Via dei Glicini, 9/b - 007 ROMA General information The purpose of the List of Services is to establish the relationship between the Clinic and its users. This document contains full information regarding the services available, including; a list of medical tests/examinations, opening hours and access to the Clinic, technology and quality control standards. Please note that that the following information is not a fully comprehensive list of RX SONIC S.r.l. company operations. It is designed to give patients essential user-information, rather than details of the Clinic s internal managerial and organizational processes. The List of Services encourages the user to actively collaborate with RX SONIC S.r.l. in evaluating the health services offered and the quality of service provision. This partnership between the providers and users of a public service allows the RX SONIC S.r.l. Management to continually plan and implement improvements in the services provided, giving medical assistance and protecting the user s health. The List, therefore, becomes a useful communication tool between the Clinic and its users and enables the rationalization of the activities and organization of the Clinic based upon users actual needs. One of the primary objectives of publishing the List of Services is to support the continual process of humanizing the health service, which essential to the fulfillment of citizens rights. The List of Services will be regularly checked and updated, taking into account the ideas and experiences of those who wish to work on the improvement of the services provided and, indeed, on the process of improvement. 3 DI

Via dei Glicini, 9/b - 007 ROMA Fundamental Principles The List of Services follows government guidelines issued on 7 th January,994, with specific regard to: Equality There will be no discrimination in the provision of services in relation to gender, race, language, religion, political opinion, psychophysical or socio-economic. Impartiality Services will be provided in an objective, impartial and neutral manner. Continuity Services will be provided with continuity, regularity and without interruption. In the event of an interruption or irregularity in service provision, all necessary action with be taken to avoid any inconvenience to the user. Right to choose In accordance with current regulations, wherever practical the RX SONIC S.r.l. Radiology Health Clinic commits to provide flexibility in the delivery of its services. Participation The RX SONIC S.r.l. Radiology Health Clinic fully encourages both users and staff to participate fully and responsibly in the implementation and management of the List of Services, in line with Clinic procedures. The simplification of procedures and the provision of information in a full and transparent manner. Efficiency and effectiveness The activities performed by the RX SONIC S.r.l. Radiology Health Clinic conform to criteria of efficiency and effectiveness, also with regard to opening hours for the public. 4 DI

Via dei Glicini, 9/b - 007 ROMA Services available and general details RX SONIC S.r.l. is a specialist, diagnostic radiology health clinic. The Clinic was established by Dr. Valter with the aim of developing and expanding a service based on respect and safety and which guarantees users/patients a highly-professional and reliable standard of care. The Clinic is situated at Via dei Glicini 9/b, in Rome and is divided into the following sections: Medicine Appointments desk and radiology waiting room; Diagnostic department for digital images; Dental surgery with the following specialist medical equipment: QR TAC (cone beam) NewTom 3G model; Instrumentarium OP 00 D ortopantomograph with digital telecranium; Instrumentarium Focus digital intraoral equipment. Electronic imaging diagnosis room with the following specialist medical equipment: Folding, remote-controllable Villa TomoJupiter, with image brightness intensifier; Pacioni Mammo Mammography X-Ray System Bone Mineraldensity Test (MOC) room with the following specialist medical equipment: ACN Unigamma computerized bone denstiometer Scan diagnosis room with the following specialist medical equipment: Esaote MyLab 50 scanner Specialist/consultant s examination room Administration and secretarial Appointments desk for clinical services and diagnosis reports; Waiting room for 0 people; Administrative office. The Clinic contains a total of four bathrooms (with separate facilities for patients and staff), one of which is situated within the scanning room to accommodate patients who need to undergo medical examinations with a full bladder. In 003, RX SONIC S.r.l. began preparing to reach the necessary standards to achieve the national certificate ISO 900:000. On 0th February 005, the Clinic was awarded l Ente Certificatore DNV Italia. On 4 th February 00, the Clinic received the ISO 900:008 certificate, therefore confirming its adherence to the latest government guidelines. RX SONIC S.r.l. selects the best-suited and qualified people for each service it offers and benefits from the many years of experience gained by the Management team. The level of services offered is based upon the company s series of quality requirements; in particular: knowledge of the sector and willingness to accept new working ideas; qualified staff with the technical skills to guarantee precise and punctual services, in line with the company s Quality Control System; a well-organized, hierarchical organizational structure, built on professional experience and capable of confronting any managerial difficulties relating to the provision of medical services to patients/users; utmost attention paid to patients needs and expectations; 5 DI

