expired license o paien abuse and drug diversion. Similarly, he board may impose sancions ranging from a privae leer of reprimand or censure o license revocaion. Mos ofen, if he board decides here is reasonable cause o proceed wih disciplinary acion, he board will place he nurse s license on probaion. The probaionary erms vary from case o case. Probaion is designed o monior nursing pracice and remedy any pracice issues he board has idenified. Generally, probaion will include quarerly repors from he nurse and his or her supervisor, yearly requiremens for coninuing educaion and a requiremen o review and follow all he saues and rules ha govern nursing. The goal of license probaion is o insure ha he nurse is able o coninue o pracice safely and appropriaely. For nurses who have had problems wih drugs or alcohol, he board has esablished he Nurses Assisance Program (NAP). Mos nurses enrolled in he NAP coninue o work in nursing. The NAP closely moniors each nurse o insure compliance wih he program requiremens. Nurses paricipaing in he NAP are acively working on recovery for subsance abuse. Wih paricipaion in he NAP, recovering nurses can coninue o conribue o he profession. Niney-five percen of he nurses on probaion coninue o be employed in nursing. Anoher concern was raised ha online schools are allowing nursing sudens o se up heir clinicals in Monana. We found consensus ha responsibiliy for using ionizing radiaion applies o healh care personnel a every level. As is rue for all boards, BORT s primary charge is proecion of he public. Boh boards agree ha inappropriae applicaion of ionizing radiaion, especially he use of he new C-Arms, has led o grave concerns over public safey and ha nurses play a key role in ensuring public safey during hese procedures. Ionizing radiaion usage is increasing in frequency and level according o respeced sources. BORT s license inspecor, Dan Owen, revealed a number of siuaions which have been addressed bu we coninue o receive phone calls quesioning he appropriae use of ionizing radiaion, boh hypoheical and real. Some of hese involve nursing saff being required o operae C-Arms wihou knowledge of or raining in radiaion safey or X-Ray producion. Nurses are seing up he C-Arm, urning on he uni, maneuvering and posiioning he uni and he paien and seing exposure facors. NPs are compleing bone densiy exams relying solely on vendor informaion and applicaions. Limied Permi Technologiss are direced o operae everyhing from fluoroscopy unis and C- Arms, o CT Scanners and old i s OK as long as he PA, NP, or a physician is presen and orders hem o do so. For clarificaion, nurses canno do so unless licensed o pracice under BORT; a limied permi holder can only expose afer films following a fluoroscopy exam. The radiologis mus se all exposure parameers and conrol all oher radiaion producion facors of fluoroscopy. Misinformed individuals repeaedly relae ha he new C-Arms don use radiaion any more or he amoun is so minue, here is no concern o anyone. Radiaion warnings, paricularly fluoroscopic applied radiaion warnings, are issued by he FDA and oher agencies bu apparenly are no reaching all providers in Monana. Chip Pickering (R-MS) under he ile: Consumer Assurance of Radiologic Excellence (CARE). The CARE bill is biparisan legislaion, wih 10 Republican and 10 Democra original cosponsors. The bill would amend he Consumer-Paien Radiaion Healh and Safey Ac, a 1981 law ha esablished minimum sandards for he educaion and credenialing of radiologic echnologiss. Robera Three, LPN resigned her posiion effecive in April and we are awaiing a new appoinee o he vacancy by Governor Schweizer.
