(Click the icons to advance to respective page) 3 Medical Aetna is our plan administrator. We offer two medical plan options from which to choose. Dental Our dental plan is administered by Aetna, and offers a choice of DPO and DMO plans. Vision Vision Service Plan is the largest vision plan administrator in the country. Life Insurance This benefit is 100% by EMPLOYERS. You may also elect to purchase additional, supplemental coverage. Disability Income Our disability policies are insured and administered by Sun Life Financial. Note that EMPLOYERS pays 100% of short-term and long-term disability premiums. 401(k) Retirement Savings Our 401(k) plan is administered by T.Rowe Price. Use tools and resources to plan for your retirement Employee Assistance You and all members of your household can receive up to the designated number of face to face counseling sessions that have no direct cost to you (i.e. copays or deductibles). Business Travel/Accident This program provides you with immediate access to doctors, hospitals, pharmacies, and certain other services when faced with a medical emergency while traveling on business.
Medical Plans Medical EMPLOYERS offers the choice between three comprehensive medical plan options, administered by Aetna. Aetna Select Plan With the Aetna Select EPO plan, you receive care from primary care physicians (PCPs), specialists, hospitals and other health care providers who belong to Aetna s network. These network providers are independent physicians and facilities that must meet strict requirements for quality and service. Note that with the Aetna Select EPO plan, all care must be received within the Aetna network of providers. Aetna Choice HD Health Savings Account Plan (HSA) The Aetna Choice HD Health Savings Account plan (HSA) is a high deductible PPO plan that is accompanied by a special savings account. In 2015, EMPLOYERS will deposit $600 per year into the accounts of individuals currently employed at EMPLOYERS and enrolling in the HSA plan, and $1,200 per year into the HSA accounts of employees actively employed at EMPLOYERS and enrolling themselves and eligible dependents into the HSA plan. The EMPLOYERS annual HSA deposit is divided by the 26 pay-periods and deposited as earned. Flex Spending Accounts and HSA: Your Spending and Saving Accounts About Flexible Spending Accounts There are three separate and distinctive flexible spending accounts, the health care reimbursement account (Medical FSA), the dependent care assistance program (DCAP), and the Commuter Benefit. A health care FSA is a separate benefit plan that allows you to direct a part of your pay, on a pre-tax basis, into a special account that can be used throughout the year to reimburse yourself for certain out-of-pocket health care expenses. Because this money goes into your health care FSA before federal income, FICA (Social Security and Medicare) and most state taxes are withheld, you pay less in taxes which gives you more disposable income. Plan Cost Comparison Tool We ve built a dynamic, interactive tool that compares the relative differences in overall cost of our medical plans. Use the tool to illustrate differences in payroll contributions, enrollment selection, potential tax savings of hypothetical deposits into an HSA account, and hypothetical future medical expenses. 4
Flexible Spending Flex Spending Accounts Your Flex Spending Account (FSA) is a tax-favored spending account that allows you to save tax dollars while spending money on everyday, eligible medical expense. Using pre-tax dollars to pay for everyday eligible medical related expenses can put tax savings in your pocket. If you d like to receive your FSA reimbursements in the most timely and efficient manner, you should take advantage of the FSA Streamline option. Commuter Benefits The Commuter Benefit Program works differently than the medical and dependent care FSA programs. EMPLOYERS allows you to pay for qualified transit and parking expenses with pre-tax dollars. Click here for more information. Dependent Care Spending Account The dependent care assistance program (DCAP) is a great way to pay dependent care expenses and lower your taxable income. Here s how it works: You direct part of your before-tax pay into a special account to help pay workrelated dependent care costs You can use your account throughout the year to help pay for eligible expenses Your expense must be for the purpose of allowing you and, if married, your spouse to be employed Remember: When clicking links, use Ctrl-Click (PC), or Cmd-Shift-Click (Mac) to have your media open in a new window. 5
Dental Plans Dental Plans EMPLOYERS offers the choice between two comprehensive dental plan options, administered by Aetna, the DPO dental plan and the DMO plan. Aetna DPO Plan The Aetna dental DPO and DMO plans provide you a choice between more flexibility or low-expense. The DPO plan allows you the flexibility of using a dental provider contracted with Aetna or a dental provider this is out of the DPO plan network. The advantage of using the DPO provider that is in-network is that the provider has agreed to charge significantly less for services than what would be the normal retail charge. If you use a dental provider that is not contracted with Aetna, your services will be covered, but you may pay an additional amount to your provider for services. Using a provider in the network reduces your out of pocket expenses. The DMO plan offers a more narrow