Via dei Glicini, 9/b - 007 ROMA Location The RX SONIC S.r.l. Radiology Clinic is located close to Piazza Dei Mirti and Piazza S. Felice da Cantalice, off Via dei Castani. The Clinic is easily accessible by public transport: Tram 4, 5 e 9 from Termini Station, Piazza Ungheria, Verano and Porta Maggiore; Bus number 54 from Piazza Bologna, number 3 or number 3 from Largo Preneste or number 554 from Piazza Cinecittà. 7 DI

Via dei Glicini, 9/b - 007 ROMA Opening Hours and Organization Information and Appointment Booking Requests for information and appointments for examinations and specialist visits can be made directly at the Clinic, or by telephone on 06.33704, at following times: Monday Friday from 9.00 to 3.00 and 5.00 to 8.30 Saturday from 9.00 to.00 RX SONIC S.r.l. provides detailed information sheets for patients on how they can best prepare in advance for each medical examination. Appointments desk The appointments desk, which manages bookings and payments, is open during the following hours: Monday Friday from 8.30 to.30 and 5.00 to 8.00 Saturday from 8.30 to.45 Payment of tickets (i.e. fees for national health service-approved medical tests/visits) with SSN or invoices for private treatment should be made when the patient checks-in at the Clinic for their treatment. Receipts and invoices are available. Further information and assistance is available from staff at the Appointments Desk. Additional care and flexibility is provided for patients with any type of injury or physical disability, or those with critical conditions, with regard to booking, checking-in for treatment, the conduction of medical examinations and the collection of test results. The collection of test results or medical report Reports on specialist medical examinations are made available at the end of each appointment. All other types of test results - radiography, scans, TAC, MOC, etc can be collected from the Clinic starting from the date stated on the receipt/invoice provided during the check-in process. Test results can be collected during the following hours: Monday to Friday, 9.00 to 3.00 and 5.00 to 8.30 Saturday, 9.00 to.00 The patient receipt/invoice must be presented when collecting test results. If someone other than the patient will be collecting the test results, the Clinic requires signed authorization from the patient and evidence of an ID document from both the patient and the delegate. Authorization forms (which conform with Law 96/003) are available from the Appointments Desk. With regard to minors, a written delegate is only required if the person collecting the results is neither a parent nor a legal guardian. 8 DI

Clinic, a specialist medic is always on site in the events that patients require further information on which tests to take, clarification on their test results or any other medical advice. Via dei Glicini, 9/b - 007 ROMA ISO - Quality The Management team take great care to ensure patient/user satisfaction and focuses in particular on the continual evaluation of the quality of services provided, with the development of an annual quality improvement plan. The radiology section and the organizational management of the Clinic are formally approved by the national certificate UNI EN ISO 900:008. Clinic staff are high-qualified and undertake annual training programs, both in technical scientific matters and in quality management. The company uses high-quality, precise technology and instruments and subjects equipment and production methods to daily/periodical internal and external quality control checks. The equipment is subjected to a pre-established schedule of tests, calibration and maintenance. The operational processes are run by integrated computerized systems, under the continual control of technical and specialized staff. The Clinic continually updates its technology with regard to the instruments, methods and products, with the aim of satisfying its users. Quality Indicators The Management team have specifically identified quality indicators designed to measure and direct the improvement of Clinic s activities. These indicators relate to: Users Suppliers Staff Administration The indicators are monitored on a monthly, quarterly, bi-annual and annual basis. Users Satisfaction In order to continually monitor the users perception of the quality of services provided, both customer satisfaction questionnaires and compliant forms are made available in the waiting room. These can be filled in anonymously and are intended to gather information, ideas and proactive criticism to help us in our efforts to improve the general organizational of the clinic. DI
