Information on Cattolica Health Insurance Plan Società Cattolica Assicurazioni Lot 1 CIG 4520767EEE Sub-lot 1b (Foreign Bursary holders and Foreign Researchers) Accident Insurance Policy n. 2106/31/300030 Health Insurance Policy n. 2106/30/300002 Broker GPA S.p.A. For the sole period of the bursary granted by the Italian government, foreign bursary holders are covered by an insurance policy that covers expenses, if any, incurred as a result of illness and/or accident during their stay in Italy. The insurance policy will provide cover only for the bursary duration. The Convention between the Ministry of Foreign Affairs and Cattolica - Società Cattolica Assicurazioni (or Society) for bursary holders includes the following clauses: a) Direct payment to the Italian National Health Service network, (Servizio Sanitario Nazionale) or facilities acknowledged by it, in case of hospitalization expenses due to urgent medical reasons, maternity, accident or illness and any subsequent and related surgery procedures. In these cases, besides doctors fees and hospital fees, the insurance covers all expenses related to surgery and treatment during hospitalisation. In case of surgery, the insurance also covers the costs of room and board in a hospital or if a bed in a hospital is not available a similar hosting facility. Expenses connected with transportation to hospital are also covered for the patient and one caregiver. In such cases the maximum daily coverage is 65.00 Euros, with a yearly maximum of 650,00 Euros. b) Reimbursement of expenses for diagnosis (including doctors fees) for payments made up to 120 days before hospitalization, if such expenses are directly related to the illness or accident that required treatment. c) Reimbursement of expenses for medical assistance, exams, medications, physiotherapy and rehabilitation treatments including buying and/or renting health and prosthetic instruments (i.e. wheelchairs), for payments made up to 120 days following the last day of hospitalization, providing they are directly caused by the illness or accident that required treatment (up to a maximum of 100.000,00 Euros for each insured person for each year of coverage). d) Reimbursement of medical expenses and visits, specialist visits both at home or at a hospital, if related to illnesses and/or accidents covered by the insurance, up to the yearly maximum of 5.000,00 Euro per insured person. In such cases coverage includes: - analysis, diagnostic check-ups and laboratory exams; - physiotherapy and rehabilitation treatment; - medication expenses including those related to vaccination and prophylaxis that cannot be performer prior to the starting date of the specific period of coverage; - dental and parodontal care and extractions, caused by diseases, with the exclusion of dentures. e) The payment of an indemnity of 100.000,00 in case of accidental death during the bursary holder s stay in Italy.

f) In case of illness or accident related death of the insured, the Society reimburses the expenses for the body repatriation up to a maximum of 20.000,00 to those who are entitled. g) The payment of an indemnity of 120.000,00 in case of permanent disability caused by accident or illness. How to do in case of accident or illness In case of accident or illness the bursary holder must send a written statement within the deadline of 30 days from the event to: - GPA SPA Via Melchiorre Gioia 124 20125 Milano. The envelope, with the statement and the doctor s certificate, must be addressed to Silvana Costantini. The written statement must be attached to a doctor s certificate and state the place, day and time as well as the causes of the accident. A copy of all this documents have to be sent to: DGSP Ufficio VII Borse di studio Piazzale della Farnesina, 1 00135 Roma (RM) Medical Expenses In order to obtain the reimbursement of the MEDICAL expenses related to hospital stays and/or treatment caused by accident or illness, the insured person must apply within the deadline of 90 days from the last day of treatment. The request must be attached to a medical prescription with a diagnosis of the disease that has caused it; other required documents are originals of medical records, receipts, invoices, fee notes, and bills. Reimbursement applications submitted further to an accident must be attached to a certificate issued by an Emergency Room or by a similar unit or by a medical prescription stating the injury sustained by the victim. The Society will reimburse the expenses, when treatment is complete, within 30 days from receipt of the above mentioned original documents. Reimbursement will be paid in Italy, in Euro, into the bursary holder s bank account. If the Society makes available its own network of private clinics covered by a convention, the insurance company will pay in advance all expenses for the insured person. In order to obtain the reimbursement of the expenses for medicines, specialists visits, treatment related expenses, X-rays, medical exams and tests, tooth extractions and dental care (with the exclusion of dentures) the bursary holder must submit the following original documents: - medical prescription with diagnosis of the disease, including a copy of the patient s medical records;

- photocopy of the prescription and of the doctor s bill, if any; - originals of the chemists receipts; - copy of the award letter; stating his given name, family name, address and telephone number. Originals must be sent to GPA S.p.A and copies to the Ministry of Foreign Affairs, DGSP Ufficio VII to the above mentioned addresses. The yearly limit for specialist treatment at a surgery or patient s home is of 5.000,00 Euros per person. Insurance coverage for bursary holders starts when the insured person arrives in Italy and ends when he/she leaves the country to return to his homeland at the end of the course of study for which he was authorized to stay in Italy.
