Long Term Home Health Care Program (LTHHCP) or Nursing Home Without Walls: The goal of this program is to help the chronically ill and disabled who require longterm health care to continue to live in the community. A multidisciplinary team of nurses, social workers, therapists and home health aides provide services to people over 18, who qualify for these services and have or are eligible for Medicare and Medicaid. VNS Choice Managed Long Term Care (MLTC): VNS Choice is a program for frail older adults who wish to remain at home but require ongoing assistance. The program includes home, community, facility-based long term care and health related services. Members keep their own doctors and work with a multidisciplinary team that fosters independence.