network of available providers, but very low out-of-pocket expense. With the DMO, you must elect a Primary Care Dentist (PCD). The PCD must authorize all care provided by the plan. Vision Plan Vision Experience the private-practice difference with a VSP doctor. You can get everything you want from a retail chain at a private-practice doctor, including weekend and evening appointments and the same eyewear selection, including designer brands. How To Use VSP Using your VSP benefit is easy. Review your benefits information. Login to review your plan coverage and see if you re eligible for services before your appointment. At your appointment, tell them you have VSP. There s no ID card required. A vision card is available to print from the full site or download from the mobile site. Life Insurance Life Insurance Life insurance is important because it provides financial security for your loved ones in the event of your death. With life insurance, you are able to ensure that your beneficiary has some replacement of income you would have provided. The Basic Group Term Life insurance benefit is equal to 2x your Basic Annual Earnings to a maximum benefit of $600,000. Guarantee Issue is $600,000. Accidental Death and Dismemberment (AD&D) insurance which would pay an additional benefit, up to the amount of your Life benefit, if you suffer a covered loss due to an accident. Supplemental Life Insurance/AD&D You may also elect to purchase additional supplemental life insurance and AD&D on a voluntary basis with payroll deducted premiums. Voluntary Employee Life Can be elected at 1x, 2x, 3x, or 4x s your annual salary. Up to a maximum of $500,000.00. Optional Spouse Plan Voluntary Spouse Life can be elected in increments of $10,000.00. Voluntary Spouse Life cannot exceed 100% of employee coverage. Optional Dependent Child Life Coverage Voluntary Child Life can be elected in increments of $2,500.00, $5,000.00 or $10,000.00. Covers all of your dependent children up to the age of 19 or 25 if full time student. 8
Disability Income Protection Disability Insurance Disability insurance is important because it provides replacement income should you become temporarily disabled. You may need this income to pay for ongoing expenses such as your mortgage, car payments, medical expenses, and other daily expenses. There are two distinct disability income insurance programs that work together to protect you from financial hardship if you are unable to work due to accident or injury, the Short Term Disability (STD) and Long Term Disability insurance (LTD) plans. Both plans are paid for in full by EMPLOYERS. Short Term Disability Short Term Disability insurance (STD) is intended to provide income for a short period in the event you become ill or injured and are unable to work. Coverage for all full-time employees working 20 hours or more per week. Coverage for accidents and sicknesses for up to 13 weeks. Weekly benefit is 60% of your weekly salary. Maximum benefit is $3,500 per week. Benefits begin on the 1st day absent for accidents and on the 8th day for sickness. Long Term Disability Long Term Disability Long Term Disability (LTD) coverage provides income protection in case of an extended illness or injury. Available to all full-time employees working 20 or more hours per week. Coverage for accidents and sicknesses. Benefits are 66.67% of monthly earnings up to a maximum of $15,000 per month. Benefits may begin after the elimination period of 90 days of absences due to a covered accident or sickness. 9
401(k) Retirement Savings 401(k) Retirement Savings The purpose of the 401(k) plan is to help you save for retirement and other long-term financial goals. That s why the IRS allows you to save on a before-tax basis. Retirement Where will your retirement money come from? It s a fact that people today depend less on traditional sources of retirement income, such as Social Security and pensions. For example, the average retiree (someone 65 or older with at least $55,889 in annual income in 2008) currently receives only 18% of his or her income from Social Security. For the rest, the retiree must depend on personal savings, including retirement plans like the one your employer offers. In 2015, by law, the maximum amount you can defer is $18,500; for those 50 and older, an additional $5,500 catch-up contribution can be made, for a total of $23,000. EMPLOYERS will match the first 3% of your contribution, dollar for dollar, and the next 2% at 50%. 10

Enroll Online ADP Workforce Now ADP Workforce Now, also known as your Employee Self-Service Center (ESS), is the website you use to sign-up for all of your benefits with the exception of 401(k) plans. The ADP Employee Self-Service Center allows you to view and manage your HR information including, time and attendance, payroll, and personal information. Enroll To perform this self-registration task, please use the step-by-step quick reference instructions. Click here to download and print. The first time you log into ADP, you will be prompted to go through the one time HRB Data Update Wizard. This allows you to verify and update your personal information. Once you review and update your information through the step-by-step wizard, you will see the ADP Welcome landing page. Once you see the ADP Welcome page, you will be able to go through the new hire benefit election process. After you have self-registered, you can use ADP Workforce Now to select your benefits.
